While mining towards the head on the 7W longwall face EE stepped over a relay bar and EE's foot slid on a piece of slick slate causing EE to fall and strike EE's left knee on a relay bar resulting in a contusion. No problems with equipment or compliance issues, training or PPE

Employee was hosing around the tail roller of surface belt when employee tripped on a piece of grading injuring right knee. Employee continued to work until 08/11/2026 then sought treatment. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was walking back to the Miner when employee slipped and reached to stop from falling and caught small finger on rib strap. Mining systems was not a factor, compliance with rules and regulations was not a factor. PPE was a factor.

While walking down 10 belt at 75 block observing 2 employees carrying an air drill a rock fell from the top and struck EE, knocking EE off balance. EE fell to the ground on top of EE's left shoulder, resulting in a contusion to EE's shoulder. No problems with equipment or compliance issues, training or PPE.

Injured exiting 126 mantrip at 37 block Main West with seat in raised position, the seat fell and caught their foot causing them to fall and injure neck. Went to doctor on 06/30/26 and started missing work. No problems with equipment or compliance issues, training or PPE.

Weekend warrior slipped while walking down stairs from the plant feed belt, re-injuring lower back. Sought medical attention 6/28, missed 6/29, returned 7/4 missing 1 day of work due to a muscle strain of the lower back. No problems with equipment or compliance issues, training or PPE.

Employee was exiting a Loader and slipped and landed on the Loader injuring their groin area. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor. The employee continued to work unrestricted until 7-22-26 then sought treatment.

IE was part of a crew building cribs on top of a steel set. A post was being hoisted up on top and struck a piece of v-deck that IE was standing on, causing EE to fall through. Injury was not the result of a training or compliance issue.

IE was helping the crew supply the miner. EE slipped on a channel plate while standing on the bumper of the miner and cut EE's hand trying to catch EE's balance. Injury was not the result of a training or compliance issue.

Employee was standing on the back of the miner, reaching up and fixing a tube chain, when EE slipped and twisted EE's left knee. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was walking down the 800 Belt catwalk "spotting" a roller for change out, when EE slipped on loose coal spillage and fell backward on EE's left leg causing a fracture to EE's fibula bone. Employee is experienced and was wearing required PPE. No rules or regulations were violated.

Employee was scaling rock down from top when a piece of rib started to come down. While EE was moving away from rib EE slipped and fell injuring EE's knee and ankle. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was walking to center bolter when EE slipped and fell, EE tried to catch self and cut the top of EE's hand on a piece of slate. Mining systems was not a factor, compliance with rules and regulations was not a factor, PPE was a factor.

Injured employee was carrying a belt roller with both hands along the 7 North #1 Belt between 24-25 blk. when they tripped on a piece of loose coal and lost balance. They fell against the side of a roller cradle, smashing right hand between the cradle and roller they were carrying. Suffered a contusion & swelling. Was wearing required PPE including metacarpal gloves.

Employee was cleaning at belt tail when EE stepped into a hole and twisted EE's ankle. EE also fell and injured EE's wrist. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Injured was attempting to dismount Volvo rock truck with wet muddy boots and EE's foot slipped while climbing down steps causing an unexpected load on EE's left shoulder resulting in a sprain/strain type injury. Injured started missing work on 3/13/26 and was put off work pending an MRI. No problems with equipment or compliance issues, training or PPE.

Injured was carrying tools past 2 parked mantrips in 8W belt spur. While walking between mantrips EE stepped on an uneven surface causing EE to fall. Injured started missing work on 2-18-26. No problems with equipment or compliance issues, training or PPE.

Injured Employee was walking beside a parked Shuttle Car and slipped on loose coal (sloughage) and fell against the rib. They stuck right forearm out against the rib to catch themselves. Forearm hit a spad that was in the rib causing a small puncture and a cut requiring 3 stitches to close. Employee has training and experience. Returned to work next scheduled shift.

Injured employee was walking and tripped on a crib block causing EE to stumble and roll EE's right ankle. Has been diagnosed with a torn ligament. No rules or regulations were violated and employee has training and experience.

Injured was walking away after throwing switch at Sugar Run slope back switch when EE slipped on the rail, falling onto the point rail. Injured started missing work on 2-19-26. No problems with equipment or compliance issues, training or PPE.

Employee was was on the 2 Left Longwall face and stepped in a hole at 7 shield, resulting in a severe sprain to left ankle. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was walking along the 8 South Back Switch Track, when EE tripped over the rail and fell, striking EE's right knee on the rail. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Injured was hanging a run through curtain up in #2 entry of St Leo Mains 131-132 block, standing on the third rung of a 4' ladder leaning off the ladder to install a nail through the curtain when EE fell off the ladder and bruised EE's back when landing on the mine floor. No problems with equipment or compliance issues, training or PPE.

Employee was working at power center when EE stepped back and twisted EE's knee. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was stepping out of the diesel locomotive they had parked. They stepped off the locomotive as they reached for a handle for support but the handle wasn't on this locomotive. Lost balance and fell landing on right shoulder. Has muscle damage in the shoulder. Employee has training and experience. Was wearing required PPE.

Injured employee was hanging a check curtain on the 7 North Mains CM Section. EE used an 18" high work platform so EE could reach the mine roof. While standing on the platform, EE pulled up on the curtain, lost EE's balance and fell off it, landing on EE's right shoulder. Result was a dislocated shoulder. Employee has training and experience. No rules or regulations were violated.

Employee was walking along trip of cars when EE sprained right ankle. PPE was not an issue, mining systems was not an issue, compliance with rules and regulations was not an issue.

Employee was getting roof strap off of miner when EE fell backwards and strained EE's knee. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was climbing down off of 777 dump truck, when they slipped off of bottom rung on ladder. They felt a pop in knee. Employee worked after the injury until March 4th 2026, when they began missing time for surgery.

Injured standing in first crosscut outby 8 west equipment doors near the end of loose take up rails while a scoop was unloading a double stack of take up sections on top of the loose rails. Duckbill drug across a loose rail, kicking it up when it got to the end of the rail striking injured, knocking EE down and landing on EE resulting in a fractured vertebrate.

Injured employee was walking past the Longwall Stageloader toward the face when a piece of coal fell from the roof and hit EE on the back causing EE to fall against the side of the Stageloader near the #1 shield. Received a cut above EE's left eyebrow requiring 5 sutures to close. Returned to work EE's next scheduled shift. No rules or regulations were violated.

Injured employee was operating longwall shields when EE stumbled on a relay bar. EE put out EE's right hand to catch self but EE's hand landed on a bolt plate (Jennmar Mini Monster Mat), causing a laceration. Employee sought medical attention and the laceration was glued closed and bandaged. No rules or regulations were violated and employee has training and experience.

Having trouble fixing an air tugger, EE was on the platform and coworker just looked at them and had eye contact. Coworker turned to keep working with third person and they heard a noise and EE was on the ground. No one knows exactly what happened. The assume that the safety catch for the drill steel came out.

Employee was changing steels on pan line bolter when EE's foot slipped and was caught under the drill pod. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor. body positioning was a factor.

Employee was attempting to assist to move power center cars. Employee was walking into the track spur and slipped, tripped and fell over a piece of pipe.
