Employee was stemming a blast hole, when EE was finished EE was using EE's foot to level it off when EE's knee gave out.

While operating hand valve on dewatering equipment, employee slipped on muddy ground and struck knee against the pipe flange causing two small (approximately 1") lacerations. Employee went to ER and they closed the wound with medical adhesive, gave EE a prescription antibiotic and returned EE to full duty immediately.

Miner was coming down the surge bin ladder and fell landing on the ground.

Miner was standing on a 6 foot step ladder when the miner tried to step down the miner's foot got caught and the miner fell head first with head and shoulder hitting the ground first. ***Miner had personal neck surgery previously. No medical would see them. We took next available with previous surgeon. Miner was immediately released to full duty.***

Miner was walking through our maintenance shop and tripped falling to the concrete and hitting the left side of face.

This incident became reportable when the miner finally agreed to go to the doctor and an MRI revealed a torn meniscus on 7-18-22. On the date of the incident the miner said miner was fine and no medical care was needed. On May 12th the miner slipped on the bumper of miner's stockpile truck when shutting the hood during pre-operation inspection.

MECHANIC FINISHED WORK ON HOIST CYLINDER ON TRUCK TURNED TO GO BACK TO SERVICE TRUCK AND TRIPPED ON BUCKET OF SKID LOADER FALLING ACROSS BUCKET AND DRAW BAR. BRUISED LEFT SIDE, RIBS AND BACK.

MINER WAS WALKING TO THE TRUCK, SLIPPED ON ICE AND FELL TO THE GROUND. X-RAYS REVEALED TWO FRACTURED RIBS.

Employee was washing this truck at the end of the day and fell down an embankment. The employee cannot remember why or what caused him to fall.

Employee was working at the Moscow Quarry location when he slipped and fell on the ice. He wasn't sure exactly how he landed, but had left shoulder pain.

PLANT HAD SHUT DOWN AND EE WAS CLEANING ROCKS OFF CONVEYOR BELT. HE HAD 3 POINTS OF CONTACT ON CONVEYOR AS HE WAS CLIMBING DOWN. HIS FOOT SLIPPED ON A TIRE AND HIS FINGER CAUGHT ON A SHARP CORNER AND SLICED HIS LEFT RING FINGER, REQUIRING 9 STITCHES.

Employee tripped over jack stand and landed on head. Prescribed flexeril for neck stiffness.

EE WAS EXITING A PARTS TRAILER WHEN HE SLIPPED ON THE BOTTOM STEP. AS HE WAS FALLING, HE HIT THE BACK OF HIS HEAD ON THE TOP METAL STEP. EE RECEIVED 9 STITCHES AS A RESULT.

THE EE WAS GREASING THE PINS, ON THE LIFT ARMS, OF A MODEL 275 MICHIGAN LOADER. WHILE HE WAS PERFORMING MAINTENANCE, HE SLIPPED, FELL, AND HIT THE SIDE OF HIS FACE ON THE BUCKET OF THE LOADER. HE RECEIVED TWO STITCHES ABOVE THE EYE, AND SOMEBRUISING TO HIS HAND AND SIDE.

EMPLOYEE TRIPPED WHILE EXITING A SKID STEER. THIS RESULTED IN AN OPEN WOUND TO THE RIGHT LEG.

EE WAS GETTING OUT OF THE STOCKPILE TRUCK AND TURNED HIS ANKLE WHEN HE LANDED ON THE GROUND.

EMPLOYEE WAS STEPPING DOWN FROM A SEMI TRAILER WHEN THE STEEL STEPS SLID AWAY FROM THE SIDE OF THE TRAILER, CAUSING THE EMPLOYEE TO FALL 4-5 FEET TO THE GROUND.

EE WAS CARRING SCREEN DOWN PLANT STEPS, TRIPPED AND FELL.

EMPLOYEE FELL FROM AN ELEVATED CONVEYOR WHILE PERFORMING MAINTENANCE. REPORT FROM PHYSICIAN DATED 12-16, RECEIVED 12-30, REVEAL AN OBLIQUE FRACTURE OF THE THIRD METACARPAL OF HIS RIGHT HAND. N O OTHER INJURY WAS REPORTED. THE EMPLOYEE WAS ABLE TO RETURN TO HIS SCHEDULED JOB.

EMPL WALKING DOWN STEPS OF TOOL TRAILER WHEN HE SLIPPED.GRABBED THE CASING ON SIDE OF TRAILER AND CUT LIFT INDEX FINGER.INJURY REQ 7 STITCHES.AFTER TREATMENT EMPL RETURNED TO WORK WITHOUT REST RICTIONS.

EMPLOYEE WAS WATCHING PLANT TO MAKE SURE ALL CONVEYOR BELTS WERE RUNNING. BELT ON THE STACKER STARTED TO SLIP AND WHEN EMPLOYEE MOVED TO STOP THE PLANT HE SLIPPED AND SPRAINED HIS LEFT WRIST.
