EE WAS ATTEMPTING TO CLEAN SOME DIRT OFF A TROUGHING ROLLER SEGMENT WITH A CRESCENT WRENCH. APPARENTLY, THE WRENCH FLIPPED UP HITTING THE BELT AND PULLED THE WRENCH ALONG WITH THE EMPLOYEE'S H AND INTO THE BELT (BETWEEN THE BELT & ROLLER SEGMENT). ANOTHER EMPLOYEE PULLED THE EMERGENCY STOP CORD AND EE COULD FREE HIS HAND. HE SHOULD NOT HAVE BEEN TRYING TO CLEAN A MOVING ROLLER.

EMPLOYEEWAS OPERATING HAUL TRUCK. WHILE GOING DOWN AN INCLINE, THE DYNAMIC BRAKES DID NOT SEEM TO HOLD THE TRUCK BACK.THE TRUCK HIT A HOLE AT THE BOTTOM OF THE RAMP CAUSING THE OPERATOR DID NO T USE THE TRAILER BRAKES. NOTHING COULD BE FOUNDWRONG WITH THE DYNAMIC BRAKES.
EMPLOYEE WAS TROUBLE SHOOTING BACK-UP ALARM PROBLEM ON A FORD F-350 SUPPLY TRUCK. ENGINE WAS OFF, KEY ON AND TRANSMISSION IN REVERSE. EMPLOYEE RAN A WIRE FROM FUSE PANEL TO UNDENEATH THE TRUCK . WHILE LAYING UNDER TRUCK, HE TOUCHED A WIRE TOTHE NEUTRAL SWITCH(THIS WENT STRAIGHT TO THE STARTER)CAUSING TRUCK TO START & ROLL IN REVERSE. EMPLOYEE TRIED TO ROLL OUT OF WAY & TIRE HIT HIM
EMPLOYEE WAS DRIVING A DIESEL FUEL TRUCK ACROSS THE TOP OF A CHECK DAM TO REFUEL EQUIPMENT. HE STATED THE TRUCK PULLED TO THE LEFT AND THE TIRES WENT OFF THE EDGE OF THE ROAD CAUSING THE TRUCK TO ROLL OVER AND DOWN THE DAM SLOPE(4-1 ANGLE).RAOD CONDITIONS WERE SLICK, MUDDY AND ICE. SEATBELT NOT WORN. THE TRUCK WAS FULLY LOADED AND CARRIED ABOUT 3000-3500 GALLONS OF FUEL.
