The employee was standing beside of EE's tractor (7024) when the 7041 tractor that was parked across the road from EE rolled into EE's right leg causing a laceration to EE's right leg that required sutures.

Employee was loosening a J-bolt on a bottom belt hanger bracket with right hand and left hand contacting the top chair for leverage. While loosening the bolt left small finger accidentally contacted the top roller causing a friction burn from contacting the roller. They received three stitches to the finger.

Employee was reclaiming belt framing on the old 4C belt when the lo-trac EE was operating came in contact with a piece of hanging framing that shifted contacting the employees left arm causing a laceration that required sutures.

Employee was riding on the 5114 mantrip going to the working section when the mantrip contacted a steel square set leg, causing the mantrip to come to an abrupt stop and the employee to hit the interior on the ride with right shoulder causing a suspected sprain/strain.

The employee was crossing a shuttle car cable near the anchor. As they stepped on the cable to cross, the car picked up cable slack from two entries away causing the cable to contact the bottom of the employee's boot resulting in a fracture to their left foot.

The employee was using EE's ride and a strap to pull the needed miner slack on #7 road, EE placed the strap on the end of EE's boot that was in the operators compartment. Before the cable could be pulled the strap got under EE's rear tire causing the strap to be pulled towards the rear wheel causing EE's foot to be pulled out of the ride resulting in a sprained foot/ankle.

The Employee was traveling on 4-54 road at crosscut 5 when a roof bolt flipped up from the tire striking Employee in the forehead causing a laceration that required 5 stitches.

The employee was turning into crosscut 16 on the 10/48 road when a pin buried under some loose material came up and struck EE in the forehead resulting in a laceration that required 6 stitches.

The employee had unhooked a hauler from a water tank, while walking back to the operators deck EE noticed EE's lunchbox wasn't in a good place so EE stepped next to the hauler tire with EE's left foot to reach for EE's lunchbox when the hauler moved causing the tire to contact EE's left foot resulting in a fractured big toe.

Employee was riding behind the driver on a four man diesel when the drivers hat caught a piece of wire hanging from the mine roof that flipped back striking the employee in the upper lip resulting in a cut that required 5 stitches.

Employee was backing out of a crosscut when a wooden runner flipped up striking the employee's left hand smashing the middle and ring finger resulting in a minor fracture in the tip and stitches in the ring finger and stitches in middle finger.

Employee was putting boards under the feeder. There was a miscommunication between the feeder operator and the employee. The feeder was lowered instead of raised causing the employee's middle left finger to be caught between the boards. The employee's middle left finger was mashed between the boards.

Employee hit head on car canopy after employee ran over a rock in the travel way, employee had a small cut that required two staples.

Employee was backing the hauler out of a cross cut and had right hand resting on the side of the hauler. When employee steered the hauler, employee's little finger was caught between a guard and the steer stop that was mounted near the center section of the hauler. Employee finger had laceration that required stitches.

While riding in vehicle on supply road, driver hit pot hole in road unexpectedly. Jarring caused back pain.

Employee was driving a 2 man diesel ride when EE hit head on an overcast. Multiple MRI's and Cat Scans were done, but no fractures or bleeds were found.

Employee was helping to put a cutting motor in a continuous miner when the motor slipped on low trac forks, catching EE's hand under motor and forks.

Employee was riding on a 4 man diesel ride behind the driver. As the ride entered a turn an object was kicked up in the air and struck employee in the nose and mouth. Employee suffered a laceration to the nose and several teeth were loosened.

Employee ran over loose rock with the stinger ride and the steering ball on the steering wheel hit employee on right little finger and torn ligaments. On 10/1/19 employee had surgery to put a pin in right little finger. Back to work, on 10/2/2019 no restrictions.

Employee was on a diesel mantrip and ran over a piece of old belt that was lying in the supply road. The belt caught on the tire and pulled employees left leg backwards and bruised lower leg causing a hematoma. On December 13, 2018, doctors surgically drained hematoma.

Employee was hauling T3 channel with the lo-trac, the T3 channel hit the metal doors, then the T3 channel spun around hitting the operator in the right ear. This caused a laceration that required stitches.

Employee was driving a 2-man diesel ride down 13-54 road when he hit a hole in the road and lost control of the ride, he came off the ride and hit the mine roof. The employee had laceration on his head and fractured right collar bone

Employee was helping pull on belt with a diesel scoop when the belt lapper came off and struck employee in the lower left leg. The employee found out on 5/13/15 that the leg had a small fracture.

Employee was backing a diesel ride into the tool shack and his tool pouch hung on the door of the shack, pulling him around catching his foot between the diesel ride and door.

Employee was driving a shuttle car down # 3 entry on # 1 unit and ran over a unknown object, he felt pain in his lower back.

Employee was moving the forks inward on the low trac to unload supply's, the employee got his right little finger between the frame of the mast and the top of the fork, cutting his finger that required stitches.

Employee was tramming a golf cart towards #3 entry, while tramming the golf cart the employee trammed over a ledge causing the golf cart to overturn.

Employee was operating a diesel scoop and hit his head on a pin board causing pain in right arm. Employee started missing on 1/13/2014.

Employee was helping unhook the low trac from a trash trailer, when the fork of the low trac became dislodged and bounced up and struck employee on right cheek, causing a cut that requires 8 stitches.

Employee was driving down #5 unit supply road, employee hit a roof bolt cutting the top of his head.

Employee was riding in man trip, the man trip slid into a coal rib jam and twisting ankle.

EE was riding out on a mantrip, when the ride hit a bump throwing EE into canopy of the ride. EE started to miss 4/27/2012.

Employee ran over a rock and it threw him into the canopy on his shuttle car. EE went to chiropractor a couple times but did not have any lost time or restricted days.

EE was driving 2 man ride and foreign object got into his left eye.

While moving supplies on #4 unit, the hauler the employee was running hit a coal rib causing coal to hit the employee on the head, back and right arm.

While building a brattice a shuttle car turned into the area where the brattice was being built striking the employee's legs.

He was kneeling in the mouth of # 5 entry, by the curtain on # 2 unit when the scoop come through the curtain striking injury with the bucket.

Employee was in the coal loadout tunnel watching the belts while the train was being loaded. He noticed a piece of coal was stuck in the skirt rubber on the south side of the belt (main). He removed the piece of coal and the conveyor belt caught his right hand and arm

String off of the belt caught his finger.

Driving a car and hit a hole. This caused him to hit his head on the canopy.

Employee was driving shuttle car when he hit hole, causing him to hit his head on canopy. This jammed his neck.

Employee was driving a golf cart under an overcast. A tire on the golf cart hit a hole in the road causing him to jar his back. The employee received treatment from a chiropractor making this a reportable injury.

Employee was driving shuttle car when he struck hole in road. he bounced in the air and came down and struck the "E" stop button. This caused him to be knocked unconscious

Employee got off diesel scoop. Scoop rolled back & caught employee between scoop and diesel generator. Fracture to pelvis.

EMPLOYEE WAS WALKING UNDER CAR CABLE THAT WAS HANGING FROM ROOF. THE CAR BECAME ENERGIZED AND PULLED THE CABLE DOWN ON EMPLOYEE'S RT. RIBCAGE AND BACK.

Employee was driving vehicle through a curtain, when curtain drug material off ride onto employee's leg. Employee had to have sutures to close wound.

Employee said she was a passenger on a rib runner. When the driver got too close to the rib, she rolled and her foot struck the rib.

The mechanic was wiring a new golf cart in the ug shop. When he tested the foot switch the cart lunged striking his knee and another ride. He reported this injury and started missing work on 11/6/07.

The victim hit a hole in the road while driving his shuttle car. This caused him to jar and jam his neck. He went to the Chiropractor at a later date making this injury reportable.

A foreman was tramming a Lo Trac diesel forklift through a curtain. The forks hung in the curtain and flipped over striking him on the right forearm. His arm was lying on the side of the operator's deck.
