EE was driving haul truck, hit a bad spot (hole) in haul road. Right hand operators cab window broke & shattered. The ee was covered with broken glass particles w/some particles entering right eye. EE was taken to hospital ER, eye was cleaned of glass & released. He received additional medical treatment in proceeding days; prescribed antibiotics, pain meds, restricted duty.

Mostrando los 23Terex truck driver attempted to raise bed of truck to dump load. Truck bed would only raise slightly, before dropping back down. Attempted several times to raise bed before calling for assistance, which caused jarring of the truck. Later in the shift employee began to complain of back pain. Seen in ER and diagnosed with a back strain, placed on restricted duty.
Employee was crossing a dump location at approximately 2 - 3mph in haul truck. When driving over a rough spot in road, he felt a sudden onset of neck pain and later developed numbness in first 3 fingers of right hand and pain down his right leg. Employee was transported to hospital for evaluation.
Employee smelled exhaust fumes while operating haul truck and became light headed. Truck was found to have an exhaust leak. Employee was seen by MD and placed on restricted duty x1 day.
Employee complained of shoulder pain after hitting a pot hole on haul road while riding on crew bus. Sought medical treatment on 03/23/06 and became a reportable.
Employee was moving a man-lift down haul road. Man-lift struck a pot hole causing bucket of man-lift to jar employee, who then fell striking knee on deadman switch cover, causing a laceration to right knee which required stitches. Employee was placed on restricted duty.
Employee was looking over right shoulder at can of scraper and hit a hole resulting in pain and stiffness in neck. Treated by Chiropractor for neck strain, this became reportable on 02/05/06 when employee was placed on restricted duty. Employee began missing time on 02/06/06.
Driving down haul road in service truck when right rear duals dropped into a pothole on road that was not visable due to mud and rainy conditions. Hit head on computer console in truck causing neck strain. Transported to hospital for evaluation.
Employee was operating a scraper going down grade in cut area. Cushion hitch of scraper was turned off as scraper entered cut down grade. Floor of cut area was slippery as scraper entered cut; machine slid into rough area. Employee strained lower back when traveling along rough area. Local ER @ hospital did evaluation & 2 days restricted duty then full duty.
Employee was moving 40' pipe with a crane and forklift. the strap slipped that was attached to one end of pipe with forklift and allowed pipe to swing and strike ee in right lower leg causing bruising.
EE ALLEGES THAT WHILE OPERATING A TRUCK HE BEGAN TO EXPERIENCE BACK PAIN/MUSCLE SPASMS. EE MISSED 4 DAYS OF WORK DUE TO ALLEGED BACK PAIN. (MISSED 8/18-21) NO MEDICAL TX PER PART 50. THROUGH CONVERSATION WITH EE'S MEDICAL PROVIDER EE HAS ONGOING/CHRONIC BACK PROBLEMS. HIS NOTES TO FOLLOW.
Employee reported that his back and neck area was hurting. Was operating scraper. He received medical treatment and was released to restricted work.
EE CALLED UNION AGENT, TOLD HIM HAD NUMBNESS & WEAKNESS ON ONE SIDE OF THIS BODY. EE STATED THAT CURRENT INJ. WAS CONNECTED TO AN ACCIDENT OF 7/8/04 (AT THAT TIME EE WAS EXAMINED & RELEASED W/O TREATMENT). EE STATES THAT EQUIPMENT HE OPERATED 7/20/04 (NIGHT SHIFT) WAS NOT WORKING CORRECTLY & CONTRIBUTED TO HIS CURRENT PROBLEMS. EQUIP. CHECKED: UNABLE TO FIND PROBLEM.
SCRAPER OPERATOR WAS ROCKING HAUL ROADS WHEN THE EDGE OF THE SCRAPER PAN CAUGHT AND DUG INTO THE ROAD CAUSING THE SCRAPER TO ROLL OVER ONTO ITS TOP. OPERATOR OF THE SCRAPER EXPERIENCED A SORE LOW BACK. RELEASED TO REGULAR DUTY 6/15/04.
EE ALLLEGES WHILE SERVICING A FUEL TRUCK THE HOOD DROPPED DOWN & STRUCK THE BACK OF HIS HEAD. EE CONTINUED TO WORK & NEEDED NO MEDICAL ATTN. ON 6/22/04 WE RECD. NOTICE THAT EE FILED FOR WORKERS' COMP. BENEFITS FOR CHIRO. ADJUSTMENTS -CERVICAL STRAIN. EE ALLEGES HE MISSED 10 DAYS OF WORK STARTING 2/6/04. WE WERE UNAWARE OF THIS TIME AWAY & THAT IT WAS RELATED TO INJURY 2/4/04.
THERE WAS NO IMPAIRMENT AND THE WORKER WAS RELEASED FOR DUTY WITHOUT RESTRICTION. THERE WAS NO INJURY EVENT ON THE JOB, THEREFORE THE WORKERS COMPENSATION CLAIM IS BEING PROTESTED WITH THE STA TE OF WASHINGTON.
EE WAS RETURNING FROM DUMPING A LOAD OF ASH. HE FAILED TO LOWER HIS BOX & AS HE PASSED UNDER #1 CONVEYOR THE FRONT OF HIS BOX (CANOPY) CONTACTED THE UNDERSIDE OF #1 CONVEYOR CAUSING THE TRUCK TO TIP BACK, RAISING THE FRONT TIRES OFF THE GROUND. THE MOMENTUM OF THE TRUCK ALLOWED IT TO GO UNDER THE CONVEYOR AT WHICH TIME THE FRONT TIRES RETURNED TO THE GROUND JARRING THE EMPLOYEE.
EE ALLEGES WHILE BEING LOADED BY THE SHOVEL, THE LOAD PLACED INTO HIS TRUCK RESULTED IN AGGREVATION TO HIS LOW BACK. HE BEGAN MISSING WORK ON 08-18-02.
EMPLOYEE HAS ONGOING PAIN IN HIS LOW BACK WHICH MAY HAVE RESULTED FROM EXPOSURE AT THE MINE AND MAY HAVE BEEN AGGRAVATED ON 3/11/02. A MRI SHOWS DESSICATION (DEGENERATION) OF THE LUMBAR SPINE WITH DISC PROTRUSION. WE ARE REPORTING THIS AS AN OCCUPATIONAL ILLNESS DUE TO THE PROGRESSIVE NATURE OF THIS DISEASE.
EE ALLEGES THAT HE FAILED TO RELEASE THE CUSHIONHITCH ON THE SCRAPPER AFTER DUMPING ALLOWING THESCRAPER TO "BOUNCE" & GIVE HIM A JARRING SHOCK TO HIS NECK & SHOULDER. NOTE: EMPLOYER IS CONTEST ING ALLEGED INJURY THROUGH COMPENSATION SYSTEM -STATE OF WASHINGTON. EE TERMINATED ON 9/6/01 FORVIOLATION OF COMPANY POLICY: REFUSAL TO PROVIDE A VALID POST-ACCIDENT SUBSTANCE ABUSE TEST.
EMPLOYEE WAS DECENDING DOWN TOPSOIL WORK AREA. HE LOWERED THE CAN ON THE SCRAPER AND CAME TO A SUDDEN STOP, INJURING HIS BACK.

DRIVING LUBE TRUCK ON A FULLY WATERED ROADWAY, TRUCK BEGAN TO SLIDE. TRUCK HIT A DRY SPOT AND TIPPED ONTO ITS SIDE, CAUSING PAIN IN THORACIC AND LUMBAR SPINE.
