An employee was injured while crossing under an unguarded area of moving conveyor belt. The hood on their sweatshirt was pulled into a bottom belt roller, and as they attempted to free it, their left arm was drawn in between the roller and the belt.

Mostrando los 25Employee reached down and picked up a come-a-long which had fallen out of position. As EE went to hang it back up, a rock of unknown size bounced off the belt and struck EE's left hand, nearly severing EE's index finger.
Employee was operating a scoop to off-load a loaded belt structure sled from a supply car. The sled got bound up. When it freed, the scoop bounced causing the employee to hit the top of EE's head on the scoop canopy.
Employee was tramming shuttle car from the feeder to the face in the #3 entry of the 2 Right section and hit a hole. The force of hitting the hole bounced EE out of the seat and EE hit head on the canopy roof, jamming neck.
Employee was tramming the off-standard shuttle car and hit a rock in the haul road causing EE to hit the top of EE's head off the canopy, jamming neck.
On 3/13/23, employee was preparing to remove monorail on the lw headgate. EE was climbing over the crossover at the tailpiece. As EE went to take the first step down the ladder on the monorail side (tight side), a large rock came out of the LW stageloader chute, bounced off the belt, ricocheted off the ladder framework, and struck EE on the right ankle.
An experienced trained beltman was prying up a piece of metal plate to align bolt holes on a booster drive frame. EE was working under a piece of belt guarding, under the belt. As EE pushed up on the plate EE's screw driver slipped, EE's arm went up, pushing the belt guarding to the side and EE's right arm was pulled between the belt and bottom roller causing an abrasion.
Employee #1 was sitting in the shanty. Employee #2 was pulling a Dapco battery ride into the shanty, not knowing #1 was in there. #1 called out, startling #2, causing them to inadvertently hit the acceleration pedal. The Dapco ride struck #1 and pinned them and their chair between the ride and under a work bench table.
Employee was raking coal from under the 6 Left impact bed. As EE pulled the rake back, the end of the handle was pulled between the roller and the belt, sucking the rake out of EE's hand, causing the rake end to strike EE in the head and EE's hand.
While checking chutes in overland crusher building, a baseball sized rock bounced out hitting employee in the nose.
Driver was backed into plant with 20 ft of hose laid out to keep out from under belt line. Rock was ejected from belt and hit EE on left ankle and foot.
Employee was prying down to remove rock from bottom, bar slipped and rolled catching bottom belt and roller forcing bar across right forearm and chest. Resulting in bruising to chest and bruising and swelling to right forearm.
Center bolter cable was being pulled by the scoop. Employee was standing approximately 3' from cable. As the cable was being pulled, it flipped toward center of entry, striking employee in the left knee.
Employee #1 signaled employee #2 to back up motor to take tension off of the water car coupler. Employee #1 thought the water car was uncoupled from the motor and told employee #2 to pull away. The water car was not uncoupled and was pulled over top of employee #1's foot when the motor moved forward.
Employee was riding out of mine on mantrip. The mantrip hit a motor stopped on haulage track. Employee head hit Plexiglas of windshield and ee body was thrown forward.
Employee was operating a JLG scissor lift. As they moved the machine closer to the work area, their right hand was pinched between the operator's basket handrail and a steel beam.
Driver A was driving down the hill from refuse site unloaded in an articulating truck and struck driver B coming up the hill in a loaded articulating truck. Impact occurred on driver B's truck on the right side of truck. Driver B's head hit left side window glass and left shoulder struck cab interior
Employee was operating a shuttle car and hit a bump in the haul road causing EE to hit EE's head off the canopy. EE was seen at a local ER which initiated a series of specialty care appointments ultimately leading to an MRI & an appointment with a neurosurgeon, which EE had on 03/16/16. The neurosurgeon kept EE off work pending further evaluation.
Employee was tramming the fully loaded #9 Auxier Welding off standard shuttle car to the feeder when he went thru a dip in the haul road. He came out of his seat and struck his head on the canopy roof causing pain in his mid to lower back.
Operator was traveling inby on a brookville locomotive with two full cars. The track switch at 36 wall on the east mains haulage had been left for the curve causing the trip to enter the switch. The motorman was unable to stop the trip and struck his head on an 8" plastic waterline that runs parallel to the main line haulage.
Unloading block from scoop when employee instructed the operator to ram in and the employee had his hand on the scoop bucket and the ram pinched his hand.
The mantrip derailed at 12 break and hit the rib inby the corner and spun around and hit corner with left side of mantrip, thus spinning until rear of mantrip was facing out by. There were 7 employees in mantrip,5 were transported to hospital with 2 being held overnight for observation.1 suffered a broken shin & dislocated ankle, the other a laceration to the head

Employee was tramming a battery powered scoop in reverse when he run over a section of scrap water line, the pipe bent upward and came into the operator compartmant striking the operator in the left rib area.
