EE was operating a diesel getman on the supply road. EE encountered a loose concrete block on the blind side, which caused EE to be bounced in the seat. As EE bounced, they felt pain in their back. EE was given a full release to return to work without restrictions. On 05-08-18, EE had an appointment and the MRI revealed two herniated discs, EE was placed off work at this time.

As EE bounced, EE struck EE's head against the canopy of the mantrip, causing pain to EE's neck. EE continued working without restrictions until EE was taken off work on 04-04-18, at this time EE began missing time.
Employee was operating a shuttle car, while tramming they encountered uneven bottom floor, bouncing them in the seat and causing pain to their neck. EE continued working without restrictions performing normal occupation until taken off work on 03/20/2019.
EE was riding as the passenger on a golf cart. EE's left foot was under the right leg and when EE's toe caught on a piece of loose coal, the left foot pulled back, causing a strain. EE received a full release and continued working performing normal occupation until the surgery date of 01/03/2019.
The continuous miner "CM" on the unit was down and in need of mechanical repairs and the CM operator trammed outby the last open in preparations for the mechanic. EE was driving EE's maintenance ride to the CM to make these repairs and EE pulled through a check curtain striking the CM with EE's ride. EE began missing time on 2/7/17 for a bruised/sprained ankle.
Employee reported an alleged head injury that was causing him pain to his head/neck. He was seen at a medical facility, given a return to work with no restrictions. However, he was prescribed medication with known side effects that could have rendered him a potential hazard to himself and those who work around him. The MRI results confirmed regular duties without restriction.
Employee was assisting in advancing the high voltage cable for a unit's belt extension. The cable was being transported on a high voltage trailer and was being set into position by a battery bucket jeep. The trailer shifted, causing his foot to be pinched between the trailer and the jeep bucket. He began missing time on 11-24-14.
Employee was installing a line curtain to the mine roof and as the coal hauler moved, its cable struck his leg. He began missing time on 9-4-14.
Employee was riding as a passenger in an open top mantrip. The front of the mantrip bumper struck some loose belt that was stacked to the side of the entry. The belt caught his left foot/ankle and twisted his foot/ankle as well as his left knee. He began missing work on 4/15/2013.

The shuttle car had to back up due to loose coal on the mine floor. As the car backed up it struck his right leg, causing a fracture. He started missing time on 10/17/2011.

During this time the injured employee stepped between the car and miner when the shuttle car backed up to try and free the tires. At this time the bumper of the car struck the injured employee's right leg between the bumper of the car and the bumper of the miner.
The employee was caring a hundred ton jack down a air course on a ride when the jack started to slide off the ride and he tried to catch it, when he caught his finger between a timber and the jack.
The employee was driving a diesel ride down the road when he ran over a rock that was lying in the road. It flipped up and hit him in the left knee. The employee continued to work and his knee became worse until he could not work anymore. The employee started missing work on 10-25-2010.
The employee was up in the face writing some notes down in his book when a shuttle car missed his turn going back to the feedeer and hit the employee that was up in the face. The employee's leg was broke in this accident.
EE was adjusting the forks on a forklift. He had one hand behind the forks and one hand on the end of the forks. He let go with the one hand on the end of the fork it dropped down and caught his fingers on the other hand behind the fork pinching the ends of the fingers. The EE received stitches in finger tips.
The employee was backing across the run with supplies in his bucket for the roof bolter, when he ran over something that threw him up in the air. The employee reached to grab something to brace himself. When he grabbed the outside edge of the canopy, the scoop hit the rib at the same time. The employee's pinky and thumb were broken and received several stitches.
The employee was backing a load of supplies into a crosscut, while looking over his shoulder he ran into a pin board that was hanging down, jamming his neck. He didn't start missing work till Dec. 14, 2009.

Employee was changing a shuttle car tire. As they raised the car with a scoop bucket, a pin board kicked out from under the scoop bucket hitting him in the mouth causing a laceration.
EMPLOYEE WAS TURNING A DIESEL MANTRIP AROUND IN A CROSS CUT. AS HE BACKED UP HIS RIGHT HAND WAS ON THE SIDE OF THE FENDER AND BECAME MASHED BETWEEN THE FENDER AND THE UNIT SAFETY SIDE, CAUSING A LACERATION TO THE BAK OF HIS HAND. HE REC'D 4 STITCHES.
The pinner was hung up in the bottom of a swag. The scoop backed down the hill to pull the pinner out. As the employee was hooking up the obain the scoop rolled back catching his leg between the bumper of the scoop and the atrs on the pinner causing a fracture.
EE WAS RIDING THE UNIT 3-WHEELER. AS THEY DROVE THRU A CURTAIN ON THE SUPPLY ROAD A 12'CABLE BOLT THAT WAS ON THE 3-WHEELER HUNG IN THE CURTAIN AND HIT THE EE ON FOREHEAD CAUSING A LACERATION .HE REC'D 7 STITCHES.
EE WAS WALKING ACROSS THE SECTION. A SHUTTLE CAR CABLE PULLED TIGHT AND LANDED ON HIS RIGHT FOOT. THE CABLE FELL IN BEHIND HIS EXTERNAL METATARSAL AND CAUSED HIM TO FALL TO THE GROUND. THE CABLE PULLED TIGHT AND CAUSED A STRAIN TO HIS LIGAMENTS AND TENDONS. HE STARTED MISSING WORK ON 4/21/05.
Employee was riding in the passenger seat of a diesel personnel carrier. The driver was passing a diesel duster in the supply road. the personnel carrier hit the duster causing the employee to fall forward hitting his face. His knee swelled up. The x-rays were negative but because of the bruise he missed work for seven days.
EE WAS TURNING AROUND IN SHUTTLE CAR AND PUT FOOT OUT ON THE MINE FLOOR. WHEN THE MINER OPERATOR RAN INTO THE SHUTTLE CAR, EE FELT A PAIN IN RIGHT KNEE. MRI CONDUCTED AT THE END OF THE MONTH SHOWED THE EE NEEDS SURGERY AT A LATER DATE.
EE WAS CLEANING RETURN ROOMS WITH A SCOOP JEEP WHEN HE RAN OVER A TIMBER. THIS CAUSED HIM TO BOUNCE UP IN THE CANOPY AND LAND, HURTING HIS BACK. HE WENT TO THE DOCTOR ON 7/27/04 AND STARTED PHYSICAL THERAPY. HE STARTED WORKING RESTRICTED DUTY ON 7/28/04. HE WENT BACK TO THE DOCTOR AND WAS RELEASED TO RETURN TO WORK ON 8/16/04.
Employee was driving a shuttle car on #4 unit when he hit a bump in the roadway causing him to bounce up and jam his neck against the canopy. He went to the Dr and received chiropractic adjustments.
Employee was riding on a diesel mantrip when he hit a hole in the roadway. His back started hurting from the impact. He went to the Dr and x-rays were negative. He went back to the Dr and started therapy on 4-26-04.
EMPLOYEE WAS CHANGING OUT AN IMPACT ROLLER IN THE TAILPIECE OF THE 3RD 54 BELT LINE. AS THEY WERE TRYING TO FREE UP THE ROLLER STAND, HE CUT HIS RIGHT THUMB. HE RECEIVED SUTURES FROM THE DOCTOR.
EMPLOYEE WAS DRIVING A RAM CAR WHEN HE HIT SOME HOLES IN THE ROADWAY, CAUSING HIM TO BOUNCE AND HIT THE CANOPY WITH HIS HEAD AND THEN LANDED ON HIS BACK. HIS BACK CONTINUED TO BOTHER HIM AND H E HAD AN MRI ON 2/20/04. THE RESULTS WERE POSITIVE FOR FLUID BUILD-UP BETWEEN DISC SPACES. HE IS CURRENTLY WORKING BUT SEES A SURGEON ON 3/17/04.
PERSON WAS STANDING #2 X-CUT WHEN SHUTTLE CAR DROVE BY THROWING OUT A PIECE OF ROCK STRIKING EE IN THE SHIN CAUSING A LACERATION. THE ROCK POPPED OUT FROM UNDER THE TIRE OF THE SHUTTLE CAR.
EMPLOYEE WAS RIDING IN THE BACK SEAT OF A 12-MAN OPEN MANTRIP. AS THE MANTRIP WENT THROUGHT THE AIR DOORS, THE EMPLOYEE RAISED UP ON HIS KNEES TO CLOSE THE DOORS. WHILE HE WAS CLOSING THE DOOR S HE HIT HIS HEAD ON THE AIR-DOOR SWITCH CAUSING A LACERATION TO HIS UPPER LIP AND A FRACTURE TO HIS NOSE AND JAW.
EE WAS DRIVING A SHUTTLE CAR WHEN HE HIT A HOLE IN THE ROADWAY CAUSING HIM TO BOUNCE AND LAND HURTING HIS BACK. HE CONTINUES TO WORK BUT THE PAIN WOULD NOT GO AWAY. HE IS NOW AWAITING SURGERY. HIS LAST DATE WORKED WAS 6-17-03.
EE WAS OPERATING A SHUTTLE CAR. AS HE DROVE TOWARD THE MINER HE HIT A SET OF HOLES IN THE ROADWAY. HE BOUNCED UP AND LANDED ON HIS RIGHT HIP, CAUSING PAIN TO HIS BACK AND HIP AREA. HE WENT AND RECEIVED CHIROPRACTIC ADJUSTMENTS ON 6-2-03.
EE WAS WORKING ON #1 UNITS NEW HEADER. AS HE WAS WORKING BESIDE THE 1ST 42, A ROCK CAME DOWN THE BELT STRIKING HIM IN THE HEAD AND KNOCKING HIM FORWARD INTO A TIMBER. THIS CAUSED A LACERATION TO HIS HEAD.
EMPLOYEE WAS OPERATING A SHUTTLE CAR WHEN HE HIT A HOLE IN THE ROADWAY CAUSING HIM TO BOUNCE UP AND LAND HURTING HIS BACK. HE WENT TO THE DOCTOR BUT DID NOT RECEIVE MEDICAL TREATMENT. HIS PAIN CONTINUED AND HE WENT FOR AN MRI ON 6/12/03. HIS LAS DATE WORKED WAS 6/11/03. HE IS NOW AWAITING SURGERY.
EE WAS KNEELING DOWN BEHFIND AN S&S JEEP PUTTING A BELT PIN IN A BELT PULL TAB WHEN HE JEEP TRAMMED BACK RUNNING OVER HIM. THE OPERATOR COULD NOT SEE HIM.
EMPLOYEE WAS ATTEMPTING TO REMOVE A PIN FROM THE TOOL SLIDE HITCH WHEN THE BUCKET JEEP ROLLED CAUSING HIS RIGHT MIDDLE FINGER TO BE CAUGHT BETWEEN THE PIN AND THE JEEP FRAME.
EMPLOYEE WAS OPERATING A SHUTTLE CAR WHEN THE CAR BOTTOMED OUT IN A HOLE HIS LOWER BACK BEGAN TO HURT. EMPLOYEE WENT FOR CHIROPRACTIC ADJUSTMENTS ON 11/19/02.
EE WAS ATTEMPTING TO PULL UP SHUTTLE CAR CABLE SLACK WITH A JEEP. HE HAD THE CABLE IN THE DECK WITH HIS LEFT FOOT ON IT. WHEN THE TIRE OF THE JEEP RAN OVER THE CABLE CAUSING IT TO PULL TIGHT F RACTURING HIS LEFT LEG.
EE WAS OPERATING A BUCKET JEEP ON THE SUPPLY ROAD WHEN HE TRAMMED OVER A SOLID CONCRETE BLOCK IN THE ROADWAY. THIS CAUSED HIM TO BE THROWN UP IN THE SEAT INJURING HIS BACK.
