While ascending Cat D10T bulldozer, Asset #DZ032, serviceman lost footing due to muddy boots, slipped and LEFT rib area hit steel push arm causing soreness. Employee was taken to local Urgent Care for observation and diagnosed with LEFT 9th lateral rib fracture.
Employee was walking down a sloped embankment near the fuel tank and slipped due to muddy conditions and fell onto right shoulder. Employee did not miss any scheduled work or receive medical treatment until 1/31/2025, when employee had the shoulder operated on for rotator cuff damage.
Employee was climbing down the ladder on a CAT 777D rock truck. EE stepped off the ladder onto the ground and twisted left ankle. EE went to the hospital the next day and had an x-ray done on the ankle. It was diagnosed as a sprain.

Employee was getting out of the triaxle truck at the fuel pumps in the parking lot. Employee slipped on the step while getting out of the truck and grabbed the hand hold to keep from falling. EE felt a pop and then pain in the right arm. EE went to the emergency room and was diagnosed with a torn bicep muscle.
On 1/25/24 employee descended wheel loader #LO041, planted feet on ground and slipped. Employee then grabbed ladder w/ RIGHT hand to prevent slip/fall causing soreness to RIGHT arm area. Later on 1/29/24 employee went to local Urgent Care for observation and diagnosed with sprain to RIGHT acromioclavicular joint and placed on light duty.
The employee was getting out of triaxle truck at the bed wash. The employee's foot slipped when employee stepped onto the ground. The employee fell backwards onto employee's shoulder. The employee did not seek medical attention until 12/27/23. Employee received an x-ray on 12/27/23 & it showed employee fractured left clavicle.
Performing maintenance greasing portable screening plant, employee descended fixed ladder, slipped and slid ~4' down metal steps of ladder scraping belly and stomach area against steps, landing on feet. Employee refused treatment. Two days later, employee was sore, went to Urgent Care for observation and diagnosed with two fracture ribs with restricted duty.

Employee was walking across the cab deck towards the door of the rock truck and stumbled and fell towards the door. EE put out left hand to grab the mirror on the door and hit left hand on the mirror bracket between the little and ring finger causing a sprain.
On 1/19/22 while attempting to ascend truck, employee placed one foot on ladder, planted other foot, slipped on ice. EE then reached and grabbed hold of railing to prevent fall and hurt left shoulder area. Two weeks later EE's left shoulder/neck area was still sore. 2/02/22 EE was taken to Urgent Care for observation and diagnosed w/muscle strain.
Employee was descending ladder of haul truck at end of shift and EE's right foot slipped causing EE to lose balance. EE reached with EE's right hand to grab hold of handrail and felt pain in right shoulder area.
While accessing repairs needed in Buckwheat pocket employee slipped, reached with LEFT hand to grab onto something preventing fall and cut LEFT middle and ring fingers on piece of metal. Employee was taken to local hospital for observation and diagnosed with LEFT middle and ring finger laceration.

Employee could not reach ground and EE slipped and fell on their back. Employee said was OK. Employee did not seek medical attention until 2/16/2021. Employee said tore their bicep muscle.

Employee was going to grease a truck and had climbed down the ladder to the ground. EE had parked on a flat area near the fuel tanks. EE turned around after climbing down the ladder and tripped over some rocks, causing them to fall over backwards and they landed on left shoulder, causing a sprain.
While talking on phone to another mechanic, employee turned to begin walking back to fuel truck and feet slipped causing employee to fall to ground and felt pain in right foot and ankle area. Employee was taken to local Urgent Care and diagnosed with right ankle sprain.
On 7/22/2020 while turning a 12" x 21'L piece of steel pipe with beam turner employee reported EE slipped on wet floor and felt strain in lower back area but refused any medical treatment. On 10/16/2020 employee was taken to local Urgent Care for observation after complaining area was still sore and diagnosed with muscle strain and given restricted duty.
Employee was walking through the shop on the way to the change room. EE tripped over 2 large pieces of angle iron laying on the shop floor. EE caught the toe of EE's shoe on the angle iron, causing EE to fall forward. EE put right hand down to stop the fall and struck EE's hand on the concrete floor. EE fractured the little finger. EE did not seek medical attention until 6/25/2020.
While descending steps and carrying ladder in plant, employees right foot twisted causing foot to twist and become sore. Employee finished shift. The next day employees foot was still sore and was taken to local Urgent Care for observation and diagnosed with a sprained ligament.
On 11/01/19 employee was performing maintenance on Ripl-Flo vibrator, stepped on spring base pedestal, boot slipped and Right Knee impacted metal pedestal causing pain. Initially employee refused medical attention. Knee was still sore and employee went for MRI on 12/07/19 and diagnosed with possible hairline impact fracture.
Employee slipped and fell on ice at blast site. Did not seek immediate medical attention. Discomfort in left shoulder persisted. Diagnostic screening did not result in restrictions until 3/6/19. Originally reported to MSHA on 3/6/19. Report not found on-line. Resubmitting 8/15/19.
While performing maintenance pressure washing the 6030 SH002, employee slipped and lost balance causing fall to ground. Employee extended arms attempting to brake fall and felt pain in left wrist after impact with ground. The next day employee was taken to local Urgent Care and diagnosed with possible fracture and scheduled for MRI.

While entering lab, employee stumbled on metal grating and fell forward using both hands to brace fall which caused cuts to both hands and finger. Employee was taken to local Urgent Care for observation and diagnosed with laceration without foreign body to both hands and finger and received a total of 13 stitches.
Employee was flushing out the power washer water tank with a 2 inch water hose. EE was using bleach to clean the tank. When EE pulled the hose out of the tank, the pressure of the hose pushed EE back and caused EE to become unbalanced. EE fell approx. 3 feet from the top of the toolbox to the ground. EE fractured 2 ribs and pulled a groin muscle.
Employee parked haul truck at end of shift and was descending ladder to dismount machine and placed right foot on ground. While shifting weight onto right leg, employee felt pain and soreness to right knee area and visited local hospital for observation. Employee was diagnosed with a sprained right knee w/ possible MCL damaged.
Employee parked EE's triaxle truck in the parking lot of the 021 pit. EE was adding alcohol to EE's air tank on #9048 Mack truck. EE slipped on ice while walking across the parking lot. EE landed on EE's back causing bruising. EE was taken to emergency room as a precaution.

Employee was walking across the job site parking area from his pickup to the loader he was operating. He stepped in a washout and twisted his left knee. He did it again on 9/15/2015. He did not report the accident to his supervisor until 10/1/2015.
Employee was attempting to ascend steps on at coal load-out area and slipped causing right leg to twist. Employee was taken to local hospital for observation and x-ray and was diagnosed with a medial meniscus tear and put on restrictive duty until further notice.

He reached into his pocket to get a wrench and inadvertently stepped back and his foot slipped off the bumper area. He fell to the ground and landed on his right side, causing fractures to his ribs.

While attempting to mount the D9H bulldozer, employee turned around on the ground and felt a pain in his right leg. Employee was taken to the hospital for observation.
A 15 year old juvenile fell and injured his left ankle while tresspassing on an inactive part of the mine site.
