employee was working at heights, slipped and fell at approx. 6 ft. employee was found lying on the ground in pain, 911 was called & EE was taken to the hospital at this time. All we're aware of is that EE has to have neck surgery. Due to the accident, we were unable to get a statement from the employee of what EE was doing.

"Getting out of the water truck to use the restroom. Tripped while stepping off the steps of the water truck, fell and injured the right (hand) trying to stop the fall"

"Changing screen deck went to move my harness after taking bolts off and fell through the hole in the screen to screen deck" Left foot talonavicular dislocation, needs crutches, no weight on left foot for 10 days

Fell off Dozer while opening door.

While cleaning loader windows he lost his balance and his left ankle twisted and when he went to catch himself on the other foot he caused right ankle spiral fracture.

EMPLOYEE REACHED OVER TO OPEN DOOR OF WATER TRUCK HIS LEFT FOOT SLIPPED, HIS WHOLE BODY FLEW INTO THE DOOR CRUSHING HIS RIGHT HAND. RIGHT RING FINGER HAD FRACTURED BONE.

EE WAS CLEANING THE WINDSHIELD OF THE LOADER BEFORE SHIFT WHEN HE SLIPPED. AS HE STARTED TO FALL, HE GRABBED THE SIDE MIRROR AND IT BROKE. HE STRUCK SOMETHING AS HE FELL THAT CUT HIS ARM.

EMPLOYEE WAS STANDING ON FENDER OF PORTABLE WATER PUMP WHEN HE SLIPPED AND TWISTED HIS KNEE.
