EE STATES THAT HE WAS PULLING A HOSE AND SLIPPED WHILE WALKING ON A LIME PAD. STATES HE JARRED RIGHT KNEE AND CAUGHT HIMSELF WITH RIGHT LEG SLIPPING UNDER HIM. EE DID NOT FALL TO GROUND. EE'S TREATING PHYSICIAN TOOK HIM OFF WORK ON 08/20/2007.

EE was walking when he stepped on a hose causing him to fall backwards. EE got up experiencing pain in his left knee. EE placed on restricted duty.
EE was working on a pump when he struck his head on an overhead pipe. He lost his balance and fell, striking his left leg against a flange and his right elbow against a pipe. On 07/23/07, EE was placed on severe restricted duty by his treating physician, no use of right arm. Not able to perform his regular duties, EE was placed out of the plant. First LW day will be 07/24/07.
EE was walking to the time clock prior to the start of his shift when he tripped and fell against a support beam. He struck his right arm against the beam and landed on his right hand. On 05/04/07, an MRI revealed a small fractured bone on his right hand. EE placed on restricted duty on 05/05/07.
EE's right foot slipped off of a walkway and foot slipped into hot condensate causing a second degree burn on his foot. Restricted duty.

EE was walking on the facility dock when he slipped and fell. He broke his fall by reaching out with his left arm. EE felt a pain in his left shoulder. On 04/27/2007, EE had surgery on his left shoulder. EE is currently losing time.
EE was walking in his assigned area when he stepped on an uneven surface, rolling his right ankle. EE was diagnosed with a fractured right ankle.

Diagnostic testing indicates that EE will require surgery to repair a rotator cuff tear. EE placed on restrictive duty on 05/02/2006.
EE was ascending a ladder. He stepped off the ladder rung and onto the landing. He missed the landing and landed on his right knee. On 04/04/2006, EE had surgery on his left knee to repair a torn meniscus. EE is losing work days.

EE was pushing on some steel rods when he slipped and struck his chin against a rod mill discharge chute. He sustained a quarter inch laceration which was closed by the plant doctor using super glue.
EE was walking on the into a building while it was raining. He slipped on a wet spot and fell. He broke his fall by extending both arms out. He felt a pain in his right shoulder. Diagnosis was a torn rotator cuff. EE had surgery on 12/26/2005 to repair the torn rotator cuff. EE started missing days on 12/27/2005.
EE had allowed mud to accumulate on his work boots. He was walking on a concrete pad and slipped as result of the mud accumulation. He felt a pain in his left groin area. His personal physician diagnosed him with an inguinal hernia. Surgery to repair will be done on November 10, 2005.
EE was using a valve handle to open a six inch plug valve. The valve handle broke and the EE fell backward. He landed on his backside. He experienced pain in his lower back. He was taken to a local hospital for evaluation and the ER doctor took him off work for two days. EE is on restricted work duty.

EE sustained a severe contusion to his right shoulder. Treating ER doctor took EE off of work for one day. EE reassigned while on work restrictions.

Employee struck his head, right shoulder and right leg on the airslide. On 03/09/2005, employee was taken off work by his treating physician.
On 03/01/05, employee fell against some structural support beams and striking his right rib cage. On 03/07/05, employee was placed on work restrictions. No ladder climbing, no lifting > 10# with right arm and no use of sledgehammer.
Employee was stepping over a pipe when EE twisted right ankle. On 02/08/05, plant doctor applied air splint to immobilize the ankle. Working with restrictions.
Employee was descending a stairway. When he stepped off the last step, he slipped on some mud and twisted his left knee. His personal physician took the employee off of work on 01/22/05.

On 09/02/2005, plant physician administered a steroid injection to the affected finger. On 1/23/2006, EE had surgery on right hand. Surgeon found cyst on the bone; removed cyst. Returned to work on 1/27/2006 with restrictions.
A Building Trades Mechanic slipped and fell while attempting to step over a four inch portable pump suction hose. The employee's fell toward the portable pump and struck his head against the pump. Employee was wearing a hard hat. His head glanced off of the pump and he struck his left ear against the pump. This resulted in a laceration that required sutures to repair.
Employee was walking down a stairway. He misjudged the distance to the last step and overstepped directly onto the concrete floor. X-ray reveals a hair-line fracture of an ankle bone.
A Mobile Equipment Operator had been working inside of a ship hold collecting bauxite with a pay loader. As he was ascending the ship hold ladder, to exit the hold, a foot slipped and he lost his hand grip on the ladder rung. He fell approximately five feet, landing on his right side. He climbed out of the ship hold and reported the incident. Hairline right rib fracture.
EE was moving a section of grating in place on a platform. He decided to do it on his own and not wait for help. EE was tied off to a handrail with one lanyard & was using the other to drag the grating. He pulled the grating too far & it fell 15 ft, causing him to be pulled down against the existing grating he was standing on. EE was released to return to work the following day.
EE was walking to his office at the beginning of his shift. He stepped on some slippery material on the roadway and landed on his right knee. Eventually had surgery on his right knee on 09/16/2004.
EE was on clean up duty. He was using water to hose down the deck of a bauxite unloader. He slipped on the wet floor & reached out to an adjacent handrail to break his fall. He felt a pain in his right shoulder & wrist. Plant doctor diagnosed as a mild shoulder and wrist sprain. Plant doctor prescribed prescription strength medication for pain as needed.
