Employee was operating 4 man diesel ride in reverse. Employee struck head on high voltage cable, panicked and instead of applying brake the employee floored the accelerator causing injury to neck.

Mantrip was leaving #1 unit at end of shift. Employee stated allowed right foot to extend partially out of mantrip. Mantrip turned at intersection employee's big toe was injured when right foot came in contact with coal rib.

Employee was operating shuttle car #34 on the #3 unit. The employee turned too early in the shuttle car causing the shuttle car to turn into the rib instead of intersection between entries 7 & 8. Restricted duty as of 6/11/18.

IE was repairing bad splice on shuttle car in #5 entry. EE left for 10-15 min, returned & began working on splice. EE had not seen scoop in the area. With cap light & blinking light on, EE was on knees looking down. Scoop carrying supplies in elevated bucket was heading inby in #5 entry at walking speed. Scoop operator rang bell before & while passing through the airlock curtain.

Employee was opening equipment doors when ee Getman tractor rolled into ee lower legs resulting in fractures to both legs.

Employee was traveling back to the shop on 683 diesel ride. Along the 1SW Primary road at approximately crosscut 30 a roof bolt lying in the travelway was kicked up by the ride. It struck employee in the nose resulting in a fractured nose and multiple sutures.

Employee was pulling up slack of the 5/8 belt sheave rope with a 5/5 belt chain with the back of the cab of #4 diesel scoop. The front tire ran over the belt chain slack and it swung around striking the employee above the right eye, resulting in 7 sutures.

Miner was unhooking trailer from scoop bucket when shuttle car struck trailer causing it to roll into scoop bucket crushing lower left leg.

Driving outby, ride ran over rock, swinging ride into rib. Hit left forearm against the rib resulting in a slight fracture and surgery on 1/5/13.

Employee was riding down 6th 54 supply road on mantrip 387 when his hardhat fell off. As he replaced his hardhat, his left ring finger was pinched between his hat and the roof, fracturing his finger.

Bolter was hung up in mud in #4 entry, scoop was pulling bolter out and employee had his hand on top of his canopy. His hand was mashed between canopy and the top.

Employee was sitting on the ground in an intersection working on the miner cats when a scoop came through a curtain out-by, struck their ride that was out-by the miner, pushing it into the injured man pinning him between the ride and the miner.

While operating a battery scoop, there were some chains in the operators compartment and one of the chain ends fell through a hole in the operators compartment and was run over by the wheel and as the chain jerked the hook end struck the employee on the index finger of the left hand. He received 6 sutures.

Injured was riding out in mantrip with his left foot outside the trip. As mantrip went around a supply sled his foot caught between the mantrip and the sled bending it back.

Employee stated he was riding out of the mine in an open mantrip, tilted his head back to take a drink from a water bottle when the bottle struck the roof and shoved his safety glasses back into his forehead causing a laceration.

While riding on an 8 passenger mantrip, the mantrip driver encountered cribs on his side of the travelway and overcorrected and hit the right rib causing the employee to become smashed between two other passengers.

Employee was the last of 3 rides going into the mine through a second set of airlock doors when the front ride hit the close switch on the doors. His left leg was pinched between the doors and the ride he was on. He sustained contusions and abrasions to the left leg.

After zoning in at the top of #9 slope, we started down the slope and approximately 1/4 way down it felt like the brakes went out on the 8 man mantrip. We made the first turn to the right on the bottom and was not able to make the left turn and we ran into the coal rib. Other EE was the operator of the 8 man mantrip.

Employee strained his back while throwing the miner cable over next to the rib. Later in the shift while operating a shuttle car, the shuttle car hit a hole in the road and jammed the employee's back even worse.

Employee was riding in a mantrip and strained his back when the mantrip bumped a coal rib as it was being turned around.

While being assisted by a 2-man ride down the #9 seam slope, the golf cart lost its brake and the front of the golf cart struck the right side of the slope. The 2-man ride stopped and the golf cart turned slightly sideways, slamming into the back of the 2-man ride pinning the employee in the golf cart seat causing a laceration, fractured pelvis and two fractures to the vertebra.

While hanging a line curtain in the #1 entry the employee was slightly bumped with the battery end of a battery scoop causing a contusion to the right leg and hip.

While tramming a Getman Tractor on the supply road employee hit a pot hole and injured his back. This incident did not become lost time until 11/05/2007.

A diesel scoop was traveling inby, at the same time a 3-man tractor was pointed outby and parked against the left rib. As the scoop passed the tractor, the bucket of the scoop hit the right leg and left foot of the tractor operator. The tractor operator sustained 4 broken bones to the left foot and a long laceration on the inside of the left foot.

Employee was operating a diesel tractor and struck his head on an overcast. He sustained a teardrop fracture to one of the vertebra in his neck.

Employee was operating a scoop and ran over a piece of rock that was on the roadway, causing him to strain his lower back. This did not become a Lost Time Accident until 6/1/2007.

After loading a shuttle car, the miner operator went behind the miner to line up the sights when the shuttle car, after failing to negotiate a turn, backed up resulting in the miner operator being pinned between the shuttle car bumper and the continuous miner boom. The miner man received muscle and tissue damage to the right leg, hip, back and abdomen.

Employee was re-bolting on the 2 Main North supply road (#6 entry) at crosscut #24 when a diesel scoop with a duster attached to the bucket struck employee on the lower part of the right leg m ashing it between a metal brace on the duster and a metal bracket on the bolter. The leg was partially severed between the ankle and knee. Doctors made the decision to amputate completely.

Employee was tramming a shuttle car back and forth from the miner to the feeder when the car trammed over a bump in the road causing an abdominal hernia to surface. This employee had two prior hernia surgeries. The doctor repaired an additional small hernia that was found and not treated when the employee had gall bladder surgery in January 2006.

While scooping #7 entry employee reached for a piece of curtain to throw it out of the way when his knee hit the steering lever and the scoop swiveled pinning his left wrist against the rib. Employee suffered two broken bones in his left wrist. This accident became lost time as of 4/11/2006.

While building a brattice the shuttle car operator bumped the brattice knocking some block on the employee. This accident did not become lost time until 03/28/2006.

Employee was reaching into his back pocket as he was riding in a mantrip when his left foot slipped outside the mantrip causing his foot to be mashed between the mantrip and the coal rib. He sustained a fracture to the foot.

Employee was tramming a shuttle car in the #7 entry on the #1 unit, heading in the direction of the continuous miner, when the shuttle car trammed over a piece of rock in the roadway causing him to strike his head on the cars canopy. This resulted in the employee sustaining a strain/sprain injury to his neck.

Employee was driving a 2-man diesel ride down the supply road (#6 entry) of the 4 North Panel and ran over a piece of rock. This caused the ride to elevate striking his head on a roof bolt plate cutting his head in three different locations. He received a total of 31 sutures.

Employee was tramming a battery scoop on the #2 unit and he had his left hand through the hand hold, on the canopy, that is used to assist someone getting in and out of the scoop. When he turned toward a cross-cut his hand became caught between the mine roof and the top of the scoop canopy resulting in lacerations to his index and middle finger. He received a total of 7 sutures.

EMPLOYEE WAS CRUSHED BETWEEN A COAL RIB AND A BATTERY POWERED ROCKDUSTER BEING PULLED BY A BATTERY POWERED SCOOP RESULTING IN A BROKEN LEFT SHOULDER BLADE AND BROKEN LEFT ARM. HE ALSO RECEIVED MULTIPLE CONTUSIONS.

EE WAS UNLOADING TOOLS FROM ONE RIDE TO ANOTHER WHEN A SHUTTLE CAR PULLED IN THE X-CUT, STRIKING ONE RIDE & CAUSING IT TO MOVE FORWARD CATCHING HIS LEG BETWEEN THE TWO RIDES.

EMPLOYEE WAS RIDING ON A 2 MAN DIESEL RIDE WHEN HE RAN OVER A ROOF BOLT LAYING IN THE ROAD CAUSING THE BOLT TO FLY UP & HIT HIM ACROSS THE NOSE.

EE WAS RIDING IN A 4 MAN DIESEL MANTRIP & WHEN THEY STARTED DOWN THE UNDERGROUND SLOPE THE BRAKES FAILED CAUSING THE RIDE TO PICK UP SPEED UNTIL IT HIT A COAL RIB ON THE SLOPE BOTTOM, THROWIN G EE FROM THE RIDE DISLOCATING HIS RIGHT HIP/LEG

EE WAS DRIVING A 4 MAN DIESEL MANTRIP WHEN THE BREAKS FAILED GOING DOWN THE SLOPE CAUSING THE RIDE TO PICK UP SPEED UNTIL IT STRUCK A COAL RIBON THE SLOPE BOTTOM THROWING HIM OFF OF THE RIDE

RIDING IN MANTRIP AND HIT ROUGH BUMPS AND FELT PAIN IN LOWER BACK. DID NOT BECOME REPORTABLE UNTIL 6-25-01.

USING A DIESEL LO-TRAC TO HELP INSTALL STEEL ARCHES WHEN AN ARCH CAME LOOSE STRIKING HIM ON THE HEAD.
