Employee climbed a ladder to check the feed box in the plant the ladder moved causing them to hit their right side ribs on the wash box. Employee then climbed down the ladder and reported the accident.

Employee was servicing the miner setting bits when employee slipped on a muddy surface (incline) falling backwards onto right lower back. Employee stated back popped and pain started. The employee was transported to the local E.R.
EE stated was carrying metal roof straps to the front of the bolt machine when EE stumbled and fell. When EE fell, EE landed on the metal strap resulting in a laceration to the right forearm. EE was transported to Harlan ARH then transferred by air evac to Pikeville medical. A tourniquet was applied by air evac immediately prior to flight.
EE STATED THAT EE HAD FELT EE'S KNEE SHIFT TO THE LEFT WHILE PICKING UP BELT STRUCTURE IN MUD. EE STATED THAT EE FELT PAIN AND CONTINUED TO WORK. ON 9-20-21 EE SAW A DOCTOR THAT PUT EE ON LIGHT DUTY PENDING SCHEDULING AND RESULTS OF AN MRI.
EE was assisting in working on the take reel of the roof bolter. EE had been positioned on the top of the roof bolter and was getting down when EE twisted EE's knee. EE continued to work until 12-16-20 when EE had surgery for a torn cartilage.
Employee stated was loading roof straps into the scoop bucket when employee's foot slipped causing pain to right knee. Employee continued to work until 12-9-19 when employee had surgery.
WHILE EXITING THE 544 LOADER THE EMPLOYEE SLIPPED. THE EMPLOYEE GRABBED THE HANDRAIL WITH LEFT HAND CAUSING A "JERK". THE EMPLOYEE FELT PAIN IN LEFT SHOULDER AND NECK. EMPLOYEE CONTINUED TO WORK UNTIL MARCH 2ND, 2020 WHEN EMPLOYEE SAW A DR. AND WAS TAKEN OFF WORK UNTIL NEXT APPT.
Employee weed-eating beside sample building area on the bank between beltline & the clean coal wall. Employee stated may have twisted ankle, and may have been tripped by honey suckle vine around foot. Employee completed shift and did not seek medical attention.
Exiting truck bed, foot slipped causing fall. Landed on right side. Employee was taken to doctor and released. No work missed until 11-1-17. 10-30-17 employee visits doctor. Doctor states there may be inflammation in right arm & prescribes medication. Employee returns to doctor the evening of 10-31-17. Doctor admits employee for IV fluids & ultrasound. 11-3-17 Employee is released.
