EE was walking along inclined catwalk during freezing temperatures. Ice had accumulated on catwalk causing slick conditions. EE slipped and fell resulting in injury to knee.

Mostrando los 32The employee was climbing down off of a railcar and made it to the ground and slipped. They cut left leg on an unknown object. They reported to their supervisor and was taken to local emergency where they received stitches.
Employee was ascending steps at #4 spray dryer area to take the chute out of the car so it could be dropped down. As employee was ascending the steps they tripped and sustained an injury to right knee and right hand. They will have to see and orthopedic surgeon about their hand.
When working on mill chute EE turned to pick up tool, slipped and fell resulting in bruises and soreness. EE was seen by Dr., released w/ no restrictions on 7/24. Fllw up w/ 2nd Dr. on 7/29 released w/ restricted duty for lifting & tugging.EE has fllw up Dr. week of 8/4. EE has history of back problems prior to employment.
Employee was inspecting a drain pipe at track area, while standing on a mild slope. They slipped and fell, attempt to break the fall with their hands. They felt sharp pain in right shoulder.
Employee was going to swap lines on top of C tanks and stepped in a 6" trench that had been dug at the base of the steps for water to drain. Employee felt stress on hip and groin area. Employee initially didn't want to go to doctor but pain worsened and on 2/5/25, employee wanted to seek medical attention. Was treated for strain and put on light duty restrictions.
An employee was getting off of the Track Mobile and stepped off onto a piece of loose tin that had been blown off the track shed and was laying on the ground, causing EE to fall to the ground on EE's knees injuring EE's left knee.
The employee was attempting to dismount the right side of the forklift when EE's feet got tangled as EE was getting off and EE began to fall. In an attempt to break EE's fall EE grabbed onto the side handle and hurt EE's back.
EE was returning to the Wash Plant from the Press Plant. Exiting the building, EE slipped in mud created by rain. EE informed management 2 days later. EE went to the Dr. and received a shot of Cortisone and returned to work. There were no lost days of work resulting from this incident. EE has a prior history of back problems unrelated to EE's employment here.
Employee was walking back into the operator control room and tripped on the lip inside the door causing EE to fall on concrete floor. EE initially thought was ok and administered first aid. Next day, EE observed bruising and swelling and wanted to seek medical attention. On 5/28, EE received negative X-rays and was released to return to work with restrictions.
Employee was exiting a 3ft.trench where 24 inch pipe was being installed. As EE was walking up the trench, EE's left knee give away. EE continued to work until 5:00pm. When EE returned home, later that evening, EE's knee started to swell and become painful.
Employee was assisting coworker tilt a 55gal drum so forklift forks could get under and pick up drum to move from a broken pallet. While tilting the drum, employee lost footing, tripped and fell. This incident was initially managed as a first aid but employee continued to experience pain and wanted a medical evaluation on 2-19-24.
Employee had gotten a clay sample earlier and did not hang the wire rope back over the handrail per work instructions. Employee was checking weights on the #3 bagger and headed back to check bagger #2 and tripped over the wire rope that was partially in the walkway on the platform.
As the Excavator operator started to access steps to enter cab, the operator felt dizzy. Operator stepped back to ground level and leaned on tracks to regain stability. At that time, the operator was taken to the hospital for observation which resulted in a Medical Complication (low Blood Pressure).
The IW was climbing up a hopper car to release the brake and left foot slipped. IW tried to catch self, with right hand and IW's right foot slipped as well. The employee fell onto the track with lower back and fractured the L2 and L3. When the events occurred it was raining and cold.
Driver was climbing into truck and rolled off bottom step onto ground - on back and left shoulder
Employee was climbing down a ladder and pulled a muscle in EE's back. Continued to work throughout the year. Decided to have a procedure on EE's back on 11/10/21
Slipped on loose gravel, twisted left knee - swollen and torn cartilage.

EMPLOYEE WAS STEPPING DOWN FROM SPOTTER TRUCK AND STEPPED INTO A HOLE TWISTING EE'S RIGHT ANKLE RESULTING IN A SPRAIN.
EMP HAD A PREVIOUS RIGHT KNEE INJURY AND HAD RETURNED TO WORK. EMP WAS WALKING INTO BREAKROOM TO GET A CUP OF WATER WHEN EE'S RIGHT KNEE BUCKLED CAUSING EE TO FALL ON THIS KNEE, RIGHT ARM AND RIGHT SHOULDER CAUSING CONTUSION AND SEVERE BRUISING.

Employee had grease on the bottom of boot and slipped getting off a forklift. Employee reached around with employee's right arm to catch self with the forklift and sprained employee's right shoulder.
EMPLOYEE WAS COMING OUT OF SEPERATOR AT LOADING RAMP AND SLIPPED ON WET CLAY, FALLING AND HURTING RIGHT SHOULDER REQUIRING MEDICAL ATTENTION AND PAIN PRESCRIPTION.
EMP STEPPING OVER SILT FENCE TO TAKE A WATER SAMPLE WHEN THEIR WEIGHT SHIFTED CAUSING THEIR FOOT TO SLIP IN MUD AND TWIST THEIR RIGHT KNEE RESULTING IN PAIN. EMP DID NOT THINK THEY NEEDED TO GO TO DOCTOR AT THAT TIME. PAIN BECAME WORSE OVER TIME AND NOW IS NEEDING SURGERY.
EMPLOYEE TRIPPED AND STUMBLED OVER A LUMP OF CLAY CAUSING PAIN IN EE'S LEFT KNEE AND BACK.
WASHING UP AREA AFTER END OF SHIFT - EMPLOYEE SLIPPED ON WET FLOOR RESULTING IN LEFT KNEE SWELLING AND STRAIN TO RIGHT HIP.
EMPLOYEE SLIPPED ON WET CONCRETE AND FELL STRIKING HIS FACE AND RIBS RESULTING IN CONTUSIONS.
