Mostrando 50 de 89On 9/28/25, EE slipped in a small hole and injured EE's knee while getting out of EE's vehicle for work. EE reported it to team lead at 12:00 PM; TL informed supervisor at 6:24 PM. EE requested medical attention the next day, was evaluated, given medication and work restrictions that didn't affect equipment duties. Released on 10/2/25.
EE working on installing forms. EE went to sit down on form and form broke. Tried to catch self but cut hand on anchor while doing so. EE received stitches.
Teammate fell while walking on the ice and felt a sharp pain in right knee. Update: Teammate had surgery on 3/26/24 & has been removed from duty.
Upon shift start, EE walked into the GN Mill Bld MCC room, and seemingly got into a physical altercation with an EE. Assumption is that IE was pushed or struck and fell against a main breaker bucket damaging some of the plate metal. IE diagnosed with ac joint separation to (L) shoulder, minor cut on mouth, ear & back. Also syncope at some point. Other EE no injuries.
EE was spotting for railcar mover and slipped and hit head. EE left spotting activity to talk with RR guy in truck parked nearby. They decided to go over and talk with guy about railcars. The stepped in a water puddle and slipped and fell, bumping head on ground. EE received medical attention(6 staples to close cut)all scans/tests negatives at hospital and released same night.
while walking on the South Path teammate slipped in a wet surface and EE's knee hit the floor. after 3 days of no contact with the plant the teammate reported EE's foot was hurting.
EE has ground level fall while exiting from Bdg#2. EE stepped off traction grating and onto slick pavement from rain event prior, causing EE to fall forward and landing on right hand and knee. EE diagnosis: acute right knee sprain
The operator stated: climbed on the side of the forklift & was standing on the tire of forklift releasing the latch by hand on the self-dumping trash can when teammate slipped & hurt foot. Teammate stated did not fall to the ground. Teammate has a dislocated big toe on left foot. Surveillance video shows no such event. Awaiting teammate to work for further investigation.
while attempting to climb a stationary ladder teammate stated EE loss EE's grip and fell from approx. 3ft onto a concrete pad
A packaging line operator slipped on wet surface and landed on back and left elbow
At approx. 10:41pm prior to clocking in, employee stated to have said to the teammates in the Control Room, that they had fallen but told everyone they were fine. EE then went on to work the entire shift without issue. EE called Supervisor at about 1215 pm on July 20, 2021 and during conversation the teammate revealed to that they slipped and fell.
while walking down the icy stairs, employee fell and tried to catch themselves on the hand rail and felt a pop in shoulder. EE was medically evaluated at the time and was released to return to work they continued to feel pain in shoulder and had a second evaluation which determined they had to have surgery to repair the damage.
Two (2) workers were lifting a core drill onto the back of a pickup truck, when one slipped and fell with the core drill landing on EE's leg. The ground was slippery from the wet guragum on the ground. The worker's knee had a small laceration and was bruised.
EE was going backwards down the staircase EE missed two steps and hit EE's left knee on the staircase. On the next trip down EE noticed another EE coming down the stairs and IE tried to backup to provide room for EE to pass and again IE missed two steps and hit EE's right knee.
EE was attempting to get into the 2017 S570 Bobcat. While attempting to enter the Bobcat EE stepped in the mouth of the bucket and grabbed a hold of the handles to pull self into the Bobcat. While stepping up to the grates to enter the Bobcat EE's left foot slipped on the mud which EE was standing on in the Bobcat's bucket.
Teammate decided to cross 45 degree slope at truck scale exit. Teammate slipped and fell while descending slope, EE got up and came inside and reported incident to Traffic Manager

Miner was walking towards equipment and tripped on a guard on floor and fell to their knees injuring the right knee.
Employee was stepping down off of fuel trailer using left foot & left foot rolled. Diagnosis: left ankle sprain.
EE lifting a roll of film, heard a pop & fell to floor. ER physician found no injuries & referred EE to family physician. WCI physician expressed concern about headache and imposed work restrictions pending follow up.
Standing on lift table pulling stack of jugs (empty) up roller bed, when right foot slipped on roller. He fell backwards landed on right lower back & forearm. Strain & bruised.
Walking around elevator & tripped on clay chunks causing a same level fall. Landed on right side arm and hip(radio on hip). On 5-8-15 doctor diagnosed as contusion sent back with no restrictions. On 5-12-15 X-ray report showed wrist having a minimally displaced fracture through the triquetrum with soft tissue swelling - referred to Orthopedic doctor.
EE retrieving parts & stepped on bin divider on floor w/ right foot. Divider slid forward & he fell on left knee(same level). Had pain & swelling x-ray shows no fracture. EE on restricted duty.
Fell out of the truck on buttocks
Employee alleged that he injured his ankle due to a 3/16" change in elevation.
Miner got off forklift by sliding down off it and landed on her right leg. She felt a pop in her knee and could not walk.
EE had just applied LVM on slippery area by OMW & was leaving area when he slipped and fell.He tried to catch himself with rt arm injuring his rt shoulder & arm. Diagnosis: rt shoulder and arm contusion. Light duty until next week.
Employee was walking and fell on a flight being stored cutting side of face and hand.

Employee walking from truck shop to office, slipped and fell. Employee stated he slipped and fell in mud, behind trucks in front of his office. Oil-rig is disputing this claim due to no witnesses, no evidence in area that a fall occurred, and the employee showed no signs of soiled clothing, no mud on clothes.

Employee was walking by fan carrying some grating to make a walkway in the tower area. It was muddy and the way he had to position the metal to get through the walkway he slipped and hit the edge of the guard with his right forearm.
