Employee was operating a Chevrolet 2500 hd pickup enroute to the pit. He said he heard a noise, panicked and hit the gas pedal, veering into and striking the embankment. His head hit the steering wheel. He was not wearing a seat belt.

Employee stated that the A/C was to cold in the cab. He then inserted a rag into the vent and shoved his finger into the A/C fan cutting his left index finger.
EE had left the office going home and was traveling on the mine access road when a deer ran out in front of her personal vehicle causing her to hit the berm and flip her jeep.

Employee was spotting for another employee that was operating a forklift. The forks on the fork lift got in a bind on the load that was being picked up. The fork came off the lift and smashed the spotters finger between the fork and the load that was being moved.

Bulk powder truck was ascending a wet/snowy/icy ramp when it lost traction. Driver shifted into reverse to attempt to back off ramp when it slid. Driver attempted to scrub off on side of berm. Truck rolled onto berm and fell over on to side. Driver and passenger were both buckled in and sustained light injuries. Both treated and released at local hospital ER.

Employee stated that he was traveling off the property at the end of his shift when a vehicle coming in on the property hit the vehicle he was a passenger in. At contact he stated that he hit his head on the rear view mirror cutting his forehead & above his right eye.
While operating a Komatsu 730E rock truck, an employee left the haulroad striking the time study on the right side. The rock truck came to rest on top of the time study vehicle. This vehicle was parked 223' from the haulroad. Employee was terminated due to not adhearing to company policy.

Extraction took approximately 1 hour. Employee was treated and released from Caml Gen Div. and returned to work on his next scheduled shift.

A loaded 777D rock truck backed through the berm and overturned at the dumpster at the 4100 p&h shovel pit on south main ridge.
We are contesting this injury pending investigation - employee stated that he was traveling down warehouse hill and hit a hole in road, his seat bottomed out and jarred him. He complained of numbness and tingling in his left arm & shoulder. He refused medical treatment and an ambulance.

EE DROVE TRUCK UP HILL TO A DRILL BENCH. HE STOPPED AT THE CREST OF THE HILL. WHEN HE ATTEMPTED TO GO FOREWARD, THE TRUCK STARTED TO ROLL BACKWARD. THE TRUCK ROLLED OVER ON IT'S SIDE.
THE COMPANY WAS NOTIFIED ON 5-15-00 OF THE ILLNESS. EE BEGAN MISSING WORK ON 10-14-00.
INJURED WAS WORKING ON TOP OF ADDLER SYSTEM AFTER KNOCKING BREAKER. THERE WAS ANOTHER MEMBER WORKING ON TOP OF LAUNCH WHEN HE HEARD THE BREAKER KNOCK HE WENT OVER AND RESET THE BREAKER. THIS C AUSED A ROCK TO FALL FROM THE CAR TO THE GROUND LANDING ON INJURED'S LEFT LEG.
