The employee attempted to step on material at the bottom of a rib after the employee completed identifying (spray paint) the rib. Employee fell and bruised back.

Mostrando los 9EE pulled off to the side of the road and exited truck 177 to add salt to the load of stone. It was 2 degrees outside and EE got ice on EE's shoe and when EE went to step down and EE fell straight down off of the step and hit the ground where EE was immobilized. 911 was called and an ambulance was sent.

EE was exiting the passenger side door of a Co-workers pick up truck and while swinging his legs toward the door felt an excruciating pain in his lower back.
Employee stepped off a conveyor onto wet material,causing him to lose a firm foothold.Employee was taken to the shop,where his ankle was iced for several hours.Employee stated that he wanted to see a physician.Took employee to a physician, where it was found that he had a sprained ankle.Employee returned to work on 5/2/12 @ 6:30 a.m.
Employee was in route to adjust feed gate on hopper. Upon climbing a two rung step, the employee's right boot slipped off top rung with his leg sliding down the rung and edge of catwalk. The edge of catwalk came in contact with inside right leg between knee and ankle lacerating the skin. Employee was treated and released from the local clinic with no restrictions.
