On 3.4.2021 employee was turning around to change the direction employee was traveling by foot and stumbled from ground level and landed on left shoulder. Employee was released for full duty 3.4.2021. Left shoulder has been irritating employee and employee returned to Doctor on 3.29.2021 and was then placed on restricted work duty.

Mostrando los 8Employee was in route to clear an obstruction in a surge bin; while ascending a vertical ladder the employee lost their gripping and fell to the platform below. This fall was approximately 9 feet, and employee landed on right knee which caused a laceration to that knee. Said ladder is in good condition, with a safety gate at top landing, and has a proper back cage installed.
Employee was assisting the maintenance crew change bags on the bag house and it started to rain so they set the cover to the access hole back on top of the hole to wait out the rain, after doing so employee stepped on cover which was not secured. Upon doing so the cover tipped upward resulting in the employee's right foot falling into the access hole.

Employee slipped on some hard mud after exiting the Secondary wet plant screening tower.
Employee went to step on to a bulk truck EE was loading and missed the last step of the safety cage causing EE to lose balance and fall on to EE's left shoulder.
Employee was walking across the employee parking lot to the maintenance truck and slipped on a patch of ice and fell to the ground. A fresh snow fall had covered some icy patches.
