While drilling a backstope with a long hole drill employee noticed a basket in the wire mesh. While re-positioning the drill the wire separated at adjoining seams allowing some small material (rock) in the basket to fall in front of the drill, startling the employee, causing the employee to retreat quickly and in doing so sprained left ankle in the process.

After Jacking up a rail flat car to re-rail. 2 employees began to push car back on the track. 1 employee slipped causing both employees to fall & in the process the employee who slipped, boot contacted the other employee in the upper lip causing a laceration. Seen site medic & referred to ER, sutures were required, full duty release, no days lost, making it a medical reportable.

While preparing to off load a dumpster at trash yard employee slipped & fell on snow/ice in a non-designated travelway while attempting to chock vehicle. Employee was seen by site medic complaining of low back pain & was referred to the ER, X-rays were positive for a tail bone fracture & restrictions were issued.

Employee was operating a jumbo drill and when EE went to dismount the drill using the handrails and steps EE twisted left ankle as EE stepped to the ground. EE continued to work for about 2 hours but pain was persistent so EE contacted supervisor, seen by medic, referred to ER where restrictions were issued for swelling.

Employees low back/right leg had been sore for a period of time, and EE could not specify an event causing the pain. Exiting a mantrip at the start of shift EE re-aggravated the condition, and reported later in shift to the supervisor. EE was seen by the medic and referred to the ER and restricted until further care from a specialist.

A Non Employee (MSHA Inspector) was startled by an approved on-shift blast causing them to lose footing during their retreat and fell injuring right middle finger. They were seen by the site medic and referred to the ER where they were diagnosed with a dislocation to finger and required to wear a splint.

Employee was pulling a jack-leg machine & driver off a newly installed bolt when EE slipped causing EE to fall with the machine. EE's right ribs made contact with D-Handle. Employee reported to supervisor at the end of the shift and was seen by the medic who referred EE to the ER. X-Rays were positive for a non displaced fracture to a rib and restrictions were issued.

While stepping out of a 2.5 yard loader, employee lost balance causing EE to step backwards and the foot bent awkwardly. The following day, the employee was experiencing discomfort and EE was evaluated and diagnosed with a hairline fracture to the 5th metatarsal and issued restrictions.

Employee was walking around a parked loader and tripped and fell striking head on the ground. Employee was seen at the ER and received sutures for two lacerations on forehead. Full work release was issued, no days lost.

Employee states that they slipped in the parking lot while plugging in their personal vehicle block heater prior to their shift starting. EE stated that they caught themselves and did not fall, reported to the medics with lower back pain. Employee was sent to the E.R. for further evaluation and was given restrictions.

Employee was stepping back up on a Development Bolter to retrieve a needed wrench. In the process his left foot slipped off the bottom step of the bolter's access causing the employee to roll his left ankle when his foot contacted the ground. Employee was maintaining three points of contact. Employee was sent to the E.R. for an evaluation, x-rays were positive for a fracture.

Employee was moving/carrying a piece of cribbing & had both hands on it. While walking, he stumbled/tripped and started to fall; he put both hands out on the rib to brace his fall, which resulted in jarring to both wrists. X=Rays were negative. Restrictions were issued.

On 10/2/15, EE was carrying a valve & lost his balance. When he lost his balance, he contacted a handrail & valve he was carrying contacted his side/rib area (left side) resulting in a contusion. Took him to ER, x-rays were negative. Full Release issued. On 10/7/15, he said he "re-aggravated" it & he went to doctor. Restrictions were issued. (10/9/15-1st shift missed)

Employee was cleaning shotcrete off TBM-motor #1, when he stepped forward & twisted his right ankle. Restrictions were issued. 4 days missed. Full release issued on F/U appt on 4/1/2015. He RTW on 4/2/2015.

Employee was bolting with jackleg drill & lost his balance causing him to fall backwards. In the process, he grabbed the drill & got his L. ring finger pinched between the drill head & the leg on the drill resulting in a laceration & fracture to finger tip.

Employee tripped over his own feet while walking. He tried to catch himself on the canopy post and his right hand struck the post. Went to medic on site, then to ER. X-rays resulted in a metatarsal fracture. He was given a full release. On 1/27/2015 Orthopedic doctor issued restrictions~ then it became a Lost Time injury.

Employee was climbing a fixed ladder when he lost his balance, he grabbed the rung to regain his balance and experienced right shoulder pain. Doctor issued restrictions.

Employee strained his right shoulder while grabbing & pulling on water hose. He got sprayed in the face with the water, causing him to spin & fall on his right shoulder. X-rays were negative; Doctor issued restrictions.

Employee was walking at the 65 Sandplant when he slipped & caught himself - he felt discomfort in his back. He went to medic. At this time it was a Report Only. On his days off, he still felt discomfort & employee went to doctor -Phys. Therapy was ordered & follow up appt in 1 week.(1st shift missed was 10/17/2014)

Employee was walking & he stepped on the edge of the concrete & twisted his right ankle. Restrictions were issued.

As employee was stepping down off haul truck, he twisted his right knee. He felt some pain & discomfort. Doctor issued restrictions.

Employee was taking jackleg off bolt deck, took 1 step back & his right ankle "gave out". X-Rays were negative. Restrictions were issued.

Employee lost his footing while dismounting skid steer, slipped & landed on his right elbow. Went to medic, then to ER. X-rays were positive for fracture(bone chip) to tip of elbow. Full release issued.

While hanging pipe, employee slipped & he struck his rib-cage (left side) on edge of bucket causing pain & discomfort. Went to medic on site, then to ER. X-Rays were negative-but restrictions were issued.

While stepping up onto Jumbo drill to start drilling, employee rolled his right ankle in the process. Xrays were positive for fracture. Restrictions were issued.

On 2/25/2014 Employee was walking & slipped on snow, twisting left ankle. Went to medic & Doctor-full release issued. On 3/11/2014, employee went back to doctor/MRI was done & read-revealing an upper hairline fracture to his fibula. Doctor issued restrictions

Employee was walking & fell down striking elbow - causing a laceration which required 2 sutures. Full release issued. No Days Lost.

While using a scaling bar, employee lost his balance - while catching his balance, he strained his upper Right arm. Doctor issued restrictions

Employee was attaching tag line & fell striking shoulder, head & knee. Restrictions issued in which we can work with.

while wetting down muck pile, employee turned to walk out & tripped, started to fall, putting his left hand out to catch himself - resulted in laceration to #2 finger - sutures required. Full release issued & he returned to work. No days lost.

While on surface, walking towards mule, employee slipped - did not fall but he felt discomfort & some pain in his right knee.

Employee was stepping off Kiruna truck, slipped & left foot got caught and he fell to ground landing on his right knee, causing discomfort.

Employee finished drilling a hole, lost his balance, fell & his jackleg contacted his left shoulder.

While moving wire bundle, employee slipped & his left arm was struck by the wire bundle. X-rays were negative. Doctor issued restrictions.

Employee was walking towards a mucker when he allegedly tripped & fell onto his left knee - causing discomfort. Went to doctor - xrays were negative. MRI was ordered.

EE was sitting in the guard house when he had a coughing spell. The coughing spell caused him to fall out the chair and hit his head on the floor.

Employee was moving jackleg drill, when he lost his footing and jackleg drill fell striking his R. wrist against footwall. Went to medic on site. We took to ER for x-rays, which revealed a fracture to his wrist.

Employee was walking across sand base in stope sill when the sand sank, straining employee's left calf muscle.

Employee was walking in slusher stope and he tripped & fell onto left knee, causing discomfort. Went to medic on site. took to ER - xrays/MRI were negative, however doctor issued restrictions, which we could accommodate.

Employee was stepping off tractor & rolled his ankle. Went to medic on site. We took him to ER - x-rays were negative, however, restrictions were issued in which we could not accommodate. This is a LT injury.

While employee was walking UG, he slipped on some rocks and started to fall, he stuck out his R. hand to catch himself and the palm of hand struck a mat, resulting in laceration to palm, which required sutures. Full release issued. No days lost.

Employee was walking in parking lot before his shift & slipped on snow & ice.(parking lot was plowed at the time). He landed on his L. shoulder. Went to medic. His shoulder was dislocated. He was taken to ER for x-rays, which resulted in a fracture to his shoulder cuff.

Employee rolled his R. ankle stepping off step onto sill from alimak nest. he went to medic on site. we took him to ER Room. X-rays were negative. Doctor issued restrictions.

Employee was in the locker room, walking to his locker & slipped on a wet spot on floor, he caught himself & felt a strain in L. shoulder & neck. went to medic, we took him to ER, xrays were negative, Doctor issued restrictions in which we are able to accommodate.

Employee was using a scaling bar to pry a piece of angle iron straight, when the bar slipped he fell forward landing on his hands & knees. He did not see our medic on site. He worked his shift. The next AM (7/10/11)he woke up at home & said he is sore. He was going to see a doctor, not come into work. X-rays resulted in fracture to L. thumb & elbow, sprained R. wrist.

Employee grabbed a bundle of bolts, while walking back he tripped and fell onto his face, which resulted in a laceration to lip requiring 3 sutures & a chipped tooth.

Employee slipped & he stretched out his hand to catch himself & it hit the diamond drill rods, causing a laceration to his left thumb, which required sutures.

Employee was stepping off jumbo platform when he felt a twinge in the middle of his back, causing a strain. He went to medic on site & felt ok. The next shift he worked, but his back was still bothering him. He went Doctor, and restrictions were given to him.

Employee arrived at work on the bus, when he was getting off the bus, his left knee buckled & he felt pain. He reported it to the medics on site. We took him down for xrays, which were negative, but he was given restrictions.

Employee was walking to his truck, and he tripped. He went to catch himself & slammed his left hand (thumb) into the side of the truck. We took him in for x-rays, which resulted in the tip of his thumb being fractured.
