EE was injured while preparing S31 for moving from bottom of the Hematite Pit using the motivator. A cable crew was pulling slack and loops out of trailing cable. The trailing cable became taut contacting the ee causing a fall that resulted in injuries. The ee was transported to a hospital for evaluation and treatment of injuries, and later hospitalized resulting in Lost Time.

IP was in process of completing a delivery to customer site. Foot became tangled in rope between wheel chocks causing IP to fall. IP place arms out to break fall and when landed caused fracture of the bone.

While conducting industrial hygiene duties at the Tilden Concentrator, employee reported stumbling and falling forward landing on right hand. A minor injury was sustained, but employee did not seek medical attention. Employee continued to have a problem with the finger and was later evaluated. On 08/24/17, employee underwent surgery and was back to work the next day.

EE reports exiting cab of cable truck when experienced right knee pain. EE claims traction soles on protective footwear may have caught on step tread causing stress to knee while turning. After conservative treatment, follow up evaluation was performed where restrictions were ordered on 2-2-17.

EE was working on #3 Poly pump in the deslime basement on midnight shift and when going to retrieve a tool, he slipped and fell onto his back. He was evaluated by a Physician, given restrictions that the site could not accomodate. He was given a full duty release at his evaluation on 10-26-15.

A repairperson was operating a pneumatic impact inside a balling drum when material fell from the drum striking the ee causing a fall from the staging platform to lower level, about 2 ft below. EE sustained abrasions to back reporting pain in shoulder, back, & neck.EE was assessed by site MED PERS,& taken to HOSP for further eval.EE was removed from work, resulting in LOST TIME.

EE was stepping onto a metal stair system when right foot slid over the edge of a tread causing a slip down several steps results in right ankle getting caught on stair tread. He was evaluated by a physician, diagnosed with a fracture and taken off of work.

EE was turning on a water valve on the fifth floor of the Tilden crusher when the ladder being stood on slipped out from underneath. EE was evaluated by a Physician and taken off of work.

Alleged - Employees was standing next to a cyclone feed pump. When he stumbled backwards striking left elbow on a machine guard or concrete floor. The employee was evaluated by a Physician and taken off of work.

EE was lining in a primary mill. When he attempted to step off a ladder, he landed on uneven ground losing his balance. He reached back to catch himself, straining his R shoulder. He was evaluated by a Physician, given restrictions and returned to work.

EE was stretching a hose between 2 balling lines. He was backing up when his right heel caught a rolled up belt causing him to fall with his left shoulder striking the grating. He was evaluated by a Physician and returned to work with restrictions until his full duty release on 1-2-15

EE rolled left ankle as he was stepping down 11" onto the grating from line 7 flot feed pump concrete base. EE was evaluated by a Physician and ee was given restrictions.

EE was walking on the grating outside of the ER shop when his boot got caught in the grating resulting in the ee reporting pain to his left calf. He was evaluated by a Physician, and given an air splint for his left ankle. He returned to work full duty.

Employee alleges right knee pain after getting out of a pickup truck. He was evaulated by a physican and surgery was preformed on 8/18/14.

Employee was cleaning the drill towards the end of the shift. As he attempted to exit the drill, the employee slipped on the top step of the exit steps; (stairwell had a total of 4 steps) and fell, landing on his back. Ice had accumulated on the top step from the wash water the employee was using to clean the drill.

The employee was stepping over shovel tracks when he lost his balance and fell. His right foot was twisted in the tracks during the fall, resulting in injury. He was evaluated by a physician and given a splint for a dislocated right toe. He was released back to work.

EE was walking down a stairway and ee slipped on the bottom stair tread which was bent resulting in back discomfort. The ee was evaluated by a Physician on 11-6-13 and rtw with restrictions.

While deboarding a cable truck employee missed a step and fell onto his back. On 10/13/13 employee was evaluated in an ER and was taken off of work. He missed his scheduled shifts on 10/13 & 10/14. He returned to work full duty on 10/17/13.

Employee reported left knee pain after tripping on a chain. He was evaluated on 7/11 and taken off of work.

On 4/24/13 the employee tripped on grating and twisted his left ankle. On 5/17/13 he was evaluated by a physician and diagnosed with a fractured left ankle. He was given restrictions that prevented him from working. First day missed was 5/18/13. At a follow-up evaluation with a physician on 5/20/13, employee was taken off of work.

EE was walking and tripped over a grease line landing on his right knee. He has been followed by a Physician and on 7-18-13 underwent surgery resulting in lost time.

EE was walking up a stair case and stepped on the 11 tread and it fell out from under his right foot on 10-25-12. He has been treating with a Physician and was taken off of work on 1-10-13.

The employee was walking up a flight of graded stairs, stumbled and his left hand landed on the grading and his fingers went through the grading. He was evaluated by a Physician, taken off of work and hand a surgical repair to the 2 fractured fingers.

This injury occurred to a driver while on mine property at the Tilden Flux dump site. As the driver was pulling his wheel chocks from underneath the tractor tires in preparation to leave the dump zone, the driver stumbled backwards and stepped on his left foot at an odd angle. After receiving treatment at a local walk in clinic, the driver diagnosed with a broken bone in his foot.

Employee was carrying some guarding when he tripped on grating and fell-twisting his back. He complained of back pain. He was evaluated by a physician and was taken off of work starting 7/12/11.

Employee was walking down the ramp from the upper repair bay and rolled his ankle.

Employee was walking over a snow bank when he sunk into the snow causing him to strain his left leg. He was evaluated by a physician and placed on restrictive duty starting 12/15/10.

Employee was walking in to work. He fell on the landing at the bottom of the stairs from the parking lot. He was evaluated by a physician and physical therapy was prescribed.

Employee slipped and fell while pulling on the power cable to a shovel. He was evaluated by a Physician and started Physical Therapy on 1-30-09.

Employee tripped and fell on a charged washdown hose-catching himself with his hand. Upon falling, employee fractured his left ring finger.

While walking carrying a 48# wt. he felt his ankle give and he fell to the ground.

Employee was putting fuel into a portable welder-tripped on a cable and struck his right elbow against a steel plate. He was evaluated by a physician and taken off of work.

Employee had unloaded a truck. He stepped backwards and caught his heels on the pallet. He fell backwards striking his hand on the part that was on the pallet. He was treated for a strain and was given a splint.

As employee was leaving his work area there is a vertical beam that he went to duck under. He slipped and struck the beam with either his hardhat, glasses or his face. He was diagnosed with a fractured nose.

When employee was moving from the lube truck to the loading dock, he slipped on some grease and strained his left knee. Lost time started on 7/13/06.

Employee was walking in a low headroom walkway. He went to lift his head up and hit a pipe causing him to fall forward. He grabbed onto the handrailing. He was treated for shoulder strain and on 5/17/06 he started physical therapy.

Employee was working on dozer and when he stepped off of the platform by the cab, his feet slipped off of the track. He fell on the push-arm and floor. He fractured his left leg and was taken off of work.

While positioning hoses in his work area (the launders) he stepped in an opening within the launder. He was treated and later ordered physical therapy.

While walking on the walkway, employee tripped. As he went to catch himself his hand was caught between a troughing idler and the conveyor belt. He received stitches to the left hand.

Employee slipped on mud and hit his left knee on the handrail.

Employee was skirting on 17 conveyor when he fell backwards off the conveyor structure hitting his thigh on 2" angle iron. He was admitted to the hospital on 9/24/04.

Employee was lining inside 1B flux mill (2-3" steel balls are inside). He took a step and the balls underneath his foot came loose causing his foot to slide and he felt a sharp pain in his back. He was evaluated by a physician and physical therapy started on 9-10-04.

Was a passenger in a van - while stepping out lost his balance & fell to the ground - chipping his front tooth.

When reaching to check to see if seal water was on to 7B DTV pump, Ray slipped in mud and when trying to catch his balance he dislocated his right shoulder. He had surgery on 8-2-04.

EE was hooking up hoses to the north stock pile track cars and slipped hitting her head on pet cock. She complained of a pulled muscle in her neck. She was evaluated by a physician and physical therapy was started on 6-8-04.

WORKING AT BOTTOM OF PRECIPITATOR, HEARD LOUD CRASH OF STEEL FALLING, AREA FILLED WITH DIRT & DUST. SCRAMBLED TO GET DOWN AND OUT.

While repairing fans above the tripper in the ore barn, ee stepped on a trap door. He fell through the trap door, skinning his shin and catching himself, straining his right shoulder. He returned to the job and sought medical care.

EE PARKED HIS PERSONAL TRUCK IN THE DAY SHIFT PARKING LOT UPON ARRIVAL AT WORK. WHEN HE EXITED HIS VEHICLE HE SLIPPED ON THE SNOW COVERED GROUND AND TWISTED HIS KNEE. HE WAS EVALUATED BY A PHY SICIAN AND WAS TAKEN OFF OF WORK. KNEE SURGERY SCHEDULED FOR 2-16-04. (WAS OFF OF WORK STARTING ON 2-4-04.)

EMPLOYEE WAS WALKING FROM T-1 PRECIPITATOR BINS TO T-2 PRECIPITATOR BIN AREA VIA THE PELLET PLANT FAN FLOOR. HE WALKED THROUGH SOME MUD AND SLIPPED TWISTING HIS LEFT KNEE. HE WAS EVALUATED BY A PHYSICIAN AND KNEE SURGERY WAS SCHEDULED 2-19-04.

EE was climbing the stairs to the time scan clock area when he tripped on one of the steps & fell hitting his left elbow on a step. He had surgery on 3/12/04 resulting in lost time.
