While bagging 40# bags of coal, employees hand slipped and struck metal clamp. Employee was taken to local Urgent Care for observation, received four stitches to LEFT thumb and diagnosed with laceration without foreign body.
Miner got out of the truck, put the hose down the hole to pump the water out of the drilled hole and their knee popped.
At the Truck Shop, EE was unloading flat steel from a flatbed truck. EE picked up the steel. The weld broke off the washer on the right side as one leg of the chain broke on the right side of the steel which left the whole sheet of steel fall on EE left hand, and causing extensive damage to EE's left ring and pinky finger. Surgery was successful
Employee hooked a 777 wheel station up to a crane. When the crane operator picked up the wheel station to set in place, it shifted causing the wheel station to pin the employee against the rear bumper of the employees' service truck. Employee received a small laceration and bruising to the right leg.
Employee while walking toward portable restroom (Job Johnny) located at Slate Hopper area, slipped losing footing causing fall onto RIGHT side and felt pain/soreness to RIGHT shoulder area. Employee was taken to local Urgent Care and diagnosed with sprain to RIGHT shoulder.
While walking to load next retail truck in loadout section of #14 Prep Plant, employee's wet boot slid on floor causing RIGHT ankle to "twist" while attempting to change direction. Employee was taken to local Urgent Care for observation and diagnosed with sprain to RIGHT ankle.

While ascending Cat D10T bulldozer, Asset #DZ032, serviceman lost footing due to muddy boots, slipped and LEFT rib area hit steel push arm causing soreness. Employee was taken to local Urgent Care for observation and diagnosed with LEFT 9th lateral rib fracture.
Employee was walking down a sloped embankment near the fuel tank and slipped due to muddy conditions and fell onto right shoulder. Employee did not miss any scheduled work or receive medical treatment until 1/31/2025, when employee had the shoulder operated on for rotator cuff damage.
Employee had been working on a CAT 777F and had just climbed down off the haul truck. EE threw an empty can of starting fluid onto the ground beside EE's mechanic's truck. The can exploded and a piece of the can struck EE in the face on EE's right cheek, causing a laceration. EE was taken to UPMC- Altoona for treatment and required stitches to close the wound.
Employee was climbing down the ladder on a CAT 777D rock truck. EE stepped off the ladder onto the ground and twisted left ankle. EE went to the hospital the next day and had an x-ray done on the ankle. It was diagnosed as a sprain.
Miner tripped over rocks and mud and fell on left hip
Employee was performing maintenance on Volvo A40D, asset #TR403, and dropped belly pan. While working to assess for repair, bolt broke that holds clip securing safety chain of belly pan causing metal to impact LEFT index finger. Employee was taken to local Urgent Care and diagnosed with laceration to LEFT index finger and received two sutures.
Employee stated on 3/07/24 while bagging coal and working on restacking pallets of damaged 40# bags of coal when employee felt soreness to RIGHT shoulder area. The next morning on 3/08/24 employee reported incident, was taken to local Urgent Care for observation and diagnosed with strain of muscle and upper arm of RIGHT shoulder area and given restrictions.
Employee called off work 3/05/24 stating "last night I had piece of metal removed from eye, I won't be in." On 3/06/24 met w/ employee and employee stated on 3/01/24 while performing maintenance of 390 bucket they felt irritation/soreness to RIGHT eye. Employee was taken to local Urgent Care for observation, sent them to eye doctor and diagnosed with foreign body to RIGHT eye.
Employee was getting out of the triaxle truck at the fuel pumps in the parking lot. Employee slipped on the step while getting out of the truck and grabbed the hand hold to keep from falling. EE felt a pop and then pain in the right arm. EE went to the emergency room and was diagnosed with a torn bicep muscle.
On 1/25/24 employee descended wheel loader #LO041, planted feet on ground and slipped. Employee then grabbed ladder w/ RIGHT hand to prevent slip/fall causing soreness to RIGHT arm area. Later on 1/29/24 employee went to local Urgent Care for observation and diagnosed with sprain to RIGHT acromioclavicular joint and placed on light duty.
The employee was getting out of triaxle truck at the bed wash. The employee's foot slipped when employee stepped onto the ground. The employee fell backwards onto employee's shoulder. The employee did not seek medical attention until 12/27/23. Employee received an x-ray on 12/27/23 & it showed employee fractured left clavicle.
Performing maintenance greasing portable screening plant, employee descended fixed ladder, slipped and slid ~4' down metal steps of ladder scraping belly and stomach area against steps, landing on feet. Employee refused treatment. Two days later, employee was sore, went to Urgent Care for observation and diagnosed with two fracture ribs with restricted duty.
After moving 40# bag of coal, employee made contact with metal arm of robot, turned and felt soreness/pain in wrist area from bracing self on ground. Employee was taken to Urgent Care for observation, diagnosed with an abrasion and pain in wrists and placed on restricted duty.
Employee was moving material while operating wheel loader, drove over a pot hole, immediately applied foot brake causing loader to come to sudden stop. Employee felt a strain in lower back area and was taken to local Urgent Care for observation and diagnosed with strain of muscle to lower back and placed on light duty.
Performing pre-operational inspection employee attempted to clean off rear reverse light on asset #TR602 and received a cut to LEFT middle finger, applied bandage and finished shift. Next day employee went to Urgent Care for observation, diagnosed with laceration to LEFT middle finger and received three stitches.
Employee slipped off truck step due to excessive mud on boots.
Employee was using an acetylene torch to remove an old chute liner near the scalping screen. Hot slag fell into the top of employee's right work boot, causing a burn. Employee did not seek medical attention until 7/3/2023, at which time the burn was diagnosed as being infected. Employee was hospitalized for infection on 7/5/2023. Employee did not miss work until 7/6/2023.
While attempting to spread apart and adjust forks on Cat TL943 Telehandler (Asset #FL002), employee felt pain in RIGHT shoulder and neck area. Employee was taken to local hospital for observation and diagnosed with strain of muscle.
Employee was using the crane on a service truck to lift a CAT D11 final drive in the shop. EE was turning the hub with the crane and using a chain to guide the hub. The hub turned quickly and pulled the guide chain and in turn pulled EE's shoulder. EE experienced pain in right shoulder. Did not receive any medical treatment until 6/23/2023 when EE started therapy.
Employee was removing the hood springs on a tri-axle truck in preparation of removing the radiator. The spring was under tension and slipped out of their hand and the hooked end of the spring penetrated the skin of their right hand. Medical attention was required to clean and stitch the wound.
Making repairs to discharge lip on Unit #708 vibrator, employee stepped over center divider and felt cramping/pain in R. thigh area. Employee reported incident on Monday 10/31/22 and went for observation and was treated for back pain with radiculopathy.
Employee was installing a new CB radio in a dozer. Employee had left hand on seat of dozer and was cutting a wire tie with a utility knife in right hand. Knife cut through the wire tie and then struck the employee on the back of left hand causing a laceration. The laceration required stitches to close the wound.
Employee was doing a pre-shift inspection on a Cat D5H dozer. EE was checking the anti-freeze level and had opened the radiator cap. The dozer had not been started and was cold. When EE looked down at the top of the radiator, a piece of dust or dirt blew into EE's right eye. EE was later diagnosed with a cornea abrasion.
Stemming holes felt a pinch in back.
On 6/29/2022 employee was replacing vibrating unit. While grinding old weld employee reported EE felt something fall in EE's right eye, refused medical observation. Then on 7/05/22 employee complained of irritated eye. Went to local Urgent Care for observation and diagnosed w/ foreign body in right eye. Sent to eye doctor and had sliver removed.
A full 40# bag of coal slipped out of hands, employee tried to lift upwards to prevent bag from falling to floor, hands slipped on wet bag and left hand impacted bottom of metal chute causing laceration to left thumb. Employee was taken to local Urgent Care and diagnosed with laceration of left thumb.
Employee was walking across the cab deck towards the door of the rock truck and stumbled and fell towards the door. EE put out left hand to grab the mirror on the door and hit left hand on the mirror bracket between the little and ring finger causing a sprain.
Employee incorrectly placed an empty bag on bagging machine and reached to attempt to straighten bag when their left thumb impacted metal clamp causing laceration to left thumb. Employee was taken to local Urgent Care for observation and diagnosed with a laceration without foreign body of left hand.
On 1/19/22 while attempting to ascend truck, employee placed one foot on ladder, planted other foot, slipped on ice. EE then reached and grabbed hold of railing to prevent fall and hurt left shoulder area. Two weeks later EE's left shoulder/neck area was still sore. 2/02/22 EE was taken to Urgent Care for observation and diagnosed w/muscle strain.
Employee had just climbed down the ladder on a CAT 777C rock truck. They stepped down on the ground and was still holding on to the handrail and stepped on a rock which rolled out from under foot. This caused them to fall while still holding handrail with left hand, causing a strain/sprain to left shoulder.
Driver was dumping the load of ash and ash stuck in the bed and caused the tractor/trailer to roll over onto the passenger side. Driver was having neck pain and was transported to the hospital as a precaution.
The employee was operating a triaxle truck hauling coal from the mine to the cleaning plant. The tarp motor broke on the triaxle and employee was pulling on the tarp arm to manually move the tarp arm into a secure position. EE felt pain in neck and reported it to the supervisor at 2:00 P.M. on 11/15/2021. EE did not seek medical attention until 11/18/2021.
Employee was descending ladder of haul truck at end of shift and EE's right foot slipped causing EE to lose balance. EE reached with EE's right hand to grab hold of handrail and felt pain in right shoulder area.
Miner was loading packaged blasting agent and shoveling (stemming) bore holes and noticed EE's shoulder getting sore. Applied ice for several days with no relief. Reported the injury on 08/05/2021. Sought medical care on 08/09/2021.

Employee removed gloves and noticed bleeding and was taken to local hospital for observation and diagnosed with RIGHT 4th finger laceration and placed on restrictions.
Employee had just parked the CAT 992G loader and was climbing down the ladder at the rear of the machine. Employee parked loader on an unlevel area. Employee could not reach ground and EE slipped and fell on their back. Employee said was OK. Employee did not seek medical attention until 2/16/2021. Employee said tore their bicep muscle.
While shoveling, prepping and cleaning out the inside of a railcar employee slipped and fell. EE reached out with LEFT arm to catch self, arm twisted and they fell on left side causing pain and soreness to LEFT shoulder area. Employee was taken to local Urgent Care for observation and diagnosed with sprain to LEFT shoulder area and placed on restrictive duty.
Employee was walking across the area between the silt pile and coarse reject pile and slipped and fell due to snowy conditions. Employee landed on right hip/lower back area causing a muscle strain/pain in that area. *Employee did not seek medical attention until 2/8/2021.
While cleaning up coal bins at bagging department employee was shoveling coal and felt soreness to lower back area. EE was taken to local Urgent Care for observation and diagnosed with muscle strain to lower back and placed on restricted duty.
