An employee was climbing a vertical ladder to access the head section of a conveyor to perform a scheduled bearing replacement. While climbing the ladder with 3 Points of Contact they lost their grip and fell backwards from their position, and the left torso area contacted the railing of an adjacent ladder.

Mostrando los 39EE was ascending a six-foot ladder to unclog an aspiration pipe. When reaching the 3rd step on the step ladder, EE slipped on ladder and fell to same level of floor ladder was staged at. During the fall, hard hat came off and EE struck head on the grated floor resulting in laceration to EE's head requiring sutures. EE currently off work at home recovering.
Employee was in process of sanitation tasks. Employee turned and took a couple of steps backwards and struck their foot against a toe guard which resulted in an uncontrolled fall at same level. Employee reached out with right arm attempting to stop the fall. Their right hand rested on top of an electrical enclosure and the impact resulted in a fracture to the humerus on right arm.
A Dozer Operator suffered a hand fracture on 1/13/23 around 9:00 am. As EE was descending from the loader's track pad by holding the handle with this left hand and as EE was stepping down to the lower step with right foot, left foot slipped from the track and they tried to grab the push frame from the dozer, hitting left hand and head on the frame.
While working on plattco valve 7089 underneath bag house 7888, tech was finishing up job, stepped back hitting head on the surrounding structure. As tech was turning around tech lost footing, cause tech to fall. As tech was falling tech tried to brace fall using right arm causing the injury.
The employee tripped over electrical wires that were being used to perform our annual grounding checks. The employee extended right hand to catch self from the fall which led to EE's right pinky finger contacting the grated flooring.
Employee slipped on a patch of ice when reporting to work station. Employee fell at same level on EE's right arm. Initially employee said no injury occurred but later sought treatment. On 4/2/2021 employee diagnosed with right forearm tendon strain and placed on restrictions.
EE slipped without falling on floor & felt discomfort in left leg. EE did not want treatment at time of incident. On 9/25/18 discomfort increased & employee sought treatment, diagnosed with a strain & placed on restricted work duty.
Employee missed the first step of a two step platform & struck lower left leg on the blunt edge of the grating on the step resulting in a laceration to the lower left leg requiring sutures to close.

Employee was properly dismounting CAT 416 backhoe. While stepping down onto ground level, employee "rolled" his ankle resulting in a sprain. Employee was evaluated by a physician and placed on light duty.

Walking into the shipper's office and slipped on some mud in the parking lot. When he fell, he strained his left knee.
On 12/11/12 at 5:05 am, EE was walking up the Line 1 stairs when he tripped on the 3rd step causing to fall forward. He wanted to hold onto the handrail, but hit the grading with the left hand. The EE received a 5th Metacarpal Fracture on the left pinky finger. The employee was holding a coffee mug on the right hand and was not holding onto the handrail with the left hand.
This driver was delivering to this location and tripped over a rock and fell.
Employee tripped on deck floor grating and while falling, reached out with left hand to arrest fall. The trauma from grabbing railing pulled something in upper left arm. (Employee is left handed)

EE unloading jugs from a trailer. While moving jugs the plastic strap EE was pulling on with left hand broke causing EE to lose balance. EE fell backwards while holding onto other jug strap with right hand and felt pain in right shoulder arm. EE did not want medical attention at time of incident. EE sought medical treatment on 12/18/09 and given restrictions/physical therapy.
While standing on a ladder (4 foot off ground) Employee stepped backwards and fell. He was holding onto the ladder handrails but due to his size he could not support his weight. Employee complained with knee pain. Initially classified as First Aid Injury by Doctor's office. Status changed to Recordable during follow up visit on 8-17-09, due to prescription medication.
An employee states he was climbing down from the hammermill when his right foot rolled off the edge of the grating causing his knee to twist inward. He claimed to be stepping down about 2 feet. He said that the pain was not all that bad and continued working. The pain continued for several days before he went to see the dr.
While walking across the concrete pad near the shop, the worker slipped on wet clay and sustained a pulled muscle to the groin area.
On 1-10-08 at 10:00 PM, a packaging operator was clearing a jam at the second level of the line 3 palletizer. The operator was attempting to carry 3, 10 pound bags of product down the steps from the second level. With both hands full, the operator missed the last step of the stairs and fell to the floor. The operator suffered a sprain / contusions on his right knee, and leg.
EE was coming down stairs & close to bottom of stairs & slip/trip on steps. EE sprained ankle. Initially treated as first aid. Became recordable on 12/13/2007 when EE was prescribed PT. and placed on light duty work.
EE was attempting to open a napco door from a stepladder. She lost balance and fell from stepladder falling approx. 3 feet to ground. EE fractured right wrist.

EMPLOYEE WAS DESCENDING STAIRS WITHOUT LOAD WHEN HE STEPPED DOWN IMPROPERLY CAUSING LOSS OF BALANCE AND ANKLE TO TWIST.

He has been treating this injury with non-medical treatments and it improved. However, he requested to see a medical doctor now because the knee started swelling again as he goes up and down stairs.

EMPLOYEE WAS EXCHANGING LP TANKS FOR HIS FORKLIFT WHILE IT WAS RAINING AND LOST HIS FOOTING AND FELL ON THE CONCRETE. HE HAD ABRASIONS ON HIS RIGHT ELBOW, BRUISED HIP, AND SAID HIS RIB CAGE WAS SORE. HE SOUGHT FIRST AID FOR THE ABRASIONS,AT THE TIME OF THE INCIDENT. HE WAS TAKEN TO THE E.R AND DIAGNOSED WITH A RIB FRACTURE.
