The employee was changing scoop batteries in a crosscut on the Unit 4 travel way, when another employee was driving out battery-first on a ram car and struck the front of the scoop bucket, causing the scoop to roll backward. When the scoop rolled backward, the injured employee's right leg was pulled into the area between the scoop tire and the frame of the scoop.

Employee was operating a Getman Tractor pushing a rock dust pod into a cross-cut. He had his right hand resting on a pinch point warning sign. When he articulated the machine, the center-section stops smashed his right ring finger at the pinch-point fracturing the tip of the finger.

Employee ( coal hauler operator) has been exposed to repeated jarring of his neck while operating his hauler. Finally, after a bouncing occurrence on the date of the report, the employee requested to be taken out of the mine for evaluation. The diagnosis was a neck strain due to the repetitive jarring of the hauler.

While shoveling the 5C head his shovel got caught in belt and kicked back. When the shovel kicked back it dislocated pointing finger on his right hand and tore the tendon. Employee continued to work until 7/11/11 when he went off work for surgery on his finger.

Employee was driving a small mantrip into a crosscut and attempted to manuever past a coal hauler battery when his left front tire contacted the left rib. This caused the steering wheel to jerk and spin suddenly. The spoke of the steering wheel struck the top of his hand. A subsequent MRI indicated a fracture, and the employee was taken off work.

Employee stated he was operating a diesel "mini-trac" to get rock dust on the 4H road. He pulled the machine into a cross-cut as he thought the engine was getting hot. He dismounted the cab, started to walk toward the rear of the machine when he stated the radiator cap blew off and hot water sprayed him in the face and left arm. He received medical treatment beyond first aid.

Employee stated that he was operating a shuttle car and as he was turning his body around in the seat to travel the other way, he twisted his right knee.

Employee was attempting to drive his vehicle around a parked Shuttle Car that had been left out in the middle of an intersection. There was gob on the opposite corner of him and when his front tires hit this gob, it caused the vehicle to slide sideways into the car. The victim was pinned between the dump end corner of the car and the transmission housing.

Employee had walked thru a isolation curtain and going to pull the nips on the scoop, when a mantrip that had just been parked in a entry broke loose, free-wheeled down thru the curtain, struck a notch in the rib at the same time it struck the individual walking. Victim was trated for Laceration to his forehead.

Employee stated he was taking a ride down the slope when the brakes apparently failed. He caught up with another ride and thought it would stop him; also he tried emergency brake and tried to shift the Transmission into reverse or neutral sometime during this time. He turned at the bottom, glanced off the wing wall and struck his head on the coal rib.

Mantrip slid into hole while employee riding in back, bounced him up, he landed on rescuer, pain left hip/lower back. Dr put him on physical therapy, didn't miss any work from accident, later sent to receive spinal injections, injection sites began to leak fluid, patches placed over holes,started missing 9-14-09. patches applied on 9-16-09. We feel this qualifies as an illness.

Employee was walking beside a parked ramcar. The parked car was sitting a little out in the intersection and another car came through from the other direction, hit the parked car and knocked it sideways, causing the wheel guard to strike the victims ankle and foot against the rib.

Employee was taking a golf cart down to position it for an Examiner. As he went around the corner at the bottom, he lost control of the golf cart and struck the metal wing wall, causing him to jerk his shoulder. He has worked until 06/17/2009 and has been taken off work for shoulder surgery.

Employee stated that he was driving a Ramcar and ran over a 4-5" timber causing him to bounce up and hit his head on the canopy. He continued to work and then was taken off work on 01/13/2009 with a back injury. Employee has had two prior surgeries and a Doctors report showed degenerative disc disease and had back problems before this incident.

Employee was a passenger in a vehicle that was traveling on an U/G travelway. A piece of roof rock fell and struck the driver of the vehicle causing him to probably lose control. When this happened the vehicle struck a coal rib throwing the passenger out to the ground causing a fracture to his left instep (metatarsal bones).

Employee stated he was standing at the side and end of a ram car shoveling some rock dust out of the bed of the car. The stinger, liked a little pushing all the rock dust out, so he was shoveling the rest. The operator of the car told him to watch out he was going to do something and thinking the injured would move, he let the bed down and the injureds foot was under the edge.

Employee stated he struck his head against the canopy after hitting a hole in the roadway while driving a Ram Car. He also stated that he had hit his head many times through the years and the MRI showed degenerative disc disease at several places cervically. EE started missing 4/11/08.

Employee stated he was operating an underground Tractor, when he hit a bump in the road that caused him to be bounced upward, striking his head on the roof. Employee didn't miss any work until 02/14/2008.

Employee stated he was changing batteries in his car when they slipped off. This jarred the car causing the employee to hit his head in the canopy. The employee was under a doctor's care for a non work related neck and back injury. The operator does not believe that this event caused an injury. This report is being filed to abate citation # 6667460

Employee complains of back spasms/pain and has a past history of spasms/pain in the same area of his back. After driving a tractor down the 4b travelway the employee felt spasms/pain in his lower back. Employee stated that the rough road aggravated this condition. Employee did not start missing work until 8/4/2007.

Employee stated that he was riding in a mantrip sitting in the very back seat when the trip hit a bump and threw him upward and his head hit the canopy. The employee has had fusion surgery on his back and neck in the past and degenerative problems for a long time.

Employee stated that he had driven a Ramcar to the battery barn to change batteries. As he was traveling into the barn he ran over a concrete block and it threw him upward and he hit his head on the canopy. Started missing 4/02/2007

Employee stated that he was riding in a man-trip going inside. The man-trip hit a bump in the roadway and he bounced up and hit his head on the canopy jamming his neck. Employee didn't miss work until 03/06/2007.

Employee was operating a ramcar and was going around a corner. He turned the car too sharp and hit the coal rib with his cab/canopy. The rib dislodged and some pieces of coal flew off the rib and struck the victim in the mouth. This caused a laceration to his chin that required some stitches.

Employee was operating a ram car, driving it up onto a lowboy trailer. Bridge boards had been placed in brackets on the side of the trailer. When the ram car tire hit the joint of the boards, the board sprang up on the other end. The employee had leaned out of the cab to see if he was clearing the ramps & was struck by the board. Required stitches around his right eye.

EE WAS DRIVING A PERSONNEL CARRIER DOWN A TRAVEL ROAD. HE RAN OVER SOMETHING OR HIT A HOLE IN THE ROADWAY AND INJURED HIS BACK.

EMPLOYEE WAS OPERATING A BATTERY POWERED SCOOP AND WAS IN THE PROCESS OF TURNING IT AROUND. THE TIRES CAME IN CONTACT WITH A CONVEYOR BELT TOP ROLLER & THREW IT INTO THE CAB COMPARTMENT, WITH ANGLE PIECE STRIKING VICTIM.

EMPLOYEE RODE INTO THE MINE SEATED ON A MANTRIP. AFTER ARRIVING HE STARTED TO GET OUT & HAD A SHARP PAIN IN HIS BACK. STATED THE RIDE WAS NOT BUMPY & HE COULDN'T RELATE IT TO ANY SINGLE INCIDENT. EE HAD ONLY WORKED 3 DAYS.

EE WAS SITTING REPAIRING A ROCK DUST HOSE, WHEN VEHICLE THEY WERE USING WAS STRUCK BY A BATTERY HAULER. THE VEHICLE WAS SHOVED TOWARD THE VICTIM, STRIKING HIS FOOT & BENDING IT. (TOES TO SHIN. ) EE DID NOT HAVE THEIR LIGHTS ON THE VEHICLE & WAS NOT AWARE BATTERY HAULER WAS COMING DOWN THE TRAVELWAY.

EE WAS DRIVING SHUTTLE CAR DOWN ENTRY, HIT POT HOLE CAUSING BODY TO STRIKE TOP OF CANOPY.

EE WAS TRYING TO UNHOOK CHAIN FROM SCOOP BUCKET TO FEEDER AND DIDN'T HAVE ENOUGH SLACK IN CHAIN. SCOOP ROLLED BACK AND CAUGHT LEFT THUMB BETWEEN CHAIN AND SCOOP BUCKET.

INJURED STATED HE RAN OVER TIMBER IN 4B TRAVELROAD AND IT THREW HIM IN RAM CAR AND WHEN HE CAME DOWN HIS LT. LEG BELOW KNEE HIT A BOLT HEAD CUTTING HIM. STITCHES. ALSO SAYS HIS TAILBONE WAS STRUCK.

EMPLOYEE WAS MOVING BELT DRIVE, THE BEAM DRIVE IT WAS SETTING ON, KICKED OUT, HITTING HIM ON LEFT LEG ABOVE ANKLE.

MANTRIP TOP HIT STEEL ARCHES, AND TOP OF MANTRIP CAME OFF AND HIT EE IN THE HEAD CAUSING INJURY.
