Closing the lid of the crusher, EE was moving a rock that was in the way with a metal rod. When the rock moved, EE's weight shifted as well, and EE fell into the lid retainer wedge holder with EE's upper right chest.

Offloading a 40' auger from a flatbed, EE placed chocks against the auger and removed ties securing it in place, then moved to secure it to the forklift with a come-along winch. When the auger started to roll the EE jumped clear of the auger and upon landing on the ground EE's right knee was twisted and EE's left wrist was broken.

EE STATED THAT HE ENTERED THE OFFICE BLDG AND A FEW MINUTES LATER AS HE WAS LEAVING THE BLDG SLIPPED AND FELL ON THE LANDING AREA. THE AREA CONSISTS OF A LEVEL CONCRETE FLOOR COVERED BY A ROOF . THE EE WORKED UNTIL 1-28-04 AND LEFT ON FAMILYMEDICAL LEAVE FOR AN UNRELATED ILLNESS RX ONLY.
