EE WAS WALKING BETWEEN KILN AND MILL BUILDING WHEN HE SLIPPED ON ICE, TWISTING HIS KNEE. DID NOT BECOME REPORTABLE UNTIL 4-7-04, WHEN HE STARTED TO MISS WORK DUE TO SURGERY.

EE WAS WALKING ACROSS A GRAVLED PARKING LOT WHEN SHE STEPPED IN A HOLE AND TWISTED HER KNEE.
INJURED WAS DESCENDING FROM QUARRY HAUL TRUCK WHEN HE SLIPPED STRAINING HIS BACK. DID NOT SEEKMEDICAL ATTENTION UNTIL 11-19-01, AT WHICH TIME HE WAS DISABLED BYU COMPANY DOCTOR.
INJURED WAS INVOLVED IN A BI-ANNUAL MSHA INSPECTION IN THE BASEMENT OF THE PLANT PUMPHOUSE WHEN HE SLIPPED ON MUD TWISTING HIS KNEE. AS HE ATTEMPTED TO CATCH HIMSELF, HE ALSO STRAINED HIS SHOU LDER. DID NOT BECOME REPORTABLE UNTIL HE STARTEDMISSING WORK DUE TO SURGERY ON 12/21/01. HAS RETURNED TO WORK, BUT IS SCHEDULED FOR KNEE SURGERYON 1/18/02.
INJURED SLIPPED ON GREASE SPOT ON FLOOR OF AUTOMOTIVE SHOP. DID NOT BECOME REPORTABLE UNTIL INJURED BEGAN TO MISS WORK ON 7-18-01 DUE TO SRUGERY.
EE WAS PATCHING CEMENT LINE ON TOP OF CEMENT SILOS. WHEN DESCENDING SCAFFOLDING, HE JUMPED DOWN THE LAST 3-4 FEET CATCHING THE HEEL OF HIS SAFETY SHOE ON A ROD LYING NEXT TO THE SCAFFOLDING. H E FELL HITTING HIS ELBOW ON THE ROOF. DID NOT BECOME REPORTABLE UNTIL HE STARTED MISSING WORK DUE TO SURGERY ON 10/26/01.
INJURED WAS PERFORMING PRE-OPERATIONAL CHECK OF FRONT END LOADER WHEN HE SLIPPED ON ICE FREEZING RAIN AND TWISTED HIS KNEE. DID NOT BECOME REPORTABLE UNTIL 8-22-01 WHEN HE STARTED TO MISS WORK DUE TO SURGERY.
THREE CABLES WERE STRUNG ABOVE THE CENTERLINE OF THE KILN TO SUPPORT SCAFFOLD BOARDS USED AS A WORK PLATFORM. THE CENTER CABLE FELL WHEN THE ATTACHMENT BROKE LOOSE FROM THE KILN SHELL. INJURED WAS ON THE SCAFFOLD WEARING A SAFETY HARNESS, BUT HAD NOT TIED IT OFF. AS ANOTHER EE ACCESSED THE SCAFFOLD IT FELL APPROXIMATELY 9 FEET.
INJURED WAS WALKING TO PREP ROOM WHEN HE TWISTED KNEE BECAUSE OF UNEVEN GROUND. HE HAD PREVIOUSLY INJURED SAME KNEE WHEN HE TWISTED IT MOVING THE RAIL UNLOADING RAMP. DID NOT BECOME REPORTAB LE UNTIL HE STARTED LOSING TIME DUE TO SURGERY ON 8/3/00.
INJURED SLIPPED ON BOTTOM STEP WHILE DESCENDING LADDER ON FRONT END LOADER. WAS WORKING OUTSIDE AND IT WAS RAINING. DID NOT SEEK MEDICAL TREATMENT UNTIL 3-20-00. WAS DISABLED BY DOCTOR ON THAT DATE.
INJURED AND CO-WORKER WERE IN THE PROCESS OF REMOVING OLD BAGS FROM DUST COLLECTOR. AFTER INJURED REMOVED RIGGING STRAPS FROM DISPOSAL HOPPER, HE TURNED TO PICK UP OLD BAGS, STEPPED ON A SECTI ON OF NEW SIDING FOR THE DUST COLLECTOR, SLIPPEDON ICE AND FELL CATCHING HIMSELF WITH HIS LEFT HAND.
