Employee had pain in right knee after dismounting bulldozer #2572. Employee reported to on-site clinic for examination. Employee returned the following day for follow-up visit and was referred to off-site clinic for treatment. Employee was placed on modified work.

EE was standing on top step of lift pump, stepped backwards, foot slipped EE fell on right buttocks. No complaints, over time had discomfort, on 9/5 EE was seen in clinic & later seen by co. dr. EE had prior back problem, referred to specialist no restrictions that would limit duties. F/up visit w/specialist on 1/26/18 condition had not improved, EE was placed on modified duty.

Employee was descending stairs and stepped on short step and left ankle rolled causing employee to fall to knees. Employee was placed on restricted duty on 2/17/2017.

Employee was climbing into back of truck to retrieve shovel and fell backwards onto the ground landing on employee's left elbow. Employee did not have pain initially but soreness increased, continued to perform regular job. The need for rotator cuff repair was diagnosed and employee had left shoulder surgery on 4/27/17.

Employee stepped off of a step on the side of a flatbed truck and twisted right ankle. Sprain with a hairline fracture

An employee was performing hands free hydro blasting on the #3 cold cyclone, the water contacted hot product when the blockage broke through and caused steam to blow back up the pipe. The employee operating the unit stepped off the peddle, and then tried to quickly back away. He stumbled over the foot pedal and fell.

Employee was walking at Mill flotation. He stepped in an unprotected trench. Grating that covered the trench had been removed, water was covering the area due to a pump failure at the time. Received an abrasion to his right shin and reported that his hip was sore. He returned to work at that time. Became a recordable on 6/23/15 due to the employee having physical therapy.

Frozen ground conditions

Operator exited the cab of the crane and walked across the deck toward the ladder to climb down. Just as he approached the ladder, he slipped and fell to the ground landing on his feet and then buttocks. Employee was taken to hospital with complaints of pain in his right ankle/foot. Xrays revealed a fracture a small heel bone that required a cast to heal.

The employee allegedly walked off of a stair well and fell approx 3 ft to the ground fracturing his elbow. The injury was reported on day after the alleged incident and there were no witnesses to the fall. The validity of the claim is questionable.

While exiting a connex employee used a cross support bar as a step. His foot slid backwards causing him to fall. Pain in left leg. Employee taken to First Aid where ice pack was applied, OTC Aleve was given for pain. He was released back to work. On 1/2/14 he was still having pain. X-ray done on 1/3 showed an avulsion fracture at the base of his lower left leg.

Employee was tasked to check the cyclones and read the tank level at the ore storage tank. When he got to the tank, he called to the control room to shut down the 1 & 2 storage pumps because the overflow box was leaking onto steps. He went to the top of the tank of perform duties. When he came back down, he slipped on the wet steps. He strained his left shoulder and right wrist.

On 7/28 while trying to extinguish a fire on excavator, employee slipped on track of machine while reaching for handrail striking left shin on tracks. Incident was reported as report only incident. On 8/5(0700)employee presented to site clinic with unresolved leg pain & swelling from incident. The injured employee was evaluated & released to work with sedentary restrictions.

Employee was stepping off transportation bus attending a company wide safety meeting. He was stepping off the bottom step to the ground when he twisted his right ankle. Ankle was evaluated by a physician and he was diagnosed with a sprained ankle.

Employee was ascending stairs when left knee gave way causing employee to collapse onto the stairs.

Employee was dismounting D8 bulldozer when his foot slipped from bulldozer blade rail causing him to spin around hyper extending his right elbow under his body weight. He reported the incident and was able to return to work. He reported to work on 7/2 complaining of pain with restricted range of motion.

Employee stepped down 18 inches or so from one level to another on a catwalk. While doing so he extended his arm slightly behind him and up to just above shoulder height. At full extension he felt sudden severe pain.

Mechanic was loading a wooden crane pad (65 lbs)onto the front deck of a 35 ton crane. Employee slipped in the mud and fell forward catching his little finger between the crane pad and a metal loop designed to hold the pad the pad on the crane.

Employee walking on concrete walkway. Caught toe of boot and began to fall forward. Reached out with left hand (was carrying 2.5 gal. jug of oil in right hand)to catch his fall - jammed left hand into solid metal handrail. Hand flexed back at wrist.

The employee was pulling on the lever that moves the points of the rails on the railroad switch. Employee's right hand slipped off the lever and caused him to lose his balance. He fell backward and landed on his left hand.

Employee fell while exiting a caterpillar excavator. Employee states that he was using three points of contact, and was egresing the ladder correctly. Investigation of the ramp did not reveal any adverse condition of the equipment or the ppe worn by the employee.

Employee was helping remove bolts from a pump. With two bolts remaining, employee decided to go under the suction line to the east side of the pipe due to restricted working room and possible line of fire issued should the pipe move. When he crouched down to step under the pipe, his foot slipped on the metal.

Technician stepped onto fold down step at rear of service truck to enter rear service truck area. Weld on fold down step failed.

Emp. was changing out transmission filters on D-10. Emp. had to mount rear of tractor to complete job. As he was dismounting, he stated that the underlying rock/buildup shifted. At this time he experienced pain in his R knee. Emp was evaluated by an ortho surgeon on 10/29/09 and placed on restricted duty pending surgery.

General Foreman went to the #5 lift pump at the #4 matrix line to perform a key procedure audit. Three mechanics were performing a PM on the pump. As GF walked around the front of pump sled, he stepped into a hole that was covered with mud. The sudden step into hole caused his left ankle to turn, resulting in torn ligaments.

Employee was walking down stairs from hot wind box level to free board level doing morning checks. He felt a pop in his left knee.

Fell 2 to 3 feet resulting in a contusion to his left buttock.

Employee, Mechanic with 31 years experience, was fabing 'A' frame for de-watering barge. While using overhead shop hoist to move frame out of shop, chain on hoist slipped causing frame to move. Employee lost his balance causing him to fall to shop floor. Employee landed on right elbow which caused a fracture of the right humerus.

Operator attempted to close manual block valve. It wouldn't close completely. When trying to open and reclose, the handle broke. The Operator lost his balance, fell backwards and hit the back of his head on the floor grating causing a laceration that required stitches. Valve damage about $100.00.

Employee boarding bus to be transported to work site. He lost footing and slipped falling forward. He attempted to break his fall with left hand and fractured left fifth metacarpal.

Employee, Heavy Equipment Trainee with 7 months experience, stepped over a piece of 20" matrix pipe and the ground gave way twisting his right leg. He reported discomfort in his right knee to his supervisor and saw the company doctor on 1/25/07. He worked without restriction until he was placed on restricted duty by the company doctor on 3/21/07 for subjective pain.

Employee was walking up a sand incline leaving a pumping station when his right foot slipped and he fell forward as his right leg shot outward. He experienced tenderness in his right knee and thigh. The company physician referred the employee to an orthopedic physician. Outpatient surgery was performed to repair the injured knee.

Injured employee was preparing to fuel an excavator. He placed the fuel nozzle on the catwalk by the excavator and then climbed up onto the catwalk. Once on the catwalk he lost his balance and jumped/fell 4-5 feet to the ground. An x-ray taken on 8/18 revealed a cracked bone. Employee was then placed on light duty status.

EE WAS WALKING FROM GUARDHOUSE TO PORT-A-JOHN WHEN EE HEARD A CAR. EE TURNED TO SEE IF THEY NEEDED TO BE LET OUT OF THE FACILITY. WHEN EE TURNED BACK AROUND, EE MISSTEPPED ON FOOT, SLIPPED OFF THE PAVEMENT, TWISTING LEFT ANKLE AND KNEE.

Injured employee was climbing ladder on the shoe of the No. 3 dragline when his right foot slipped on the fourth rung of the ladder. Employee lost his grip on the ladder with his right hand and fell backward, approximately 3-4 feet, landing flat on his back.

Employee and co-worker were carrying a 2/0 motor cable with cable coupler from the motor sled to the Pit Car for reconnection. Injured employee's right foot twisted and he fell. He continued the job of connecting the motor and power cable to pit car. The next morning the employee notified his foreman of swelling in his right ankle. X-rays revealed a fracture.

EE WAS ATTEMPTING TO STEP FROM THE CABSIDE SHOE DOWN TO THE TUB (27") HIS FOOT SLIPPED & HE FELLSTRIKING HIS L HAND ON THE CORNER OF THE TUB. HEFELL APPROX. 67" TO THE GROUND LANDING ON HIS KN EES BETWEEN THE TUB AND THE SHOE. THE WIDTH BETWEEN THESE IS 19".

EE WAS CLIMBING 2ND LEVEL OF EAST STAIRS CARRYING HIS LUNCH BOX & COFFEE MUG, ONE IN EACH HAND. HE DID NOT HAVE A HAND FREE TO USE RAILING. HE TRIPPED ON A STEP & CAUGHT HIMSELF WITH HIS HAND.

WHILE STANDING ON A STACK OF BRICKS, EE WAS HOLDING UP WOOD FOR OTHERS TO DRILL. WHEN HE TURNED TO GO TO THE NEXT PIECE OF WOOD, HE LOST HIS BALANCE & FELL ONTO ANOTHER STACK OF BRICKS.

INJURED EMPLOYEE WAS EXITING THE SRM BUCKET SHOP RESTROOM. WHILE STEPPING DOWN TO BOTTOM STEP, WHICH IS 2" ABOVE GROUND LEVEL, HIS RIGHT FOOT LANDED ON THE EDGE OF THE STEP CAUSING HIS FOOT TO ROLL INWARD, PLACING PRESSURE ON THE OUTSIDE OF HIS RIGHT FOOT. THE GROUND IS UNEVEN AROUND THE BOTTOM STEP.

CONTRACTOR HAD REMOVED FASTENERS ATTACHING SET OF STEPS TO CONCRETE WALL. THE STEPS WERE RESET AGAINST WALL AFTER WORK WAS COMPLETED, W/O REATTACHING TO FASTENERS. MAN DESCENDED STEPS, THE SLI D OUT FROM UNDER EE. EE FELL AND ROLLED LEFT FOOT OVER. EE WAS TAKEN FOR TREAT MENT ON 10-27-01 AFTER PAIN CONTINUED. XRAY REVEALED BROKEN BONE IN FOOT. EE'S FOOT WAS WRAPPED & A SUPPORT SHOE

EE WAS KILLING WASP WITH SPRAY UNDER TABLE ON BED OF BOOM TRUCK. HE STEPPED BACKWARDS AND TURNEDRIGHT AT THE SAME TIME. HE STEPPED INTO A HOLE, LOST FOOTING AND WENT DOWN ON RIGHT SIDE.

DOZERS WERE PARKED SIDE BY SIDE. EMPLOYEE STEPPED ONTO OTHER DOZER TO GET EARPLUGS THEN STEPPED BACK ONTO HIS DOZER. REACHED FOR HAND RAIL AND FELT MUSCLE PULL.

EE WAS TO CHECK COOLANT AND ENGINE OIL ON A 627E PAN. AFTER EE HAD COMPLETED BOTH CHECKS HE WENT TO DISMOUNT THE MACHINE ON THE RIGHT SIDE. THE STEP HAD BEEN PUSHED INTO A LOCKED UNSAFE POSITI ON. EE SOMMITTED TO STEP DOWN AND WITH NO STEP TO SUPPORT HIS BODY WEIGHT, HE WAS SUSPENDED BY HIS RIGHT ARM & SHOULDER THUS PULLING HIS RIGHT SHOULDER.

WALKING DOWN STAIRS, STEPPED DOWN ON BOTTOM STEP AND TWISTED LEFT ANKLE.

ON A SNOWY MORNING, EE WAS ASSISTING A CO-WORKER WITH THE UNLOADING OF A FLATBED TRAILER. HE STEPPED ON A BOARD WHILE ON THE TRAILER AND IT MOVED ALLOWING HIS LEFT LEG TO FALL THROUGH AND HE L ANDED ON A BEAM AS A RESULT. THE FOREMAN ASKED HIM IF HE WAS OK AND HE SAID HE WAS. HOWEVER HE CAME TO OFFICE ON 2/7/00 & REPORTED LOW BACK PAIN. WHEN SEEN BY DR ON 2/10 DIAGNOSIS-SACROILIITI

EE WAS DISMOUNTING D8-L TRACTOR #084. HE STEPPED FROM COMPARTMENT PLATFORM ONTO TRACK AND LOST FOOTING. EE FELL TO GROUND CAUSING A FRACTURE TO LEFT WRIST.
