RIDING AS A PASSENGER IN A PICK UP TRUCK. INJURED AND CO WORKER WERE REFUELING A LIGHT PLANT ON THE REFUSE PILE. THE DRIVER OF THE TRUCK CROSSED THE PATH OF THE PAN AND THE PAN STRUCK THE TRUC K. INJURED RECEIVED A SEVERE CONTUSION ON THE RIGHT LEG.

OPERATING A BATTERY POWERED CAR WHILE TAKING PARTS UP A RAMP TO THE #6 AIRSHAFT ELEVATOR. THE CART MALFUNCTIONED AND DRIFTED BACKWARDS. HE CAUGHT HIS LEFT THUMB BETWEEN THE STEERING HANDLE A ND A STRUCTUAL SUPPORT BEAM CAUSING A LACERATION REQUIRING 12 SUTURES AND A FRACTURE.
INJ WAS INSIDE A 6 TON MOTOR WHILE STANDING OUTSIDE THE OPERATORS COMPARTMENT TO SET THE PARKINGBRAKE. THE MOTOR WAS MOVING, PULLED BY GRAVITY DOWN A GRADE. HE CAUGHT HIS LEFT FOOT BETWEEN THE TOP OF THE RAIL & THE BOTTOM OF THE MOTOR
RIGHT FOOT WAS CAUGHT BETWEEN A CONVEYOR BELT TAKE-UP CARRIAGE AND ITS FRAME WHEN CARRIAGE ROLLED AFTER POWER TO UNIT WAS REMOVED. HE HAD HIS RIGHT FOOT RESTING ON THE CARRIAGE FRAME WHILE HE WAS SPOTTING A BAD BELT SPLICE.
DIRECTING SCOOP OPERATOR WHO WAS LOADING ROOK BOLTER CHANNELS WITH BLADE OF SCOOP. A BENT CHANNEL CAUGHT ON THE MINE ROOF WHILE SCOOP WAS BACKING UP. IT CAME COMPLETELY OUT OF THE BUNDLE AND S TRUCK THE INJURED ON THE UPPER RIGHT ARM LACERAITING IT. THE CUT REQUIRED THREE SUTURES.
INJURED WAS DRIVING A VOLVO ARTICULATING DUMD TRUCK DOWN A GRADE FROM THE REFUSE PILE, FOR AN UNEXPLAINED REASON THE TRUCK STRUCK THE BERM, WENTACROSS THE ROAD AND STRUCK A DITCH THEN TRAVELED UP A BANK OF DIRT. INJUURED WAS JOSTLED IN THE CAB AND RECEIVED A LACERATION ON THE RIGHT SIDE OF HIS CHIN THAT REQUIRED SIX STITCHES.
OBSERVING MINER CABLE AS IT WAS BEING PULLED OFFCM WITH SCOOP. A LOOP OF CABLE CAUGHT ON THE CM.AS THE LOOP WAS BEING PULLED HE FEET BECAME ENTANGLED AND HE FELL TO THE MINE FLOOR STRIKING HS UPPER RIGH RIBS ON THE BOTTOM.
CLEARING DEBRIS FROM A FLOOR DRAIN WITH A SHOVEL. THE BLADE OF THE SHOVEL CONTACTED A MOVING CONVEYOR BEL, CAUSING HIS LEFT HAND TO STRIKE AGAINST THE BELT STRUCTURE, THEN THE SHOVEL BLADE TO STRIKE AGAINST HIS HAND. INJURED RECEIVED FIVE SUTURES ON THE LEFT HAND. MEDICAL TREATMENT ONLY.
SHOVELING UNDER LOW BELT STRUCTURE. THE MOVING CONVEYOR BELT HAD A SLICE THROUGH IT AND A FLAP HANGING FROM IT. THE SHOVEL HANDLE WAS STRUCK BY THE DANGLING FLAPPER CAUSING HIM TO STRIKE HIS L EFT HAND AND WRIST AGAINST THE MOVING BELT, RESULTING IN A SEVERE CONTUSION TO TUESE AREAS OF HIS BODY.
HANGING HOOKS FOR A PULL CORD ABOVE CONVEYOR BELT. WHILE STANDING ON BELT IT MOVED, HE FELL AND STRUCK HIS RIGHT KNEE ON A BELT ROLLER. INJURED WORKED HIS NORMAL JOB WITHOUT RESTRICTIONS UNTIL AFTERNOON SHIFT MAY 27, 1998 WHEN HE STARTED RESRICTED WORK DUTY.
TRAMMING SHUTTLE CAR FROM FACE TO BELT TAIL PIECE. SHUTTLE CAR WAS DRIVEN OVER A ROLL IN THE MINE FLOOR. HE STRUCK HIS HEAD AGAINST THE SHUTTLE CAR CANOPY.
OPERATIN 5 TON MOTOR. AS INJURED REACHED TO THROW BLOCK LIGHT SWITCH HE STRUCK HIS RIGHT HAND AGAINST THE SWITH WAN AND RECEIVE A SEVERE CONTUSION.
INJURED STATES THAT HIS BACK BEGAN TO HURT FROM OPERATING A SCOOP. HE BEGAN RESTRICTED WORK ON 7-7-97.
LOADING CABLE IN A FEEDER THAT WAS BEING TRAMMED. FEEDER WENT OVER A ROLLIN BOTTOM SHICH CAUSED IT TO DROP SUDDENLY. LEFT FOOT WAS CAUGHT BETWEEN THE FEEDER BUMPER AND THE MINE FLOOR. INJUREDS FOOT WAS FRACTURED.
OPERATING A 5 TON MOTOR. HIS LEFT FOOT WAS POSITIONED ON THE LOWER PART OF THE OPERATING COMPARTMENT. HIS LEFT FOOT STRUCK AGAINST A PIECE OF COAL, 1-1/2' THICK X 2' WIDE X 3' LONG. THIS RESUL TED IN A COMPOUND FRACTURE TO HIS LEFT FOOT.
EE WAS RIDING IN THE PASSENGER COMPARTMENT OF A MANTRIP. THE MANTRIP BOUNCHED WHEN IT HIT A DIP IN THE ROAD AND EE FELT PAIN IN HIS LOWER BACK.
INJURED WAS OPERATING A VOLVO TRUCK WHILE HAULING MATERIAL TO A CLEAN COAL STOCKPILE. ROAD SURFACE WAS UNEVEN AND FROZEN. COMPLAINED OF A SORE LOWER BACK.
VICTIM WAS INSTRUCTING A SCOOP OPERATOR WHO WAS LOADING THE BASE OF A HEAD ROLLER ONTO A FLAT TRUCK. AS THE SCOOP OPERATOR BACKED AWAY FROM THE BASE, THE PANCAKE BLADE INADVERTENTLY CAUGHT TH E BASE AND IT PIVOTED ON THE BED OF THE FLAT TRUCK. INJURED WAS CAUGHT BETWEEN THE FLAT TRUCK AND THE BASE OF THE BELT HEAD ROLLER.
INJURED WAS STRUCK ON THE RIGHT UPPER ARM BY A BELT TAILPIECE THAT BROKE LOOSE UNDER TENSION WHEN A 5/8" CHAIN BROKE AND THE TAILPIECE TRAVELED OUTBY FOR A DISTANCE OF 10 FEET.
TRAMMING A SCOOP AT NORMAL SPEED RAN OVER A WOODEN CRIB BLOCK IN ROADWAY, BOUNCED UP AND STRUCK HEAD AGAINST CANOPY. INJURED STRAINED NECK, FINISHED SHIFT.
THE MANTRIP INJURED WAS A PASSENGER IN DERAILED WHEN IT STRUCK AN UNEVEN RAIL JOINT INJURED WAS JOSLED IN THE PASSENGER COMPARTMENT & EITHER STRUCK AGAINST SOMETHING UNIDENTIFIED SUCH AS A COW ORKERS KNEE OR WAS FORCED IN A POSITION WHICH CAUAWS RHW 7RH & 9TH RIBS (LEFT SIDE) TO BE FRACTURED
INJURED WAS OPERATING A SHUTTLECAR THAT WAS BEING USED TO PULL CONTINUOUS MINER CABLE THERE WAS A ROLL IN THE MINE BOTTOM AS THE SHUTTLE CAR TRAVELED OVER THE ROLL INJURED WAS BOUNCED UPWARD & S TRUCK HIS HEAD ON THE CANOPY OF THE SHUTTLECAR STRAINING HIS NECK VICTIM WAS RELEASED TO RESTRIC TED WORK
INJURED WAS TRAMMING A 486 S&S SCOOP IN A DIRECTION OUTBY THE FACE. THERE WERE 12 THIN ROOF MATS STACKED ON THE SCOOP. AS THE SCOOP BEGAN TO PASS THROUGH A RUN THROUGH CHECK THEY WERE DISLODGE D AND 8 OF THEM ENTERED THE OPERATOR CAB STRIKING THE INJURED ON THE RIGHT SIDE OF THE NECK CAUSING A LACERATION REQUIRING SUTURES NO LOST TIME.
TURNING AROUND IN OPERATOR'S COMPARTMENT OF 5 TON MOTOR AND STRUCK RIGHT ELBOW ON BATTERY LIDS. DIAGNOSED AS BURSITIS.
INJURED CLAIMS THAT WHILE A PASSENGER INA 5 TON MOTOR HIS LWOER BACK WA SINJURED WHEN THE MOTOR RAN INTO ANOTHER MANTRI PARKED ON THE HAULAGE.
EE WAS STRUCK BY A SHUTTLECAR CABLE AND KNOCKED TO THE GROUND WHEN THE CABLE, WHICH WAS HUNG ACROSS THE ENTRY ON HOOKS THAT WERE NOT FASTENED, WAS WOUND UP ON THE SHUTTLECAR REEL. THE CABLE HA D BEEN HUNG TO PERMIT A SCOOP TO TRAVEL THROUGH THE ENTRY BYT WASN'T DROPPED.
INJURED HAD JUST BACKED SHUTTLE CAR AWAY FROM BEHIND CONTINUOUS MINER CONTINUOUS MINER HELPER MOTIONED FOR INJURED TO ADVANCE TO PROCEED WITH MINING INJUR ED WAS SEEN SITTING IN THE OPERA TORS CAB WITH MINE HAT OFF WHEN APPROACMED INJUR ED CLAIMED BEING JUSTLED & LATER CLAIMED STRIKING HEAD ON CANOPY
INJURED WAS OPERATING A VOLVO TRUCK HAULING REFUSE ON THE PILE.WHILE TRAVELING DOWN THE RAMP THE TRUCK BEGAN TO SLIDE ON THE ROADWAY.WHEN INJURED HIT BRAKE RETARDER TO SLOW THE TRUCK DOWN TRUC K STRUCK RIGHT BERM TEN LEFT BANK.THE 30 TON TRUCK ROLLED OVER ON RIGHT SIDE INJURYING OPERATORS NECK.
INJURED WAS GUIDING SCOOP OPERATOR SO SCOOP WOULD NOT CATCH DISCHARGE LINE WHILE SCOOPING. SCOOPCAME TOO CLOSE & INJURED JUMPED TO THE SIDE OUT OF THE WAY CAUSING PAIN IN INJURED'S LEFT LEG & HIP.
INJURED WAS TRAMMING A 486 SCOOP ON THE HAULAGE. THE SCOOP TIRES FELL OFF THE RAIL JOSTLING INJURED ON SEAT.
INJURED WAS RIDING IN OPERATIONS COMPARTMENT OF "DOCKBILL" AS A PASSENGER OPERATOR OF DUCKBILL. WENT THRU A SWITCH AND DID NOT KNOW OF 10-TON MOTOR COMMIT FROM OTHER DIRECTION.
INJURED WAS OEPRATING SHUTTLE CAR AND AS HE TRAVELLED OVER ROUGH ROADWAY HE WAS JOSTLED AND HIT HIS HEAD ON THE OPERATORS CANOPY.
INJURED WAS HOLDING UP CANVAS CHECK FROM SCOOP TO GO THROUGH CROSSCUT. THE SCOOP RAN OVER EDGE OF HIS TOE AND INJURED JUMPED OUT OF THE WAY AND FELL TWISTING HIS LEFT KNEE.
EMPLOYEE WAS LEAVING #3 BOTTOM AS A PASSENGER IN A 5-TON MOTOR. AS THE MOTOR CAME INTO THE PARALLEL SWITCH AT THE #3 BOTTOM THE OPERATOR COULD NOT STOP AND RAMMED INTO ANOTHER 5-TON MOTOR.
