Mostrando los 47Employee was tasked with installing a vulcanized splice in 17-6 conveyor belt. As part of the assigned task, employee tripped which caused them to fall forward and strike their mouth against the cooker. Contact resulted in a broken tooth and laceration to employee lip.
EE reports walking to open valve to fill water tank, when right foot slipped on slippery ground causing right knee to twist while trying to maintain balance and not fall. EE describes right knee pain that prevents normal movement and prevents ability to work.
EE was exiting the truck to clean windows and mirror on R side of truck and when EE stepped on the lowest step the step gave way and the EE fell hitting back and bottom on the ground and head on the bottom front tire. EE seen at ER w/no restrictions. On 12/15/25 EE had lingering complications. On 12/17/25 diagnosed w/fracture of L shoulder, not able to RTW until reevaluation.
Fell into a washout pipe while connecting it, with a dislocation of left shoulder
EE reports that EE was walking in the mine area and stepped on a spot that moved under EE's foot causing foot to twist. EE reports pain in foot and unable to walk due to pain. EE states that EE has recent problem with foot, but that the current pain is much worse.
EE while in the process of removing debris/sticks from sleeve of pump, EE tripped over a plate that is part of the sled and fell backwards into a washout landing on left side. EE reports neck, shoulder, mid & lower back pain with stiffness. EE reports previous back injury.
EE reports walking between truck and dragline and turning right ankle by stepping on uneven ground. EE states that EE reported it immediately and had applied ice to the area without relief. EE was brought to the clinic by supervisor. EE unable to walk normally due to pain and swelling to inside and outside of ankle.

EE reportedly stepped into a small hole of missing grating causing a twisting fall to the decking floor. EE landed flat on back. EE found by ERT standing holding onto desk for support in control room. EE stated back pain that prevents normal walking. EE refused ambulance transport but did request transport to ER for evaluation.

EE reports feeling a popping sensation in the right knee upon stepping down from bulldozer in the process of hooking up a cable to the pit pump. EE presents to Clinic with notable antalgic gait. EE unable to fully straighten leg due to pain. EE states remote history of injury to same knee.
EE reports right knee pain after twisting into position getting in the CAT 980-wheel loader. EE reports that the knee has not improved with ibuprofen and ice and there is additional swelling.
EE was working from the bed of a truck and hopped down to the ground. EE experienced pain in right knee with swelling. EE states that the pain is increased with pressure and motion. Restrictions were given that cannot be accommodated. EE is off work until further notice.

Employee EE Reported to Clinic on 04/02/2024 that they stepped into a hole and started having lower leg pain. EE was given first aid and OTC medication for pain and returned to work. Next day EE was referred to Co Care due to still having pain. Restrictions were given that could not be accommodated and EE is off work until further notice.
Employee (EE) reported to the Clinic on 03/21/2024, states while stepping down backwards they missed the step and fell back on to their left hand and reported left wrist pain. EE was referred to Company Care for Evaluation. Restrictions were given that cannot be accommodated. EE is off work until further notice.

Fuel technician fell 3 to 4 feet to the sand while fueling a Bulldozer. Policy was not followed; dozer was not shut down and key removed. Bulldozer operator seated inside cab bumped the forward shifter casing the Bulldozer to move 6 inches forward. Fuel technician did not follow proper egress and climbed the Bulldozer track from the rear of the machine.
An employee (E1) stepped out of the truck onto concrete, unaware it was icy. E1 slipped and started to fall. E1 landed with left arm extended straight out and felt instant pain in the left arm. *Fracture diagnosis was found 1/4/2023.

While walking dragline, EE was utilizing tractor to pull cable. Employee was descending off tractor while still on ladder, EE's left knee buckled. EE noticed slight discomfort but had good mobility. When EE awoke next morning EE's knee was swollen & had moderate pain. EE was sent for evaluation on 9/27 & placed off work. EE had previous injury to knee
EE was walking from pit car and stepped into a hole. EE stated EE did not hurt just wanted to report the incident. The following day EE did not report to work. EE was sent to Company Care for an evaluation. EE is off work due to restrictions which cannot be accommodated. Similar incident occurred 10/18/21.
Employee was walking in the area of the traveling water screens and the grating fell through the support causing employee's right foot to go into the resulting hole. Employee states that employee's left leg kept employee from falling all the way into the hole. Right ankle discomfort and placed out of work.
EE reports EE was walking in tunnel & stepped on uneven area & noticed lower back pain, later noticed right foot pain. EE went to emergency room, while there EE contacted EE's supervisor to report injury from previous shift who instructed EE to call clinic and report injury. EE off work due to restrictions which cannot be accommodated, seated tasks only.
Employee was removing a carton of parts from a cart and twisted EE's knee when EE turned to walk away. EE did not realize EE's foot was caught under the cart.
EE was walking down stairs toward East & West bins when EE got to the last two steps EE fell rolling EE's left ankle.

Employee was clearing area and got a tree on the track of bulldozer, when EE got out of cab to get tree off EE slipped off cleat on the track and experienced pain in right knee. EE's pain increased throughout the night. EE able to ambulate, but states that it is sore. EE reported to clinic 2/3/20, EE had surgery on 8/21/20.
As employee (E1) was descending a stairway, while exiting a building, they over stepped the bottom stair, catching the edge of the step with the heel of their left foot. As forward motion continued the left foot began rolling outward causing E1 to fall to the ground. E1 was transported to a medical facility where x-ray's revealed a break and fracture of left ankle
Equipment Operator (E1) had been assigned tasks involving the operation of a D6 dozer. While dismounting the equipment E1 placed left hand on the right hand rail which had some spots of grease on it. E1 slipped and fell making contact with the dozer blade. E1 reported pain in left shoulder.
Employee broke for lunch and deviated from the JSA compliant travel path. The path chosen by the employee had slip, trip fall hazards. EE as a result slipped and fell and struck the back of head on the concrete surface. EE blacked out temporarily and as a result was transported for medical evaluation which determined that employee had a mild concussion.
EE was stepping off of steps onto sled, left foot slipped causing them to land on their bottom. EE states that their back & left hip hurts and feels as though this fall has made a pre-existing back injury worse(that they continues to receive treatment for) EE was seen by company doctor June 20 and placed on restricted duty until seen by their physician on June 28.
Mine Refuse Attendant climbed the steps of a Volvo A40G haul truck to observe the material in the bed for a dump location. While descending the steps, the attendant lost footing on a step approximately 38 inches tall and fell to the ground. The ground consisted of compacted reject material.
Employee (E1)had been tasked with operating a 50-ton haul truck. E1 parked the truck and while dismounting placed right foot on the ground. As E1 stepped downward and positioned left foot on ground, the right ankle began to roll outward resulting in a fall to the ground. At local medical facility x-ray revealed a right ankle fracture.
Employee was walking along #5 Matrix line on uneven ground when left foot rolled under weight fracturing tibia and fibula above left ankle. Onsite emergency personnel responded to the scene and dispatched EMS to transport the employee to a local medical facility.

EE had exited cab of Komatsu excavator. EE turned & was holding the handrails to dismount. EE dropped EE's right foot to step on the step. Employee was going upstairs to measure raw bins, foot slipped on stairway which resulted in employee falling onto right leg just below the knee.
EE was walking under the washer and right foot caught in grating causing ee to fall. EE struck right back against concrete pedestal base as ee fell. EE was treated at onsite clinic & returned to work. On 2/18 EE had back pain and went to ER for treatment. EE was given pain meds and instructed to return to work on 2/21 (two days off).
Employee was walking beside the conveyor in the west tunnel when ee slipped on ice landing on ee right side.
