EMPLOYEE STEPPED INTO A FLOOR DRAIN, THEREFORE CAUSING HIS FOOT TO SLIP INTO THE HOLE AND BRUISING HIS LEG NEAR THE SHIN AREA. THIS EMPLOYEE HAD VACATION SCHEDULED PRIOR TO THIS ACCIDENT, TH EREFORE HE DID NOT MISS ANY SCHEDULED WORK DAYS.

EMPLOYEE WAS INJURED AND DID NOT MISS ANY WORK THIS IS A REOCCURENCE OF OLD INJRY. EMPLOYEE WAS HELPING REMOVE ROCK FALL ON BELT LINE SLIPPED ON MINE FLOOR. THIS EMPLOYEE HAD KNEE SURGURY. EMPLOYEE WAS NJURED AND DID NOT NOT MISS THIS WORK IS A REORRENCE OF OLD INJURY. EMPLOYEE WAS HELPING REMOVE ROCK FALL ON BELT LINE SLIPPED ON MINE FLOOR. THIS EMPLOYEE HAD KNEE SURGERY.

SLIPPED AND FELL WHILE LOADING MATERIAL ON TO FORD 9000 SEEDER TRUCK INJURING RIGHT ARM AND SHOULDER.

OPERATOR STOPPED D-9H DOZER WHILE PUSHING MATERIAL TO LOADER & STEPPED OUT OF DOZER TO CHECK HIS WORK WHEN HE FELL. IN DOING THIS HE STOOD TOO CLOSE TO EDGE OF TRACK PAD ON DOZER & HIS FEET SL IPPED CAUSING HIM TO FALL ON TOP OF TRACK PADS WITH HIS KNEE CAP STRIKING THE EDGE OFT RACK PADS.
EMPLOYEE WAS IN THE PROCESS OF CHECKING HYDRAULIC FLUID LEVEL WHEN GOT HOLD OF HAND RAIL TO PULL HIMSELF UP THE HAND RAIL BROKE LOOSE AND HE FELL TO THE GROUND
