Ford Truck collided with Powder Truck at N-14 Facilities intersection. Ford was headed east and Powder Truck was pulling out of a stop sign when they collided. Ford driver was transported to hospital treated and release with bruises. Powder truck driver was treated by paramedics and released.

As a scraper operator was running in the spoil cut area, was looking back at the apron and bowl and hit a large partially covered boulder. As operator made contact with boulder, the scraper bounced hard and the seat bottomed out causing operator to strain lower back.
As backhoe 450 was loading Haul Truck 409, large chunky material was dumped into the bed of the truck. The shock load of the rocks being loaded caused the truck driver to suffer back and neck pain. The IE did not report to their next scheduled shift.
Employee was installing a conveyor belt clamp, and was holding the clamp when it got hang up. The clamp was being moved by a forklift, fork got hung up released and jarred him. This release jarred him and hurt his shoulder.
The loaded truck came out of the pit when it veered across the road and struck a road berm and jackknifed on top of berm.

The scraper started to proceed down an incline following the curvature of the road when the tail-end began to skid towards the outer edge of road. The scraper bowl contacted the ground and the push block also nosed into the road, causing the scraper to roll several times down an incline. Operator escaped through the rear cab window after unsecuring his seat belt.
The welder was loading junction boxes onto lowboy trailer using a loader to lift them. As he attempted to make another lift the load shifted sideways tilting the loader and turned it over on its side. Operator struck his left knee against the steering wheel injuring it.
The haul truck went off the side of the road traveling over a berm and onto a lower section of a ditch terrain. The operator was jarred around inside the operator's cab causing multiple injuries. Initial assessement of injuries reveals laceration to chin, lips, pain to right leg and to lower back.

EE CAME OFF J-21 MAIN HAUL ROAD GOING AT APPROX. 45 MPH FROM K MINE FACILITIES, MADE A TURN GOING TO J-21 EXPLOSIVE YARD, APPROACHED THE EAST GATE GOING ABOUT 15 MPH & TRIED TO APPLY THE BRAKES. BRAKE PEDAL JUST FELL TO THE FLOOR, STEERING WHEEL LOCKED UP. FRONT OF VEHICLE WENT UNDER THE GATE - WINDSHIELD SHATTERED, GATE OPEN BROKE SIDE POST.

Operator's left hand was struck by a piece of shattered glass when a rock fell off the front end loader bucket, striking the end-dump truck cab glass during a loading operation
EE WAS ENROUTE TO THE SILO AREA AT APPROX 6:12AMWHEN SHE FELL ASLEEP & RAN OFF THE ROAD. SHE WASTRAVELING APPROX 10 MPH HIT A SMALL EMBANKMENT (2') HIGHT DROVE ALONG SIDE THE EMBANKMENT FOR ABOUT 10 FT BEFORE SHE CAME TO A STOP. SHE WAS DRIVING UNIT 301
THE OPERATOR WAS LEAVING THE PIT AFTER HE WAS LOADED WHEN HIS TRUCK HIT A SOFT SPOT IN THE PIT FLOOR (ROAD). HE WAS JARRED INSIDE THE CAB CAUSING LOW BACK PAIN. HE MOMENTARILY BLACKED OUT AND EXPERIENCED SHORTNESS OF BREATH.
ON 11-6-01 EE STATED THAT HE WAS DRIVING OUT OF THE PIT WHEN A CALL CAME IN ON THE RADIO. AS HE REACHED OVER TO PICK UP THE RADIO HANDSET HE TOOK HIS EYE OFF THE ROAD AND DROVE INTO THE ROAD B ERM. HE STATES THAT HE THINKS HE WAS KNOCKED UNCONSCIOUS MOMENTARITY AND FELL OFF THE DRIVER SEAT. HE SUBSEQUENTLY GOT BACK ONT HE SEAT AND BROUGHT THE VEHICLE TO A STOP. LATER THAT DAY HE WE
FOREMAN PULLED UP & PARKED IN FRONT OF HAUL TRUCK #464 WHICH WAS WAITING TO BE LOADED. THE FORE MAN MADE A HAND MOTION TO THE DRIVER TO COME DOWN. WHILE WAITING HE NOTICED TRUCK ROLLING TOWARD VEHICLE W/DRIVER ON THE CAB PLATFORM. THE INJURED JUMPED FROM PASSENGER SIDE BEFORE IMPACT. HE GOT PUSHED UNDER THE VEHICLE & POSITIONED HIS BODY IN CENTER OF HAUL TRUCK AS IT PASSED OVER HIM
THE INJURED WAS TRAVELING IN HIS PICKUP TRUCK BEHIND A WATER TRUCK AND AS HE TURNED LEFT INTO A PIT RAMP, A VEHICLE TRAVELING THE OPPOSITE DIRECTION SLID INTO HIS TRUCK ON THE LEFT SIDE.
