Employee was walking from the scale house to his vehicle. He slipped and fell causing a fracture to the fibula bone on his left leg. The weather conditions was clear 28 degrees with the sun out.

Mostrando los 11Employee was removing unnecessary tools and trash from the back of the service truck. While walking back to the service truck after placing trash in the dumpster, "located next to the shop approximately 16 feet from the back of the truck", he felt a pop in his right knee.
Employee was coming down a ladder from a bin and was still tied off with a harness and lanyard. He slipped on the step and fell. His arm was over the top of the lanyard when he fell. He was cut by the lanyard under his right arm, which required stitches.

Employee was performing repairs on a stockpile truck. As he went to his maintenance truck to get some tools, he slipped on ice and fell on his back. The employee experienced a strained back as a result of the fall.
EMPLOYEE WAS COMING DOWN THE LADDER OF A CAT 773 HAUL TRUCK. HIS HAND MISSED THE HANDRAIL AND HE FELL.
EMPLOYEE CLAIMS HIS KNEE POPPED OUT A YEAR AGO BUT HE GOT IT BACK IN PLACE HIMSELF. HE SAYS IT HAPPENED AT WORK BUT HE DID NOT REPORT IT. SINCE THAT TIME IT HAS GONE OUT SEVERAL TIMES. HE H AD SURGERY ON HIS KNEE 8-22. HE WORKED AT HIS REGULAR JOB UNTIL THE DAY OF SURGERY.
EE'S AHDN SLIPPED OFF OF HANDRAIL OF MOTOR GRADER AND HE FELL TO GROUND, RESULTING IN A LOWER LUMBAR STRAIN OF THE BACK.
EE WAS TESTING ROCK AND WENT TO THROW BUCKET OF ROCK OUT OF LAB WHEN HE LOST HIS BALANCE AND FELL AND HIT HIS HEAD ON THE STEP CAUSING A LACERATION REQUIRING STITCHES.
