The foreman was standing under a newly constructed supported arch. He was using a rod that he pushed between the arch boards to slide a styrofoam pad on the top for additional cushion. The foreman was looking up and tripped, fell backward and struck the back of his head on a four foot piece of track rail. The CAT scan was negative.

The employee was changing batteries on a scoop and needed to climb on top to remove the battery plug. When the employee was climbing off the scoop, he slipped and bumped his right knee. The x-ray was negative, swelling and two days away from work.
WALKING ALONG BELTLINE & STEPPED IN HOLE, FELL TO KNEES AND INJURED RIGHT KNEE
THE AREA ALONG THE NORTH 3 BELT WAS WET AND MUDDY AND THE EMPLOYEE SLIPPED, TRIPPED AND FELL AGAINST THE V-SCRAPER ON THE TAIL-PIECE. HE FELL ON HIS LEFT RIBS AND LEFT KNEE, CAUSING A SMALL LACERATION TO HIS KNEE THAT REQUIRED STITCHES. THE EMPLOYEE RETURNED TO WORK HIS NEXT SCHEDULED SHIFT.
EMPLOYEE WAS WALKING DOWN STEPS TO THE BATHHOUSE & SLIPPED, GRABBED HANDRAIL WITH HIS LEFT HAND, DISLOCATING HIS LEFT SHOULDER. THE X-RAY WAS NEG., PHYSICIAN SET LEFT SHOULDER. EE RETURNED TO WORK HIS NEXT SCHEDULED SHIFT, ONE WEEK RESTRICTED DUTY.
The ee climbed from the operator's compartment on the roof bolt machine and was not aware of the distance to the ground. He stumbled, fell and struck his left rib on the side of the machine. The x-ray was negative, bruise.
THE EMPLOYEE STEPPED FROM A BELT BED RAIL TO THE MINE FLOOR AND AT THE TIME HE DID NOT FEEL A TWIST OR PAIN IN HIS RIGHT ANKLE. HE FINISHED THAT SHIFT AND HIS FOLLOWING SHIFT. THE SWELLING IN HIS RIGHT ANKLE DID NOT GO DOWN AND HE DECIDED AN EXAMINATION WAS NEEDED. THE X-RAY SHOWED A HAIRLINE FRACTURE IN HIS RIGHT FOOT. HIS FIRST DAY AWAY FROM WORK WAS 7/19/04.
The employee was walking down the slope belt and stepped on a small lump of coal, slipped, twisted his upper right leg hip area. the employee did not return to work his next scheduled shift. strain to upper right leg, two days away from work.
THE EE TOOK HIS METACARPAL GLOVES OFF TO REFILL HIS GLUE BOX. WHILE WALKING AROUND THE BOLTER HE TRIPPED, FELL AND LANDED ON HIS RIGHT PALM. HIS RIGHT PALM STRUCK A SHARP EDGED ROCK, CAUSING A LACERATION. 10 STITCHES NEEDED.
THE SUPERVISOR WAS STANDING ON A 4-FOOT STEP LADDER INSTALLING SIGHTS AND FELL. HIS LOWER BACK LANDED ON HIS W-65 THAT WAS STRAPPED ON HIS BELT. THE X-RAY WAS NEGATIVE, BRUISE TO LOWER BACK AND STRAIN TO NECK. HIS FIRST DAY AWAY FROM WORK WAS 12/10/03.
THE SUPERVISOR WAS WALKING DOWN AN ENTRY AND STEPPED IN A HOLE, TWISTING HIS LEFT KNEE. THE SUPERVISOR COMPLETED HIS SHIFT, BUT NOTICES SWELLING TO HIS KNEE AFTER HIS SHIFT. THE EXAMINATION SHOWED A STRAIN/SPRAIN AND THE SUPERVISOR RETURNED TO WORK HIS NEXT SCHEDULED SHIFT (LIGHT DUTY 5 DAYS).
THE EE WAS CLEANING ROCK OFF THE TOP OF THE MINER & FELL. HIS HIP & LOWER BACK LANDED ON THE W-65 THAT WAS STRAPPED ON HIS BELT. XRAYS NEGATIVE, BRUISE TO HIP & LOWER BACK. HIS FIRST DAY AWAY FROM WORK 11-20-03.
WEST 2 BELT: EMPLOYEE WAS WALKING ALONG THE BELT WITH HIS DINNER BUCKET IN HAND. IN THE AREA OF THE ACCIDENT HE WAS WALKING ON THE 8-INCH WATER LINE AND SLIPPED AND FELL AGAINST AN ACS JACK. T HE X-RAY SHOWED POSSIBLE DAMAGE TO HIS ROTOR CUP IN RIGHT SHOULDER. FOLLOW UP WITH DR. 11/10/03.
THE EE WAS LOADING SUPPLIES ON ROOF BOLT MACHINE. EE WAS CARRYING A BUNDLE OF PLATES& TRIPPED ONTHE BOLTER CABLE. BEFORE HE FELL HE TOSSED THE PLATES AWAY FROM HIS BODY. HE LANDED ON HIS W-65 THAT WAS STRAPPED ON HIS BELT. X-RAY SHOWED A CRACKED RIB & BRUISES TO LEFT SIDE.
THE EE RODE TO THE SECTION IN A RAIL MAN BUS ANDWHEN HE GOT OUT OF THE CAR, TWISTED HIS BACK.
INJURED WAS CARRYING A DRIVE SHAFT AND STEPPED IN A HOLE AND TWISTED LOWER BACK CAUSING PULLED MUSCLE.
WHILE PRE-SHIFTING THE BELT THE EE LEFT FOOT CAUGHT IN THE MUD CAUSING HIM TO FALL. HE FELT PAININ THE BACK OF HIS KNEE. EX-RAYS WERE NEGATIVE AND THE EE RETURNED TO WORK THE NEXT SHIFT. ON 5-5-03 THE EE RE AGGRAVATED THE KNEE IN THE BOAT HOUSE. MRI ADVISED.
