Operator was working on crusher shaker screens clearing debris. When task was complete EE was stepping down to walking platform and twisted EE's right knee. Heard a pop. EE went home and the knee began to swell and hurt EE. EE reported it before leaving that morning of the 1st but didn't think much of it.

Employee lost balance and fell cutting hand on a piece of metal grating causing a laceration in right hand that required stitches. Employee was released back to work same day with no restrictions.

Mechanic was exiting A60H haul truck lost footing slipped and fell to the ground. landed on shoulder and side. Hurt shoulder went to urgent care got x-rays nothing visible in x-ray will return for follow-up. When swelling goes down.

Trip & fall to knees

Employee fell while doing maintenance on wash plant and landed on Right hip causing pain, employee finished out shift, returned to work the next morning and left in afternoon to go see Dr. Employee has a deep tissue bruise and is being treated by a Physical Therapist.

EMPLOYEE WAS DOING PLANT ROUNDS AND STEPPED ON UNEVEN GROUND FALLING AND FRACTURING RIGHT FIBULA

Conditions were good with mild temperatures, dry footing and good lighting. The accident occurred while the miner was installing a new belt on a belt press. There was an identified opening in the working platform that the miner was aware of and mis-stepped and fell through the opening. Operations were impaired during the time the miner was cared for and EMT's were on site.
