Employee was returning to equipment after fueling and tripped on frozen uneven terrain while returning hose and nozzle to pump.

Employee had exited equipment to speak with other operator and while walking, slipped on loose gravel/uneven terrain and fell to the ground.

Employee was adjusting flop gates with mechanical binder on the chute to bring product into spec when the binder latch jumped a cog tooth causing employee to lose balance resulting in twisting left knee.

Employee tripped on a rock when returning to truck upon completion of specific task

Employee was in the jaw rock crusher control room when EE went to step up on the platform at the control desk. Employee's foot slipped causing EE to fall. EE attempted to catch self and while doing so EE's right hand went through the glass of a window.

Employee was walking and tripped on frozen ground from tire travel tracks. Employee failed to have proper awareness of surroundings

Employee not following procedure climbed 6' ladder that was not level, employee was not wearing PPE gloves, the ladder tipped and employee cut finger.

Employee was walking near river bank feeling with foot to determine edge of bank. Once employee felt employee located it employee knelt down to feel with hand the ground gave way and employee fell about 10 feet.

Employee was walking and stepped into a puddle where the left foot came into contact with a rock which twisted their left ankle.

Walking back to machine ee stepped on a rock which made ee fall to the ground

Employee carried buckets or stone up some steps and spilled some stone on to the steps. On the way back down the employee slipped on said stones and fell on the steps injuring EE's low back on the steps.

Lack of Awareness of the employee's surroundings.

Began work and was fueling up the excavator. When fueling was complete stepped off the excavator onto a step, slipped and fell to the ground on EE's right elbow.

It was stated that the accident happened on 12/29/2015 but not reported until 1/8/2016. Was hooking water hose to fill up the water truck and tripped over wooden blocks and lost his balance. Says he aggravated his left shoulder.

Exiting fuel truck and knee twisted when foot hit ground.

HE WAS RUNNING THE CRUSHER CONTROL HOUSE, STANDING ON THE BOTTOM RUNG OF A STOOL, HIS FOOT SLIPPED, HE TRIED TO CATCH HIMSELF AND RAN HIS LEFT HAND THROUGH A PLATE GLASS WINDOW.

WAS REPLACING SCREEN IN CRUSHER SLIPPED IN HOLE HURTING KNEE TOLD SUPERVISOR ABOUT IT BUT SAID KNEE WAS OK

EMPLOYEE STANDING ON PICKUP, FIXING A HYDRAULIC HOSE ON DRILL BOOM. HE SLIPPED OFF PICKUP TRUCK AND FELL TO THE GROUND, INJURING HIS RIGHT FOOT.

HE WAS SERVICING THE ROCK SCREEN, HE SLIPPED AND FELL ON THE SCREEN CUTTING THE BACK OF HIS HEAD, HURTING HIS LEFT SHOULDER, AND CUTTING HIS RIGHT LOWER BACK.

CHECKING LOADER REACHED FOR BUCKET FELL TO GROUND PULLING LOWER BACK ON L SIDE AND L HIP Y

EMPLOYEE WAS PUMPING HYDRAULIC OIL INTO PAYHAULER, THEN WAS GOING TO PUT WATER INTO SERVICE TRUCK (#605), STEPPED UPON STEP ON THE BACK OF UNIT #605. FOOT SLIPPED AND FELL ON LEFT SHOULDER.

IT HAD BEEN RAINING & EE FELL TO GROUND AFTER HE SLIPPED ON SOME WET MATERIAL AROUND THE SAND SEPERATOR. WE THINK HE STRAINED A LOWER BACK MUSCLE.

OPERATOR WAS GETTING OFF LOADER 988-B WHEN CABLE LADDER SLIPPED AND OPERATOR FELL TO GROUND ON LEFT KNEE
