Mostrando los 34EE was up on the plant grizzly welding structure together. After running out of welding rod, stood up & proceeded across beams. When ee reached the big beam, foot slipped causing ee to lose balance, fall into straddled sitting position. This caused right leg to go one side of the beam & left twisted. Somehow striking knee on way down to other side.
Miner was climbing out of a transfer box on aggregate plant when their knee twisted.
Employee was tempting to put antifreeze in Cat740. Employee set container on step, then climbed up the steps, once up on the platform, lost balance forcing employee to jump off the platform. Employee landed on feet but fell/rolled onto left shoulder on the ground. Employee will Have to have surgery on left shoulder.
We had about 1 inch of snow on the ground. The maintenance crew drug a piece of metal across the snow, which packed the snow down tight. When employee crossed the path where the metal had been dragged EE slipped and landed on elbow.
The employee was getting off of a manlift into muddy conditions and slipped and twisted knee.

EE WAS EXITING CAT 216B SKID LOADER. HE STEPPED OUT OF MACHINE TO GROUND ON RIGHT LEG. BEFORE LEFT LEG REACHED GROUND HIS RIGHT KNEE CAP AREA POPPED CAUSING PAIN & FELL TO GROUND. DID NOT SEEK MEDICAL ATTENTION UNTIL FOLLOWING MORNING WHERE DOCTOR PRESCRIBED MEDICATION AND KNEE BRACE WITH LIGHT DUTY RESTRICTIONS FOR 5 DAYS.
RC60forklift lifting 1/4" steel plate 5'x13'with grab claw & 1/4" chain. C-clamp used to keep chain from slipping. Plate needed flipped end for end. When turning C-clamp came loose causing it to fall. Another ee clear, but ee pushed forcefully to ground in awkward manner. Plate rested partially across him. Another ee able to remove & get help.
OPERATOR MISSED STEP WHILE CLIMBING DOWN FROM THE CAB OF LOADER AND FELL TO THE GROUND.
Employee climbed up on a 55 gallon drum to retrieve a 3' length of 3" PVC pipe located behind the drums. The drum he stepped on tipped and he fell to the ground landing on his left foot fracturing his fifth metatarsal.
EE was doing maint. on crane his foot slipped off of step. The EE fell backwards striking tracks with leftside before landing on ground in a sitting position. Dr. said to take it easy over weekend and gave him medication for first few days. RTW Monday with no loss time or restrictions.
The miner was on a step ladder approx. 3 feet off the ground. He was attempting to tighten the drive belts on a drum dryer. While using a adjustable wrench, the wrench slipped off the turn buckle while he was pulling on it. The miner fell backwards off the ladder to the floor.
Employee working off 2.50' raised platform of 2"x12" board planks on back side of high speed screen, a board slipped & employee fell 2.50' to steel deck. Employee received a sprain or strain to his left wrist when he fell.
EE was sitting at his desk in a chair at the mine office. A loader operator for IMI came in the office to talk to EE. This is when EE had a seizure, loader operator said EE slumped over to his left side in his chair , and his right arm was straight out and stiff during the seizure. 911 was called, EE was taken to the hospital.
Employee was getting off dragline, stepped on track and slipped and fell between track and frame. He caught left arm on handrail.
While working outside, ee walked across an icy area. His feet flew up & he fell flat on his back knocking his hard hat off. He laid very still & possibly lost consiousness, but got up & continued working to finish the day. The next day was a snow day off & he continued having headache & soreness. He sought medical treatment on 1/29/09 & will be off work until 2/2/09.
An over-nite ice & freezing rain storm, made all areas slick and very hazardous.
EE was changing belts on the stone plant. While taking the guard off from around the belt, his foot slipped on the steel platform. He fell over backwards and fell 5-6' onto deck of crusher. Hit his head, elbow and back. Knocked his breath out, pain got worse over next few days.
Trying to clear splitmaster & fell, tearing left chest muscle.
While wiping down the inside walls of the dredge classifier, the EE was sitting on a plank with his feet on the floor. He leaned out to clean the walls surface and his hand (holding a cloth) slipped causing him to fall forward. The over-extension allegedly led to a muscle strain.
Employee was removing the spray bars while standing on the 8 inch beam that holds the screen deck, when his left foot slipped and he went to catch himself with his right foot & his ankle was turned. All of his weight was on his right ankle when he landed. The length of his fall was approx. 12 inches.
While stepping from the fore-deck of the M/V Delta Kelly J onto the river bank, the employee injured his right knee. It was a re-injury to an initial knee problem sustained while the employee was serving in the military service.

Truck driver for Nofziger Trucking was preparing tarp for loading of truck, he climbed up behind the cab on the steps and he slipped and fell. He struck his head, shoulder, hand, & elbow when he fell. Medical attention revealed a mild concussion.
They were changing screens on middle deck of stone plant. Injured was driving pins out and he turned around and stepped into opening. The hole was open so they could watch stone levels while draining bins. EE was not aware of hole being open. He caught himself while falling and 3 co-workers pulled him out.
While carrying a shaft part & climbing up stairs, the employee caught his toe on the first step. This caused him to fall forward & smash his right ring finger between the step & shaft, blowing the end out of his finger. EE needed 8 stitches.
EE was helping other ee change screens all the plant. He lost his footing causing him to slip and hit bump into a motor mount with his back.
Crusher was in final stage of reassembly & employee was attempting to move the material overflow chute back into position. He crawled on top of the unsecured chute to attach a Come-A-Long. When his full weight was on the chute, the chute moved from its unsecured resting position & fell to the ground 15' below. Employee landed on the chute as it hit the ground.
EMPLOYEE WAS STEPPING OFF A PIECE OF EQUIPMENT & WHEN HE TOUCHED THE GROUND HE TWISTED HIS ANKLE.
EMPLOYEE WAS CLEANING HOPPER OUT ON STONE PLANT AND SLIPPED ON ICE BRUISING HIS RIGHT KNEE. (CONTUSION)
