EE was stacking coal with a 980G loader. As EE backed up EE dropped off a pile of coal that EE missed that was 2 feet tall. EE's seat bottomed out and stubbed EE's lower back.
EE was getting off dozer & put all weight on left foot to step down and it popped behind EE's left knee
Employee climbed a ladder to check the feed box in the plant the ladder moved causing them to hit their right side ribs on the wash box. Employee then climbed down the ladder and reported the accident.
Coal on the push arm, dismounting the machine incorrectly.
While grinding / cutting a piece of 1/4" thick angle iron metal, the grinder jumped/kicked back and struck the left hand.
Miner picked up a 4lb hammer and EE's back hurt.
A truck driver observed EE standing beside the loader pulling pants up. Then in another moment & on a second look driver noticed EE lying face down on the ground. The driver notified others and activated emergency procedures. EE appeared lethargic, sweating, chilling and in pain. EE was transported by ambulance to emergency room.
Between haul road curves 6 and 7 the truck went into a ditch.
Employee weed-eating beside sample building area on the bank between beltline & the clean coal wall. Employee stated may have twisted ankle, and may have been tripped by honey suckle vine around foot. Employee completed shift and did not seek medical attention.
Employee was removing forms from the inner wall of the raw coal tunnel. While making a move to the next panel he was relocating his positioning hook at the same time the loose panel fell causing him to loose his balance and fall from a distance of approximately 6 feet.
