Employee was climbing down step ladder and slipped off second step causing EE to loose EE's balance, fell stumbled backwards and hit EE's head on the bumper of EE's truck causing a cut that required stitches.

Employee was entering cab of dozer and didn't see that floor board was missing. Employee's foot went in the hole and employee caught right side of rib cage against steering clutches.

Stepping down off Highwall Miner and twisted knee

Employee was walking across parking lot and slipped on the ice landing on their shoulder. Continued to work until 2/24/22 when EE informed me of the accident and that it was not getting any better. I instructed EE to go to hospital and they found that EE had a cracked shoulder bone.

Employee climbed down on a step ladder and when he got to the last step he stepped down onto uneven ground under the last step and hyper extended his right knee. Tore a tendon behind the knee cap.

Employee had just finished his shift and had parked the cat scraper he was operating. As he stepped down off the last step his foot got hung up in the last strap causing him to stagger back twisting his right knee.

The associate stepped onto the cantilever platform next to the conveyor structure the expanded metal grating gave way and the associate fell to the ground.

Party had got of his truck at the start of the shift to put a 6" pump line back together. It was down in the twenties temperature wise over night. The water on the ground froze. As he went to put it back together he slipped and fell injuring his toe.

EE was walking to his Front end loader and stepped in a small hole and twisted his knee. He worked the next day but decieded to take the 10th off to go to the doctor for an xray

Employee opened the door of his D11 dozer and stepped out onto the deck, he straddled the door to retrieve an air hose that was attached to the cab. The hose had been used for repairs. He went to throw the hose off and lost his grip and fell. He suffered a broken arm.

EE was taking off his coveralls when he slipped on the ice and pulled a muscle in his back.

EE was cleaning the tracks on his cat hoe, when he stepped on a rock and fell, landing on a rock. He cracked a rib.

Employee was climbing onto a D11 Dozer. He was stepping over from push arm to tracks and slipped and fell onto tracks. Went for xrays. Had a bruise on his rib cage.

Injured party was pushing rock in the shot, when he turned around to back up & turned back around he saw flames shoot out on both sides of the engine compartment on the tractor. Party exited the dozer & jumped off.

EE was fueling a rock truck at the end of his shift and stumbled over the hose and twisted his knee. He worked the rest of that week, and all of the next week, but on Monday morning of the 23rd he said he had to go have it looked at.

Employee was standing on the stinger of a 657 scrapper power washing. The hose of the power washer become tangled under the tire of the pan and EE pulled on it. When it came loose it caused him to lose his balance and fall backwards about 4'6".

Employee was climbing onto Trac Hoe and slipped. He cut his leg on the track. He received 5 stitches.

Correction to E- Document 781351 EE was to report to work on Thursday night , but was unable to because his knee was bruised from previous days Accident.

Employee jumped from his mechanic's truck to put a fire out and when he returned to his truck he noticed his knee was hurting. He went to his doctor and his doctor told him to be off for 2 weeks.
