EE was injured while preparing S31 for moving from bottom of the Hematite Pit using the motivator. A cable crew was pulling slack and loops out of trailing cable. The trailing cable became taut contacting the ee causing a fall that resulted in injuries. The ee was transported to a hospital for evaluation and treatment of injuries, and later hospitalized resulting in Lost Time.

While conducting industrial hygiene duties at the Tilden Concentrator, employee reported stumbling and falling forward landing on right hand. A minor injury was sustained, but employee did not seek medical attention. Employee continued to have a problem with the finger and was later evaluated. On 08/24/17, employee underwent surgery and was back to work the next day.

EE was stepping onto a metal stair system when right foot slid over the edge of a tread causing a slip down several steps results in right ankle getting caught on stair tread. He was evaluated by a physician, diagnosed with a fracture and taken off of work.

Employee was cleaning the drill towards the end of the shift. As he attempted to exit the drill, the employee slipped on the top step of the exit steps; (stairwell had a total of 4 steps) and fell, landing on his back. Ice had accumulated on the top step from the wash water the employee was using to clean the drill.

As the driver was pulling his wheel chocks from underneath the tractor tires in preparation to leave the dump zone, the driver stumbled backwards and stepped on his left foot at an odd angle. After receiving treatment at a local walk in clinic, the driver diagnosed with a broken bone in his foot.

EMPLOYEE WAS WALKING FROM T-1 PRECIPITATOR BINS TO T-2 PRECIPITATOR BIN AREA VIA THE PELLET PLANT FAN FLOOR. HE WALKED THROUGH SOME MUD AND SLIPPED TWISTING HIS LEFT KNEE. HE WAS EVALUATED BY A PHYSICIAN AND KNEE SURGERY WAS SCHEDULED 2-19-04.
EE was climbing the stairs to the time scan clock area when he tripped on one of the steps & fell hitting his left elbow on a step. He had surgery on 3/12/04 resulting in lost time.
