All we're aware of is that EE has to have neck surgery. Due to the accident, we were unable to get a statement from the employee of what EE was doing.

THE EMPLOYEE WAS CLEAINING DIRT OFF AROUND THE TAIL PULLEY OF A CONVERYOR BELT. THE PLANT WAS POWERED OFF, LOCKED OUT AND TAGGED OUT. THE BELT UNEXPECTEDLY ROLLED BACKWARD, CAUGHT THE EMPLOYEES GLOVE WHICH PULLED THEIR HAND AND ARM UNDER AND AROUND THE TAIL PULLEY. ANOTHER EMPLOYEE WAS ABLE TO GET A WRENCH AND ROLL THE TAIL PULLEY FORWARD AND RELEASE THE HAND AND ARM.
Fell off Dozer while opening door.
While cleaning loader windows he lost his balance and his left ankle twisted and when he went to catch himself on the other foot he caused right ankle spiral fracture.

Employee driving down hill on access road with 1/2 tank of water, brakes would not hold back. Turned into a run-away and truck rolled 3/4 turn up an embankment.
