ONE OF HIS PANT LEGS AT THE BOTT
INJURED WAS DRIVING UP A RAMP WITH A LOAD OF ROCKS WHEN THE ENGINE STALLED OUT CAUSING THE TRUCKTO ROLL BACKWARDS. THE LEFT REAR WHEELS CAME UPON A SPOIL TILTING THE TRUCK BED AND ROLLED IT OV ER ON IT'S RIGHT SIDE.
THE SCRAPER WAS GOING A STEP GRADE AND CAME UPON A 2 SLOW MOVING EQUIPMENT. THE OPERATOR TRIED TO STOP THE SCRAPER BY ALL MECHANICAL MEANS BUT THE MOMENTUM OF THE EQUIPMENT CAUSED IT TO SLIDE INTO THE LEFT REAR TIRE OF OTHER SCRAPER THAT WAS TRAVELING IN FRONT OF IT.
INJURED WAS ATTEMPTING TO DISCONNECT A TON CABLE FROM TOW-HOOK WHEN HIS LEFT FINER WAS CAUGHT BETWEEN THE CABLE AND THE SCRAPER FRAME WHEN THE MACHNE INADVERTENTLY TURNED RIGHT.
INJURED THINKS SHE BACKED OVER A WIND ROW AND WAS SHAKEN UP. SHE DOESN'T REALLY KNOW WHEN IT HAPPENED. SHE SAYS SHE WAS WEARING A SEAT BELT.
INJURED SAID HE WAS DRIVING A 2.5 TON MECHANIC TRUCK AND WAS TAKING A RUN AT AN INCLINE. WHEN HE GOT TO THE TOP, THE TRUCK TIRE HIT A POT HOLE IN ROAD, CAUSING TRUCK TO BE JARRED. HE FELT HE MUST HAVE STRAINED HIS LOWER BACK AT THIS TIME.
THE INJURED WAS IN PROCESS OF COMING ONTO THE TRUCK HOPPER TO DUMP HIS LOAD OF COAL WHEN HIS RIGHT FRONT TRUCK TIRE HIT THE GUARD RAILING, JERKING THE TRUCK TO THE LEFT CAUSING THE OPERATOR TO BE THROWN AGAINST THE SHIFT TOWER, & BUMP HIS LOWER BACK CAUSING SEVER LOWER BACK PAIN. HE ALSO BUMPED HIS HEAD AGIANST THE CAB WINDSHIELD'S METAL FRAME CAUSING A BRUISE & BUMP.
INJURED WAS PASENGER IN A PRIVATE TRUCK THAT WAS INVOLVED IN A COLLISION ACCIDENT WITH ANOTHER PRIVATE VEHICLE. ACCIDENT OCCURRED OFF MINE PROPERTY.
MECHANIC WAS DRIVING A MECHANIC TRUCK WHEN HE REACHED THE TOP OF THE RAMP ON THE BENCH TURNED LEFT & FRONT WHEELS OF TRUCK HIT A DRAINAGE DITCH CAUSING TRUCK TO BE JARRED & BOUNCED MECHANIC ST RUCK CAB OF ROOF & FELL BACK DOWN ON SEAT SAFETY SEAT BELT WAS IN USE
INJURED CLAIMS HE WAS BACKING UP HIS END DUMP TO DUMP HIS LOAD. WHEN END DUMP #453 BACKED INTO HIS TRUCK, IT CAUGHT HIM BEFORE HE COULD REACT. TRUCK #453 CONTINUED BACKING UP, PUSHING THE FRON T END PORTION OF THE OPERATOR'S CAB UP AGAINST HIS LEGS. THE TRUCK CONSOLE UNIT CAUGHT HIS KNEE AGAINST THE STEERING COLUMN, CAUSING A BRUISE TO BOTH SIDES OF HIS LEFT KNEE.
THE INJURED & TWO OTHER MEN WERE STANDING ON TOP OF THE FEEDER BELT INSIDE A FEEDER-CHUTE. THE CHUTE IS LOCATED UNDER THE J-28 DONE (CONE SHAPED COAL STORAGE) THAT WAS EMPTY. WHILE CHECKING FO R POSSIBLE LOOSE COAL BEFORE COMMENCING WORK, THE WORKERS THOUGHT THEY HEARD SOME COMING DOWN-MADE NASTY EXIT ON HAND & KNEES-INJURED SLIDE OFF EDGE OF FEEDER BELT DOWN APPROX. 20' CHUTE HEAD
WHILE THE INJURED WAS REMOVING ATRANSMISSION FROM A150 TON RIMPULL HAUL TRUCK. A CHAIN USED TO HOLD THE TRANSMISSION BROKE. CATCHING THE MAN'S RIGHT HAND BETWEEN PRY BAR AND TAIL SHAFT OF TR ANSMISSION.
WHILE DRIVING UP TOT HE LOADER, TURNIGN AROUND IN AND END DUMP TRUCK. FRONT WHEEL ROLLED OVER A BOULDER, CAUSING STEERING WHEEL TO TURN VIOLENTLY WHEN MAN STILL HOLDING ON WITH ONE HAND.
EMPL PUTTING AUGER TUBE BACK IN THE RACK.LEVER CONTROL HANDLES LOCATED DIRECTLY UNDER TUBE.PUSHED CONTROL LEVER IN WRONG DIRECTION AND AUGER TUMBE CAME AND PINCHED THUMB BETWEEN AUGER TUBE AND CONTROL LEVER.
THE INJURED AND HIS HELPER WERE USING A FORKLIFT WITH AMANBASKET ATTACHMENT, CHECKING FOR POWER SOURCE. THE BASKET WAS HOISTED 8 FEET OFF THE FLOOR WHEN IT WOULDN'T HOIST DOWN. THE OPERATOR HAD THE CONTROL LEVER THE DOWN POSITION LONG ENOUGH TO RELEASE ALL OF THE HYDRAULIC PRESSURE. THE SHIFTING OF THE WEIGHT CAUSED THE MANBASKET TO UNBOUND AND DROP TO THE FLOOR.

HE WAS GUIDING A HAUL DUMP DOOR BY HOLDING IT WITH HIS HANDS. THE DOOR WAS HANGING FROM A FORK LIFT BY SLINGS. AS THE DOOR SWUNG, IT HIT A JACK CAUSING THE INJURY BY PINCHING THE PINKY FING ER OF THE LEFT HAND BETWEEN THE DUMP DOOR AND THE JACK HANDLE. HE WAS WEARING GLOVES. AN ANVULSION OCCURED REQUIRING STITCHES.
EE WAS CHECKING AND OIL PUMP FOR OPERATON, THE COVER WAS REMOVED EXPOSING THE ROCKER VALVE ASSEMBLY. WHEN EE COUPLED UP THE HOSE, THE PUMP STARTED OPERATING. THE VALVE ROCKER MOVED AND SMASHED HIS RIGHT PINKY FINGER.

INJURED WAS OPERATING PAN SCRAPER AND WAS COMING DOWN A 4 TO 5% INCLINE AT A FASTER SPEED THAN NORMAL CAME UPON RECENTRLY WATERED DOWN ROAD LOST CONTROL OF SCRAPER IN SKID MACHINE TURNED OVER. INJURED WAS HAULED A LOAD OF DIRT AND WAS USINGHIS SEAT BELT.
INJURED WAS BACKING UP THE END DUMP TRUCK WHEN ONE OF THE FRONT TIRES HIT A POT HOLE CAUSING THE STEERING WHELL TO TURN WHICH CAUGHT HER LOWER ARM AND ROTATED WITH IT.

AS HE WAS RAISING BURK TEETH CAUGHT WEAR PLATE IN BED & LIFTED WHOLE TRUCK-
VICTIM WAS HITT3NG FROZEN COAL ONTO A MOVING VONXX CONVEYOR BELT WAS SCRAPER WITH HANDLE TAIL END OF BELT HIS TOOL GOT CAUGHT BETWEEN THE BELT AND CHUTE CAUSING TOOL TO PULL HIS HAND AGAINST AN I BEAM. 3
