Mostrando los 11Employee stepped up on a rock when their foot slipped cutting their right shin on the rock. Employee's leg was cut and they received stitches.
EE was in the hopper working on the warrior screen. EE stepped up and out of the hopper, onto what EE thought was steel, but turned out to be rubber. EE fell approximately 6 feet onto a pile of rocks, similar to light stone fill. EE bruised EE's right leg above the knee/thigh area and scraped the back of EE's right arm.
The employee slipped on snow/ice. Employee grabbed a piece of equipment to prevent falling and injured the right shoulder in the process. The employee did not receive "medical treatment" until 5/9/18.
The employee was welding in a chute on approximately 1/3/18. The snow and ice that melted as a result of the welding refroze on the ground. The employee slipped and fell on right elbow, injuring right elbow and right shoulder. The employee's first day of lost time did not occur until 6/14/18.
The employee was opening the tailgate to EE's truck, when EE's feet slipped, causing EE to pinch EE's fingers in the tailgate. This resulted in lacerations to EE's right index finger and right middle finger and a bruise to EE's right ring finger.

Passenger was riding in loaded dump truck down a grade when the truck went out of control. He jumped from the cab, was treated and released from a hospital. Truck involved in accident was owned by a customer, not a contractor or mine operator.

EE WAS DESCENDING FROM EUCLID TRUCK VIA LADDER TO THE GROUND. WHEN THE EE REACHED THE GROUND, SHE LOST HER FOOTING, AND SPRAINED HER LEFT KNEE.
