IE was hanging a tube chain and slipped. The chain was wrapped around EE's right wrist, when EE slipped the impact from EE's body weight fractured EE's wrist.
IE was washing shields, as the IE stepped between the shield toes on top of the relay bar IE's left foot became stuck. IE lost their balance falling backwards causing IE to fracture both the Tibia/Fibula. IE will require surgery to correct the fractures.

Walking down BC-101 belt line doing inspections and tripped over a water hose. Fell on right hand causing pain in the elbow shoulder.

The employee was hanging sample tubing. He was standing on a ladder that was leaning up against a roof support can. The ladder slipped and the employee's leg got twisted up in the ladder and he fell to the ground. Restricted duty 3-21-09.
The employee got off the John Deere Skid Steer, and stepped between the bucket and frame when his left foot slipped out from under him, and he twisted his right knee. He requested to see a dr on 3/3/09 and was placed on light duty

The employee was stepping down off a ladder after completeing tail chute work. He was holding a partial bag of rock dust in his arms. He missed the bottom rung of the ladder and fell to the ground straining his right knee in the process. MSHA restricted duty 7-23-2008.
The EE was getting down from the roof bolter. As he stepped down, he miss judged how far it was to the floor and when his foot hit the floor, his knee buckled and he fell to the ground. He finished out his shift and worked the next day's shift. He then wanted to see a dr. On 4/9/08 he was seen by a doctor and released to light duty. MSHS restricted duty on 4/9/08.
The employee was walking back to the longwall face after lunch when he slipped and fell down. He punctured his left forearm on a piece of wire during the fall. Sutures were required to close the laceration. MSHA Reportable Injury 4/3/08.
The EE was walking to the warehouse to begin her shift. She slipped and fell on the ice and caught herself with her left hand.

The employee was parking in the mine parking lot before work. As they went to get out of vehicle, they slipped on the ice and fell down. EE stood up and had a hold of the truck when they slipped again and tried to hold themselves up causing a strain to the back and groin.

The EE was standing on the cab of the shuttle car to hang a laser on the roof. When he went to get off, his boots were muddy and he slipped and slid off the shuttle car, scraping his back on the way down on the corner of the shuttle car. THE EMPLOYEE WAS ON A LADDER RAISING THE BELT LINE WHEN SHE REACHED FOR THE BELT RAIL & SLIPPED. AS SHE FELL OFF THE LADDER HER RT. HAND GOT CAUGHT IN THE COME-A-LONG CHAIN. SHE FRACTURED HER RT. 3RD FINGER. THE EE RETURNED TO HER REGULAR SHIFT.
The employee was filling the slinger duster from the air/dust tank underground. He was standing on the duster and had another employee turn off the dust and turn on the air to flush the line. The line surged and flipped up, causing the employee to loose his balance and fall off of the duster.
THE EMPLOYEE WAS IN THE BACK OF A PICK-UP TRUCK DISMANTLING A KENNEDY PANEL. HE WAS PULLING AND JERKING ON THE PANEL WHEN IT GAVE WAY. THE EMPLOYEE STEPPED BACK AND HIS LEG CAUGHT THE TAILGATE AND HE LOST HIS BALANCE AND FELL OUT OF THE TRUCK. **LOST TIME ACCIDENT** FIRST DAY OUT 12-2-04.
THE EMPLOYEE WAS TIGHTENING THE BOLT ON A PIPE HE WAS PUTTING BACK TOGETHER. HE SLIPPED OFF THE LADDER HE WAS STANDING ON AND FELL STRAIGHT DOWN. HE HIT HIS FEET AND THEN FELL OVER A BUCKET OF TOOLS. HE COMPLAINED OF LOWER BACK PAIN. ***RESTRICTED DUTY***
THE EMPLOYEE WAS UNLOADING BELT STRUCTURE WHEN HE PICKED UP A TOP ROLLER AND TURNED TO MOVE, STEPPING ON UNEVEN GROUND, AND ROLLED HIS RIGHT ANKLE.
THE EE WAS WALKING UP THE CROSS CUT IN THE 6 LEFT SECTION WHEN HIS RIGHT KNEE POPPED. THE KNEE SWELLED AND BECAME VERY PAINFUL. THE EE SOUGHT MEDICAL ATTENTION AND IS OFF WORK UNTIL HE CAN SEE A SPECIALIST FOR HIS KNEE. FIRST DAY OUT: 6/17/04.
EE WAS WORKING MAINTENANCE ON LONGWALL. EE WAS HURRYING TO GET UP ON TOP OF SHEARER. PUT 1 FOOT IN CABLE TROUGH & OTHER ON TOE OF SHIELD. FOOT SLIPPED OFF SHIELD & FELL TOGROUN TWISTING HIP & KNEE. CON'T TO WORK BUT HIP HURT. MRI 10/27/04 NORMAL. WENT TO SPECIALIST WHO DID EXPLORATORY SURGERY 3/31/05. LOST TIME ACCIDENT 3/30/05.
THE EMPLOYEE HAD JUST ARRIVED AT WORK & WAS GETTING OUT OF HIS PERSONAL VEHICLE WHEN HE SLIPPED ON ICE IN THE PARKING LOT & SPRAINED HIS RIGHT ANKLE.
EMPLOYEE WAS STEPPING OFF THE BUCKET OF THE GEHLAND STEPPED ON A PIECE OF COAL AND TWISTED HIS LEFT ANKLE. THE EMPLOYEE RETURNED TO WORK IN A RESTRICTED CAPACITY ON HIS NEXT SCHEDULED SHIFT 11-12-01.
EE WAS SHOVELING THE BELTS WHEN A ROCK BOUNCED OFF THE BELT. HE TRIED TO GET OUT OF THE WAY & FELL DOWN. HE STRAINED HIS LEFT SHOULDER & HIP.
THE EMPLOYEE STATES THAT HE WAS WALKING TO THE DUST TANK AND STEPPED IN A HOLE CAUSING HIS RIGHT KNEE TO HYPEREXTEND. HE HAD NO PROBLEMS FOR THE REST OF THE SHIFT. HE SAID HE HAD PROBLEMS ALL WEEKEND AND REPORTED IT ON 8-7-00.
