At 11:20pm, a non-conforming item went through high speed shredders at Green America facility which resulted in a fire. Contacted MSHA at approximately 1130pm to notify them of the incident. Fire department arrived on site at 12am to assist with putting out the fire.

Operator saw flames on conveyor heading towards GAR bunker and radioed and contacted control room to shut down system, while operator pulled estop cord. Fire department was contacted immediately. Arrived on sight at approx.02:15 am. Plant fire suppression system went off, fire department stayed on sight till approx. 04:15 am to make sure fire was extinguished.

Employee was initially injured on 7/8 at 0830am as EE tweaked EE's shoulder. This was initially treated as a first aid for a strained left shoulder with no work restrictions. Based on radiation of symptoms to EE's right upper extremity and lack of pain improvement, an MRI was ordered which resulted in the reclassification from first aid to recordable was made on 8/6.

There was a fire in the GAR facility. The employee was fighting the fire and suffered smoke inhalation. The employee went to the hospital and received Vitamin B IV. They were just getting over Pneumonia. The employee was released from the hospital after 2 hours.

Employee was shoveling and wheel barrowing material when EE felt a pull in lower back.

Employee was cleaning out the inlet duct in the raw mill. Miner stepped on the incline side of the chute and lost footing on the loose material falling on their left elbow.

Miner caught R thumb between bridle of rigging chain and storage point. Medical care was provided and prescriptions written.

Miner was lifting empty barrels which had been crushed to load into loader bucket.

Miner was walking 208 Belt line, felt a twinge in right knee.

Miner was prying cable from under dock roller, bar slipped and struck miner in EE's mouth

Supervisor was observing cleaning the CKG Bin. Supervisor stepped back to allow work to continue and was struck by an air blast from the exhaust of Jack Hammer. Air and dust particles went under Safety Glasses and were blown into the supervisors eye.

At ~ 6:35 an employee reporting to work saying there was coal coming out of the stack. Around 10:00 PM CO sensors were found offline, at about 10:45 a smoldering fire was discovered, and Buffalo VFD was called.

Tertiary air duct was being cleaned. Miner placed a jackhammer bit against equipment and stepped away to cool off. Miner returned to close the door to tertiary and bumped the jackhammer bit knocking it over, striking an air cannon. The air Cannon discharged and blew material out of tertiary, striking the miner. Miner was burnt on Chest, Neck, and Left Arm by flying debris.

Miner was using a handheld pipe threader to thread a 2" pipe, the threader jammed and caused the miner to be pushed vertically. The miner had arm locked in place and resulted in trauma to the Right Shoulder.

Miner was working with crew to clean up clink using a Vac Truck, while sucking up material the hose plugged causing it to collapse. Miner was struck by hose collapsing and swept EE off of EE's feet, causing EE to land on right arm/shoulder.

Employee injured left thumb while changing belts on #46 Blower. Employee's Left Thumb was caught between the Chiv and Belt, system was de-energized.

The explosion vents on the coal mill cyclone triggered and hot coal was pulled into the coal mill dust collectors. The kiln was shut down and the fire department called to wash the coal out of the dust collectors. The membranes on the explosion vents need replaced and the dust collector bags need replaced due to water damage when the coal was washed out of the hopper.

Field crew EE was in process of cutting pipe loose that was to be replaced in clinker processing area. Dust and debris fell from structure and got into right eye. Was wearing face shield and glasses.

EE was bringing the CKD sample to the core building to drop it off. It was raining and EE didn't want it to get wet. As EE went to set it down in the door way it dropped from EE's hand and the dust went into the air and went up under EE's safety glasses causing EE's left eye to be burned.

The initial injury happened in 2019, employee was going into a chute at a belt transfer and as EE was climbing in EE's foot slipped off of the step and EE caught self with EE's arms. EE's shoulder hurt but EE did not want to seek medical attention. EE ended up having surgery on 5-18-20 and returned to work 5-19-20

CO was rising in Baghouse B of the coal system, it dropped down and the alarm was acknowledged by CRO. A few minutes later a loud bang was heard and smoke and dust were seen coming from coal mill. Ignition occurred in baghouse area. No injuries, damage to explosion vents

Employee using a pry bar to move a door on the tertiary damper out from its frame so they could inspect the tamper. It was almost out when the employee gave a final burst of force to free it, the bar slipped and the employee smashed EE's middle finger. 9 stitches in upper finger.

The inlet duct of the coal mill had smoldering material that was ignited from a combination of a blaster firing air into the system and coal dust being present in the system. The blaster forced air into the coal and heat causing an explosion. It has also been discovered a fast acting damper which should have closed automatically did not.

Employee was dismounting a skid steer and as EE placed foot on the bucket and went to stand up EE's ankle twisted and EE went to the ground. EE was dismounting correctly and the bucket was not slick. This was not an MSHA reportable injury when it occurred. Employee had surgery 11/20/19 because ankle felt unstable.

Slip on 6' ladder 2nd step up using small bar to align bolts. EE did not maintain 3-point contact on ladder.

Employee was cleaning material from a tongue door on the 4th floor riser. Employee was using a 3 foot long t-handled bar to push material from the door into the riser. Accident was indoors.

Employee states that as ee was tightening a bolt with a wrench, ee felt sharp pain in left shoulder

Employee was cutting generic gasket material because the seal material would not work. Employee was working in a dusty area with low light and had to cut material on the door

Employee was standing on unsteady footing that rocked when employee was taking down a heavy porta power with shoulder

Employee was pulling a 100 lb. propane bottle onto the elevator, had the bottle tipped at an angle to clear the lip of the threshold when the safety guard employee was using to pull the bottle came off and the bottle fell striking left leg and lacerating thigh. Employee had 8 stitches, a tetanus shot and prophylactic antibiotics prescribed.

Employees left middle and ring fingers were pinched in between a suspended load and stationary structure as the load was being guided into place. Dislodged fingernails on both fingers and fracture tip of left ring finger.

Employee was jackhammering an area of buildup under a conveyor belt take up pulley that was elevated approximately 15-18" above area employee was working. Upon startup of the conveyor a hard piece of cement fell from take up pulley area and struck the injured employee in the hard hat and left shoulder area.

Employee was attempting to unplug 4th stage when material flushed; hitting his chest, which knocked his face shield and hard hat off.

After opening the drop box door on the roller mill, dust flowed onto the floor and over the top of her boots, causing the burns.

Employee was cleaning the kiln riser using the Woma Lance. As he pulled trigger, a pin hole in the barrel allowed the high pressure water to shoot thru ee's glove.

While moving the crank arm roll, the chain hoist came unhooked from roll and swung and hit employee in the leg. Safety hooks were in tack.

Employee twisted his right knee while getting out of the Kubota. He did not report this to his supervisor. On Sept 11, he slipped and fell, where he felt his knee pop again with immediate pain. Case reported and managed through Health Bridge. Employee had surgery on 1/28/2015.

In the quarry, a loader and haul truck backed into each other. The haul truck operator complained of neck and back soreness and was taken to the doctor. He has been authorized to return to work with restrictions.

While operating jackhammer felt a pull in groin area.

Lifted loaded wheel barrow to move and felt "off" Went home at 3:30 and felt feverish and noticed a lump on the left side of groin area.

Climbing down ladder and stepped on rock at bottom and twisted knee.

The Ee was driving haul truck in the quarry. He felt pain in his lower back. He feels this was due to a pothole in the road. On 4/30/12 the Ee ask to visit the doctor and was placed on restricted duty.

Assisting in cleaning up pile of material and felt back pain while twisting.

Slipped on second step. Placed 6' step ladder on material and did not maintain 3 point contact on ladder. While climbing up to give his co-worker a shovel.

Explosion somewhere in our coal mill system. Investigation is on going.

An employee was traveling across a railroad access bridge on a skid steer. About 3/4 way across, the bridge gave way and the employee fell below. The employee received a laceration to the rear of his head that required sutures to close. No time was missed from the accident.

Ee had begun to clean the riser with a water lance, when a large piece of build-up dislodged. As the material fell, some of it struck the end of the lance causing it to hit the Ee's aluminized hood. The Ee's hood flew up striking the Ee on the cheek and causing a laceration. The employee had to have 8 sutures to close the wound which was approx 1" in length.

EE was descending down a fixed ladder on a loader. The EE lost his grip on the ladder and fell to the ground. The EE injured his left foot as a result of the fall. Medical attention was sought and the EE was diagnosed with a fracture to his foot.

Ee was climbing down a ladder. Stepping off the ladder, he slipped on some water and turned left ankle. Ee reported his ankle was sore to his foreman and continued to work the remainder of his shift. The next day his ankle was swollen and he went to the doctor. The doctor noted hairline fracture to his fibula. The employee was placed in a walking cast and put on restricted duty.

While cleaning the preheat tower, a chunk of material broke loose from above. Then material fell below, hitting the Woma gun the employee was using to clean the Preheat Tower and the Woma gun handle hit employee in the lip, causing a laceration.

Ee was inspecting screw conveyor. As ee was putting the lid back on the screw, the lid slipped out of his hand, he lost his balance, and put his hand into hot dust. Some of the hot dust got inside his glove. Thus, the ee was burned.

Ee walked past the shed at River and slipped on gravel. Became MSHA reportable when employee went for follow up visit and was prescribed drugs on 10/21/08.

Employee was holding a wedge while another employee swung a hammer, a sliver of metal hit employee in forearm causing a laceration.

Ee was operating a 40lb rock drill to drill a 1 1/2" hole in boulder which was stuck in the opening of rock hopper. Having difficulty starting the hole Ee reached down to the lower end of the hammer to help guide the drill steel & bit with his leather gloved right hand. When ee applied air to the drill, right middle finger was caught between the drill & the end of hammer.

Employee was cutting a lid off a box which was sitting on a table with a utility knife. While pulling up on the knife, the knife slipped, puncturing his arm.

Injured was standing in front of the open door to 225 feed chute when a baseball size rock was ejected and struck injured on the safety glasses. EE dropped to knees and covered face w/hands. The other ee noticed bleeding and called control room to call ambulance.

Employee was kneeling on 496 belt using a REMA belt buffing disc, which is highly abrasive, to make a groove for belt repair patch to fit in. Disc then gripped belt that was getting hot and sticky from grinding, causing grinder to twist and kick back into knee.

While removing the water lance hose from the spray in the new ga conditioning tower, hot water sprayed back on the chest of employee causing a burn.

Employee reported lower back discomfort from a previous accident which happened on 3/11/07. An apppointment was made on 4/2/07, with the company doctor.

Employee was air lancing hard material out of 225 impactor chute to allow contractors in to do blow bar welding. Material was hard and air lance was a little shorter than needed to perform job. While air lancing, the lance deflected off material and hand went into opening causing a pinch of the finger and the door frame.

Employee was bending over setting a high bay light down in a basket. The weight was approx. 35-40 lbs. He twisted while setting the light down. He felt pain in his back, could not put weight on legs.

EE was in the process of using a breaker bar to remove the transmission drain plug to change the oil. EE stated that after he loosened the drain plug, his back did not feel right.

While preparing to hang the kiln 8 gauge gun, it was temporarily stood up against the handrail. The gun began to slide over and the employee attempted to grab it to keep it from falling on floor. He pinched his index finger between the gun and the top of the kick plate on walk way.

While cleaning riser with woma lance, chunk fell off wall hitting end of lance and driving handle up under chin.

Employee strained back while shoveling off 489 belt.

EE had just finished adding 3 gallons of coolant to the D8N dozer. As he was about to step down to the crawler tracks from the step mounted on the front side of the dozer, EE felt a pain in his back.

While unplugging the tertiary drop box, hot material broke losse and flushed on to employee's left arm. Material was fluid & made its way under his heat protective coat. Burnt his forearm and wrist in five different spots.

Employee was grinding on the bottom of an old MCC to remove rust and dirt so new steel could be welded to unit and the steel dust went around his safety glasses and into his eyes.

Working on the clinker bucket elevator, the hook could not properly latch, so when ee looked under to bolt the guard it slipped out and struck him in the forehead.

Using a torch to patch a hole in the CKD dust line, there was a hole in torch line and the gas got under his welding coat and was ignited by sparks from cutting the dust line.

Employee and a co-worker were preparing for turn around by putting oxygen & acetylene cylinders in place by the coal mill area. When unloading the cylinders from the pickup, employee's finger was pinched between the tailgate and the tank.

While employee was walking out of crusher building door by clay feeder, he slipped and caught himself. He did twist his right knee.

Employee was assisting a coworker installing an inspection door on a cooler fan. Coworker was using a cutting torch to cut out door opening. Injured ee was holding the steel housing and the employee repositioned his hold and came in contact with the flame of the torch. The back of his gloved right hand caught on fire cauing a burn on the top of his hand.

Ee was assissting with the removal of a pull rod eye in the roller mill. A chain fall had been attached to the wrench. The pipe wrench he was using broke stiking him in the hand. His right hand was cut and his right thumb was fractured (minor, does not need a cast). The cut required 8 sutures.

EE WAS REMOVING STRUCTURAL MATERIAL, ENVIRONMENT WAS VERY DUSTY. EE WAS WEARING SAFETY GLASSES AND A FACE SHIELD.

EE parked his vehicle in the maintenance parking lot, stepped out of his truck and fell in a hole. The hole is an access to disabled electrical wiring from the old cement plant. Normally, there is a manhole cover over the hole. Due to plowing snow the cover was missing. The hole was obscured by a recent snowfall that had yet to be plowed.

EMPLOYEE WAS CLEANING AN OIL SPLILL WITH OIL DRY AND HE SLIPPED AND STRUCK HIS FOREHEAD ON A FLANGE ON THE FINISH MILL BALL TRAP. THE INJURY REQUIRED 15 SUTURES.

EE WALKING UNDER AN OVERHEAD WORK AREA (15 FT. ABOVE), NEAR QUITTING TIME. TOOLS WERE PICKED UP BY EMPLOYEES ABOVE, A SMALL PIPE TOOL ACCIDENTLY DROPPED. SMALL PIPE TOOL STRUCK EMPLOYEE ON THE HARDHAT AND PARTIALLY ON SIDE OF NECK. EMPLOYEEWAS SENT TO MEDICAL CARE FACILITY. X-RAYS WERE NEGATIVE. HE WAS ALLOWED TO RETURN TO WORK WITHOUT RESTRICTIONS OR SCHEDULED FOLLOW-UP.

EE WAS ATTEMPTING TO CLOSE THE DOOR ON THE DRILLTEC D40K QUARRY DRILL FROM INSIDE THE CAB WHEN HIS LEFT MIDDLE FINGER GOT SMASHED IN THE DOOR. CO-WORKER WAS OUTSIDE THE DRILL AND PUSHED THE DO OR SHUT. INJURED DID NOT REALIZE THAT HE WAS GOING TO HELP HIM CLOSE THE DOOR AND HAD HIS HAND PLACED IMPROPERLY ON THE DOOR FRAME. THE DOOR ON THE DRILL IS DIFFICULT TO CLOSE.

EE WAS OILING THE #1 SOUTHEAST KILN BEARING WHENSOME CLINKER DUST WAS BLOWN INTO HIS RIGHT EYE. A COUPUS BLOWER WAS BEING USED AT THE TIME TO HELP COOL THE BEARING & MOST LIKELY CONTRIBUTED TO THIS INCIDENT.

EE was standing in the storeroom talking with a coworker. He turned to walk away and twisted his knee. He was in a lot of pain so we sent him for an examination. He was given a prescription, work restrictions, and is currently receiving physical therapy.

EE WAS OPERATING THE CAT 775 HAUL TRUCK AND WAS BEING LOADED BY A MICH. L480. UNKNOWN TO THE LOADER OPERATOR, A LARGE BOULDER WAS BURIED IN THE BUCKET, WHEN THE LOADER OPERATOR DUMPED HIS LOAD THE BOULDER FELL OUT ROCKING THE HAUL TRUCK. THIS WAS THE FIRST LOAD DUMPED INTO THE TRUCK, CAUSING THE IMPACT OF THE BOULDER TO BE GREATER. EE WAS SEEN BY A PHYSICIAN. AND GIVEN TWO PRESCRIP

CEMENT WOULD NOT FLOW FROM A CEMENT SILO. THE EE WAS PROBING A CHECK VALVE WITH A SMALL ROD. WHEN DEBRIS AND CEMENT BEGAN TO FLOW, HE TURNED AWAY, THE CEMENT STRUCK THE SIDE OF HIS HEAD AND WA S DEFLECTED INTO HIS EAR AFTER STRIKING HIS SAFETY GLASSES. EE WAS NOT WEARING SIDE SHIELDS. NEITHER SIDE SHIELDS OR GOGGLES WERE REQUIRED TO PERFORM THIS TASK.

EE WAS WALKING ACROSS THE BARGE LID TO HOOK UP A CABLE SO THE LID COULD BE REMOVED, HE SLIPPED ON THE LID AND FELL 3 FT 2 INCHES LANDING ON THE BARGE DECK. HE LANDED ON HIS BACK AND RIGHT SIDE . THERE WAS A SLIGHT MIST FALLING AND EE HAD TO WALK THROUGH MUD TO GET ON THE BARGE. HE WAS TRANSPORTED TO HOSPUTAL WHERE HE RECEIVED PAIN MEDICATION PLACED ON RESTRICTIONS.

THE EE WAS CLIMBING OFF OF THE TEREX TRUCK. AS HE WAS FINISHING HIS DECENT, ONE LEG LEFT ON THE TRUCK'S LADDER AND ONE LEG RIGHT WAS ONT HE GROUND. THE FOOT THAT WAS ON THE GROUND WAS PLACED O N A SMALL ROCK CAUSING THE EE TO HIS BODY, INJURING HIS KNEE. THIS INJURY WAS CLASSIFIED AS A FIRST AID INJURY. THE EE IS NOW RECEIVING PHYSICAL THERAPY.

THE EE WAS HELPING TO INSTALL A CURVED PROTECTIVE LINER ON THE RAW MILL ROLL YOKE. A HOOKING EYE WAS WELDED ONTO THE YOKE AND A 1 1/2 TON CHAIN HOIST WAS USED TO LIFT THE YOKE. AS THE CHAIN WA S BEING TIGHTENED, THE EYE BROKE CAUSING THE YOKE TO FALL STRIKING THE EE IN THE RIGHT SHIN.

WHILW WALKING THROUGH THE TUNNEL TO CHECK ON THE SUMP PUMP A PIEC OF CONCRETE CEILING BROKE LOOSE HITTING THE EE ON THE HELMET, NECK, AND SHOULDERS. ABOUT 50 LBS OF CONCRETE FELL. THE SIZE OF THE CHUNKS RANGED FROM PEBBLE SIZE TO ABOUT 2 POUNDS.

EE WAS CLEANING ON THE SOUTHWEST FEED SHELF DOOR WHEN HIS SECOND RIGHT HAND FINGER WAS IMPACTED BETWEEN THE WOMA GUN HANDLE & A STRUCTURAL STEEL SUPPORT BEAM.

EMPLOYEE WAS INSPECTING THE 752 DRAG CONVEYOR WHEN THE CHAIN HIT THE LID, PINCHING HIS FINGER. THE LID WAS NOT FASTENED AND THE LOOSE CHAIN HIT THE LID.

WHILE LIFTING ONE END OF WATER SPRAY PUMP BY THE SAND PIPE, OFF THE STUD BOLTS ANOTHER EE PRIED THE OTHER END OFF THE STUD BOLTS. THE PUMP FLIPPED OVER, AND THE STAND PIPE FLANGE THAT I WAS LI FTING WITH SMASHED MY FINGER AGAINST THE NEAR BY WATER RESERVOIR TANK.

EMPLOYEE WAS REINSTALLING A FAN GUEAR ON A HAUL TRUCK. HIS FEET SLIPPED ON THE LADDER AND HIS ARM CAUGHT THE STEEL ON THE FAN GUARD. THERE WAS A LACERATION TO THE RIGHT FOREARM.

EE WAS OILING KILN BEARINGS. WHILE SETTING BUCKET OF OIL ON NUMBER 3 PIER, HE FELT PAIN IN HIS BACK.

EMPLOYEE WAS PULLING OUT WATER SPRAY WHEN HE BACKED INTO CORNER OF STEEL PLATE THAT WATER SPRAY IS ATTACHED TO.

WHILE CHIPPING HOLE THROUGH WALL WITH PNEMATIC CHIPPING HAMMER, THE BIT WENT THROUGH THE WALL AND WEDGED EE'S RT. PINKY FINGER BETWEEN WALL & GUN. THE TRIGGER WAS STUCK ON THE GUN WOULD NOT PULL BACK THROUGH HOLE.

WHILE CLIMBING UP THE LADDER TO STACKER CONTROL CAB, EMPLOYEE HIT HIS HEAD ON A SAFETY CROSS BARAT THE TOP OF THE ACCESS LADDER, JAMMING HIS NECK.

WHILE ON ROUNDS CHECKING THE RECLAIMER EMPLOYEE SAW GARBAGE IN A PILE. THE EMPLOYEE PROCEEDED TO REMOVE THE GARBAGE. IN DOING SO, HE SLIPPED ON THE ROCKS WHILE HE HAD BARBED WIRE IN HIS HAND. THE BARBED WIRE CUT HIS RING FINGER.

EMPLOYEE WAS CHANGING DOORS ON THE BARGE. WHILE LIFTING THE BARGE DOOR EMPLOYEE FELT PAIN IN HIS LOWER BACK.

WHILE PICKING UP REJECTS, EE TURNED THE BOBCAT TO THE RIGHT TO TAKE THEM OUTSIDE. HE MUST OF BACKED UP TOO FAR. THE LEFT REAR SIDE HIT A SUPPORT BEAM AND THREW HIM AROUND IN THE BOBCAT. HE END ED UP WITH STRAIN OF THE NECK AND BACK.

EE WAS CLEANING CMENT SPILL AT THE RIVER SILOS. EE WAS PULLING ON A TARP WHICH WAS UNDER A PILE OF EMENT AND WAS IN THE WAY OF SUPER SUCK HOSE. EE FELT A PAIN IN HIS LOWER BACK.

EE HAD REMOVED COVER TO EE MATERIAL INTO THE 3RD GRATE. WHILE PLACING THE COVER OUT OF THE WAY, EE STEPPED INTO THE ACCESS HOLE.

WHILE CLEANING EE BACKED INTO GRATING AND STRUCK RIGHT FOREARM AND RIB AREA.

WHILE SHOVELING CLINKER COOLER, EE FOUND SHOULDER WAS SORE. NEXT MORNING HE WOKE & FOUND THAT HIS SHOULDER WAS EXTREMELY SORE & WAS HAVING TROUBLE MOVING IT. COULD NOT LIFT HIS ARM ABOVE WAIST & WAS EXPERIENCING PAIN WHEN TRYING TO ROTATE HIS SHOULDER.

EE WAS WALKING IN FROM THE PARKING LOT TO THE PACKHOUSE WAREHOUSE TO START WORK. HE EITHER TRIPPED OR SLIPPED ON GRAVEL PARKING LOT.

EMPLOYEE WAS DROPPING THE MILL LID TO CHECK WATER SPRAY. TOOK NUTS OFF OF BOLTS, WAS HOLDING SOCKETT ON IMPACT WHEN LID SLID DOWN CATCHING HIS INDEX FINGER BETWEEN NUT STIFF BACK ON MILL.

EMPLOYEE WAS CUTTING WITH TORCH, WHEN RAGS CAUGHT ON FIRE UNDER THE TABLE AND BURNED FOOT. A NYLON RAG WAS LAYING UNDER THE WELDING CUTTING TABLE. NOT SEEING THE RAG WHILE CUTTING STEEL GUSSET TS, RAG CAUGHT ON FIRE, STICKING TO HIS WORK BOOT, BURNING HIS LEFT LITTLE TOE AND FOOT.

THE LOADED HAUL TRUCK WAS COMING DOWN HILL WHEN CONTROL WAS LOST, CAUSING THE VEHICLE TO ROLL OVER.

INJURED WAS PICKING UP PAPERS FROM A LOADER OPERATOR. EE STOPPED PICKUP PARALLEL TO LOADER & SLIGHLTY BEHIND LOADER. AFTER EE CLIMBED ONTO LOADER THE OPERATOR SAID THE PICKUP WAS ROLLING. PICK UP WAS TOO CLOSE TO LADDER, SO WHEN THE TRUCK CLEARED THE LADDER EE JUMPED FOR THE BUMPER, SLIPPED OFF AND STRUCK THE GROUND. HE GOT UP AND THEN STOPPED THE SLOW MOVING PICKUP TRUCK.

EMPLOYEE AND A CO-WORKER WERE TO INSTALL & SECURE AN AIR BLASTER ON THE TERTIARY DUCT. THE TWO EMPLOYEES SUSPENDED THE BLASTER 6' ABOVE THE CAT WALK WITH A 3/4 TON CHAIN FALL AND ATTACHED THE SAFETY CABLE. A FOUD INCLE PIPE FLANGE CONNECTION WAS MISALIGNED, EE PUSHED BLASTER BACK TO GET BOLT HOLE ALIGNED. THE BLASTER SLIPPED COMING FORWARD THUSH HITTING EE'S THUMB BETWEEN THE TW

EMPLOYEE WAS CLIMBING UP ON THE KILN PIER TO OIL BEARING. HE SLIPPED AND HIT HIS CHEST ON THE LADDER.

EMPLOYEE WAS IN THE PROCESS OF EMPTYING A "B" SILO.

WHILE USING THE HI-VAC HOSE TO VACUUM UP THE SPILLED CEMENT, THE 8" STOPPED WORKING. EE STARTED TO PICK UP THE BROKEN HOSE, WHEN HIS RIGHT HAND WAS SUCKED INTO THE HOSE. WHILE TRYING TO GET HI S ARM OUT OF THE HOSE, MACHINE WAS TURNED OFF. EE LOOKED AT HIS ARM AND BECAME DIZZY. HE FELL BACKWARDS HITTING HIS HEAD ON 1104 SCREW. EE BLACKED OUT FOR 1-2 SECONDS.

EE HAD HAD PROBLEMS BREATHING DURING HIS OFF TIME ON WEEKEND THAT HAD CONTINUED WHEN HE STARTED HIS WORKDAY. IT DID NO9T IMPROVE AND HE WENT TO ER.

EMPLOYEE PUSHED THE UP BOTTOM ON TEH OVERHEAD HOSIT ABOVE 719 ELEVATOR TO RAISE THE BUCKET CHAIN OFF OF THE 3" SHAFT HOLDING THE BUCKETS FROM FALLING DOWN INTO THE ELEVATOR HOUSING. THE SHAFT SLAMMED FORWARD SMASHING 2ND,3RD AND 4TH FINGERS AGAINST THE HOIST CONTROL BOX WHICH WAS UP AGAINST THE BEARING BASE.

EE WAS LOADING RAIL CARS. WHILE OPENING OR CLOSING TOP DOORS OF A RAIL CAR. EE BUMPED HIS HEAD ON THE CONDUIT ABOVE HIM.

EE WAS WALKING DOWN THE STEPS BEHIND ANOTHER EE WHO WAS CARRYING A 4"CHANNEL 10' LONG ON HIS SHOULDER. EE CARRYING THE CHANNEL STOPPED AND INJURED WALKED INTO THE CHANNEL CUTTING HIS LIP AND K NOCKING 2 TEETH LOOSE.

EMPLOYEE WAS LOWERING THE DRILL MAST IN PREPARATION TO MOVE TO NEXT DRILL SITE. WHEN THE MAST WAS ABOUT 25% DOWN, WELDS LET LOOSE THAT WERE HOLDING MAST SUPPORT PIVOT. THE MAST FREE FELL THE REST OF THE WAY. THE MAST DECK CAME IN CONTACT WITH THE OPOERATORS CAB, LOOSENING IT AND SHAKING UP THE OPERATOR. HEAD CONTUSION AND NOSE LACERATION.

EE WAS IN PROCESS OF REMOVING FOUNDRY SAND CORE BUTTS FROM BETWEEN THE DOZER SIDE FRAME AND THE BLADE PUSH ARM. WHILE SWINGING A 20 LB SLEDGE, SLEDGE GLANCED OFF SIDE FRAME STRIKING THUMB. HAI RLINE FRACTURE TO LEFT THUMB.
