Mostrando los 47Employee was dismounting a 777 haul truck on mounted ladder located on the non-cab side. EE took a step onto the bumper ladder and when EE did, the steps swung, causing EE to throw off EE's balance. EE's right hand slipped off the ladder, spraining EE's middle finger.
Employee was walking approaching chemical batch station when EE slipped on dry concrete spraining right knee.
Foot slipped off the catwalk on excavator and went between the catwalk and the step. Employee's foot was caught and EE fell down off excavator, breaking EE's leg due to foot being stuck. EE then fell to the ground. Broke lower right leg.
Employee stepped on shaker arm and when employee went to step down, foot slipped on the wet wood and caused employee to fall back and hit the back of head and neck on the wall causing a contusion
Employee noticed dozer was not operating well and pulled out of dump area and requested someone to look at dozer. Then the engine caught fire and employee jumped approximately 8 feet to the ground landing on ankle and shoulder causing a strain/muscle tear on right shoulder.
Employee was climbing down the ladder of a CAT 777B Haul Truck at end of shift. EE missed placing EE's right foot onto the last step and EE's right foot slid to the ground, causing left shoulder to hold EE's body weight. EE's right hand slid off ladder because EE was in the process of taking the step and was removing EE's right hand to slide it down the rail.
Employee was dismounting the D10 dozer at the end of EE shift. While utilizing steps and per Employee "3 points of contact", EE slipped off step located on blade arms, landing on EE's Left Side.
Employee was carrying a case of water down to cleaning plant and EE slipped on fresh snow/ice landing directly on back, knocking the wind out of EE. Employee continued to finish out the shift picking rock. On 1/31 EE left work early to seek treatment because still in pain from contusion.
Employee was removing snow from the patrol pick up truck and suffered a slip when stepping on the running board.

EE came out of the man door of the welding shop and walked in front of the garage door to walk over to the demag bucket they are working on. EE walked on some ice and slipped. EE didn't fall down but caught self and did a half split. That's when EE pulled the muscle in EE's leg.
STANDING ON LADDER. WENT TO STEP DOWN FROM THIRD STEP AND FOOT SLIPPED AND SLID DOWN LADDER. DID NOT FALL TO GROUND (STANDING). Pulled neck.
Stepped on a small crate to get up to a higher pallet to count bags of coal for inventory. The small crate was empty and cracked when employee stepped on it. The crate flew up and hit ee and ee fell down and landed on the broken crate. Broke 4 ribs.
Climbing down off the D10 to do a walk around inspection, EE missed the step. EE was holding on to the grab handle to catch self when EE fell and EE's body twisted, hurting EE's shoulder.

Employee was using an electric welder to stick weld stainless steel sheets to a coal feed hopper when he slipped on a sheet of stainless steel, twisted trying to catch himself and fell backwards.
STANDING ON FRONT TIRE OF 966 LOADER WHILE PEROFRMING MAINTENANCE ON HYDRAULIC CYLINDER. CONDITIONS WERE WET WITH SOME OIL PRESENT.
EMPLOYEE WAS DISMOUNTING THE D-6 BULLDOZER FROM THE TRACK TO THE BLADE WHEN HE SLIPPED. BOTH TRACK AND BLADE ARM HAD WET SILT DEPOSITS ON THEM. HE RETURNED TO WORK THE NEXT DAY AND WORKED UNTI L 6/14/00. CALLED IN ON 6/15/00 AND SAID HE WOULD BE OUT OF WORK BECAUSE HIS BACK HURT. HE HAS NOT RETURNED.
EMPLOYEE FELL OFF OF TRAILER. HIT LEFT ELBOW OFF OF STEPS. EMPLOYEE SOUGHT MEDICAL ATTENTION AND X-RAYS AT LOCAL HOSPITAL.
EE WAS GETTING UP IN THE TRUCK WHEN HE SLIPPED AND FELL TO THE GROUND TWISTING LEFT KNEE AND LEG. LEFT KNEE HURT WHEN BENT. ON 2-23-00 EE SAID ABOIL HAD FORMED AND HAD TO BE LANCED.
