EMPLOYEE WAS WALKING AROUND THE CONVEYER AND SLIPPED 0N THE ICE. HE THEN FELL AGAINST THE TIRE OF THE CONVEYER AND BROKE HIS UPPER LEFT ARM.

Employee was attempting to get into the wheel loader with muddy boots. When he got onto the step or ladder of the loader, he slipped and hit his right knee on the step and fell to the ground. He did not seek medical attention until 11/20/07. He was treated & released.

EE was attempting to get down from the generator trailed by using a ladder. When he put his foot on the top rung, the ladder kicked out from under him. He fell approx 5' to the ground and landed on his right side.

Employee was assisting the mechanic in repairing the loader. He was attempting to tighten a nut and when the wrench slipped off the nut he lost his footing and fell from the front fender to the ground and landed on top of a come-along tool.

EE WAS STEPPING DOWN OFF OF THE EUCLID DUMP TRUCK AFTER CHECKING THE ANTI FREEZE LEVEL IN THE ENGINE. HE STEPPED ONTO THE BUMPER AND THEN ONTO THE STEP AND LOST HIS FOOTING AND FELL ONTO THE G ROUND. HE STATED THAT HE WAS HAVING PROBLEMS WITH HIS KNEE AND IT WAS POSSIBLE HIS KNEE GAVE OUT.

HE WAS WELDING ON THE GRIZZLY. HE HAD MUD ON HIS BOOTS- SLIPPED AND FELL THROUGH THE BARS ON THE GRIZZLY. ONLY LEGS FELL THROUGH UP TO HIS KNEES. HE WAS CONCERNED ABOUT RE-INJURING A PREVIOUS INJURY.

EE WAS HELPING TO CLEAN THE SLUICE BOX WHEN HE DECIDED TO WALK TO THE BOTTOM TO TALK TO THE SUPERVISOR HE SLIPPED AND FELL DOWN FLAT ON HIS BACK.

EE WAS WORKING ON LOADER AND SLIPPED OFF TWISTED HIS ANKLE MEDICAL REPORT LISTS SPRAIN.

HELPING WORK ON THE EL JAY SCREEN SLIPPED OFF STEP AND SET DOWN ON TOP STEP I NAME FINISHED WORK THAT DAY HE RECEIVED ASPRIN IMMEADIATELY AFTER HE DIDNT FELL NEED TO GOTO DR ADVISES HE HAS A S TRAINED LOWER BACK MUSCLE

HE WAS BACKING DOWN TO GET LOADED. HE WAS IN A FORWARD GEAR AND ROLLED BACKWARDS AND KILLED THE ENGINE. THE TRUCK ROLLED BACK INTO THE BACKHOE- NO SUBSTANTIAL DAMAGE TO EQUIPMENT. HE WHILE EXI TING VEHICLE SLIPPED AND TWISTED HIS BACK CAUSING, SUPPOSEDLY, A PINCHED NERVE IN HIS BACK.

HE WAS GREASING BROOM TRUCK SMALL STREET SWEEPER WHEN HE FELL BACKWARD.

WHEN GETTING INTO TRUCK HE LOST HIS FOOTING, SLIPPED AND FELL TRYING TO STOP HIMSELF WITH RIGHT ARM.

UTILITY MAN PUT7ING OIL IN TRUCK, SLIPPED OFF THE STEERING BAR HE WAS STANDING ON, GRABBED FOR GROOVED STEPS, CAUGHT RING (LEFT RING FINGER) ON STEP, PARTIALLY AMPUTATED FINGER WHICH WAS SURGI CALLY RE-ATTACH9D.

THE MAN SLIPPED ON THE ICE IN THE YARD AREA AND BROKE HIS RIGHT LEG NEAR THE ANKLE
