THE EE WAS ASSISTING IN UNCOUPLING HYDRAULIC LINES FROM A CABLE REELER ATTACHED TO A CAT LOADER.IN THE PROCESS OF RELEASING THE STORED PRESSURE OF THE HYDRAULIC LINES, THE LOADER BOOM ATTACHME NT SHIFTED AND PINCHED THE EE'S FOOT BETWEEN THE BOOM AND GROUND.
THE INCIDENT OCCURRED WHEN AN O&K DIRT SHOVEL WAS LOADING A LARGE ROCK INTO THE BED OF A 190 TON CAT HAUL TRUCK. THE SHOVEL OPERATOR PICKED THE ROCK UP WITH THE SHOVEL BUCKET & PLACED THE BUCK ET OVER THE TRUCK BED. THE OPERATOR THEN CURLED THE BUCKET & THE ROCK FELL ONTO THE DUCKBILL OF THE HAUL TRUCK. THE TRUCK OPERATOR'S NECK BECAME SORE LATER, DIAGNOSED AS NECK STRAIN.
AN OILER WAS DRIVING A LUBE TRUCK DOWN A PIT ACCESS ROAD. THE TRUCK SLID DOWN A PIT ACCESS ROAD.THE TRUCK SLID SIDEWAYS, IMPACTED A BERM WITHT HE FRONT BUMPER AND TIPPED ONTO THE PASSENGERS SI DE OF THE TRUCK. EE WAS DIAGNOSED WITH SLIGHT KNEE STRAIN AND PLACED ON RESTRICTED DUTY.
EE WAS OPERATING LOADER WHEN HE FELT DUST OR SOMETHING BLOW IN HIS EYE. EYE CONTINUED TO BE IRRITATED. SENT TO HOSPITAL. HOSPITAL WASHED EYE OUT. FOUND EYE IRRITATED AND SCRATCHED. NO OBJECT F OUND IN EYE. PHYSICIAN PRESCRIBED ANTIBIOTIC OINTMENT. RETURNED TO WORK FULL CAPACITY SAME DAY.
