Mostrando los 7The EE stated he was starting to climb on the machine after finishing his pre-inspection and his foot slipped off of the bottom step and hit the ground, injuring his back. The accident was not reported till the end of the shift. This was a non-lost time accident until 11/27/06. The EE went for test because of back pain. Restricted duty as of 11/28/06
Employee was loading blasting supplies out of a box trailer into a pickup truck. When finished he stepped out of the trailer onto the tailgate of the truck and his foot slipped off the tailgate causing him to fall back against the trailer landing on his rt hip.
Employee had climbed up on dozer with fuel hose to fuel dozer. When he started to remove fuel cap he said a strong gust of wind caused him to lose his balance causing him to fall to the ground, about 11'.
EMPLOYEE HAD JUST FILLED D8R DOZER WITH FUEL. HE DROPPED FUEL NOZZEL TO GROUND AND TURNED TO COME OFF DOZER. HE SLIPPED & FELL TO GROUND INJURING HIS LOWER BACK. HE FELL ABOUT 7 FT & 6 INCHES.
COAL TRUCK DRIVER PULLED ONTO SCALES AT THE PREP PLANT. WHEN HE GOT OUT OF TRUCK HIS FOOT SLIPPED OFF TOP STEP OF FUEL TANK ABOUT 3'. HE FELL & HIS LEFT KNEE & SHIN STRUCK 6" PIPE ON WALKWAY.
EMPLOYEE GREASER WAS WOMING DOWN LADDER OF LUBE TRUCK TO GROUND. HE PUT HIS FOOT ON A SMALL ROCK. HE STATED THAT THE ROCK MOVED AND HE FELL TO THE GROUND HURTING HIS RIGHT LEG BELOW HIS KNEE. THEREFORE CAUSING BODY WEIGHT TO BE ON RIGHT FOOT AND ANKLE.
