The employee was operating a 777 cat trk on the haul rd when he stated he hit a bump in the rd. At which time he felt a pinch in his lower back and tingling. The employee had stated that he had previously had problems with his back when in the military in the 1990's

The employee was operating a 777 rock trk when the trk bounced and the air ride seat bottomed out. At which time the employee experienced discomfort in his lower back.

While sitting in his rock truck waiting to be loaded, a rock rolled out of the shot, striking the rock truck in the rear of the truck, causing a quick jerk to the operator.

While turning and backing down a slope, the 30D Volvo mud truck turned over on its side, the driver was not injured.

EE was leaving the mt top area after fueling the equipment, leaving the mt top ee lost control of the fuel truck coming down the hill causing the truck to roll over on to its side

EE stated that while being loaded, the loader man dropped a rock into the bed of his rock truck, causing back pain and tingling in his legs.

Was riding on rear raised section of man trip. Right shoulder and coverall shoulder strap caught roof bolt plate pulling him off back of man trip.

EE was running roller on strip and when he out of the roller at the end of the shift. He said his back was hurting him badly and couldn't hardly walk to hid truck. EE had a knot come up on his back on lower left side.

Boom on loader kicked out hitting bed rail of trip 67 causing driver to hit his head.

Unloading roof bolt supplies out of scoop bucket onto #2 roof bolter when the scoop bucket pressure leaked off slowly onto the top of the left foot causing pain to developed in his left foot. The injured refused medical treatment and refused to be transported to medical facility for treatment.

EE said that the truck jumped out of gear and he could not get it to go back into gear. EE got the truck out of gear himself. (Shifting)

EE was tramming scoop on section between #2 and #3 entry when operator's light cord caught a roof bolt plate jerking his head into the canopy of the 488 scoop.

While picking up a bucket of refuge EE was curling his bucket in the pile and the rear wheels came off of the ground and then came down hard causing the seat to bottom out. EE states that he felt pain in his lower back.

Rock truck driver left work at 11:15 AM to go see his doctor. He said he wasn't feeling well and he felt his face going numb. He mentioned that the bed on the RT23 truck was coming down hard, but didn't think this had anything to do with the way he was feeling. ***Days later he said the Dr said the bed coming down hard could have been a factor.***

EE was feeding the raw coal feeder with a 980 Loader. As he approached the feeder at a right angle, he caught the rt. corner of the feeder with the lt. corner of the loader bucket. EE stated that when he hit his forward momentum was stopped by the seat belt causing pain to his lower back.

Driver in pick-up passed empty coal truck and hit a stopped truck in the front

EE was mounting the shuttle car anchor when the shuttle car operator turned the corner the EE was on wrong side and buggy cable pulled against the rib hitting the EE's knee area.

Coal truck driver was coming out of pit where there was a rut, driver straddled rut and overturned. Driver said he was dizzy got emt to him they looked at him ,gave him oxygen, called 911 for ambulance.

Two EEs were injured in an accident involving a pickup truck & a highwall drill. The truck traveled down the drill bench slope & struck the drill. The two EEs were apparently outside the drill, both on foot at the time of the accident & were struck by the truck. The exact location of these 2 EEs have not been determined at this time & the accident is still under investigaton.

Small ledge in roadway, s/c ran over it causing the s/c to bounce man into canopy striking his head.

Visbility was limited. Foggy night. Rock Truck was backing up to dump when it hit the top rear section of a D9T Dozer breaking the glass and creasing the top of the dozer

EE was cleaning up a stockpile area around the outside edge and hit a rock that threw him forward hitting his right leg.

992G loader operator mistakenly hit the quick drop on loader while reaching into floor board area to pick up a snuff can. The boom came down on the side of the truck jamming the truck driver; driver felt pain in back.

992 operator was helping move large rock out of pit; bucket slipped off the rock, operator stated that it bounced him up and the seatbelt caught him. Slung him back down and he hit his left elbow against joystick controller.

EE was hauling rock from #1 Point Pond Creek Pit to #2 Valley fill, EE said he hit bump in haul road causing seatbelt to jam his right hip. He said he has pins in hip.

The roads were wet & slippery, EE was trying to pass a coal truck, lost control of his S10 and went to into a slump hole around Marker #22. It skinned his cheek, put a laceration in his knee requiring 5 stitches, and a contusion.

He backed the coal loader into the Highwall. Said he hurt his back.

MVA - Driving unit # 88103 shot loader down a haul road to bins, made a turn, truck flipped making the turn, hurt shoulder.

The 988 coal loader was chopping coal to load and the 992 spread was working same pit. As 988 was backing up chopping coal, the bumper of the Loader hit the bumper on rock truck.

EE was moving beam augers from pit #12 to the parts container. He was dumping them off his forks, when an auger slid off the forks and came through the front windshield. Busted out the windshield and EE got glass in his eyes from the windshield.

Loader caught solid rock on right side jerking operator, resulting in sprained neck.

Loading 777D with 988H (High Lift), tire fell in a hole and EE said he hit his head, causing a neck sprain.

Employee clocked out @ 11:29 AM & was on his way in his personal vehicle. He drove 3.2 miles down the mine when he was struck by another vehicle. Unknown to us the mine did not file this MVA. We just found out the employee wants to file this claim with us. Initially this claim was not filed since he was off the clock. Unknown information @ this time on the other driver.

Truck driver backed truck under loader bucket to be loaded loader operator dropped a rock into truck bed causing Neck problem this incident did not require first aid or rescue squad.

Loading in shot, large rock rolled off the top of shot and hit on loader bucket. Dislocated 4th and 5th vertebrae in operator's back.

Operator was loading a rock truck, on his second pass to load his bucket he came out of the seat hitting his face on the windshield
