EE was using a cross-over to go make a seal on the examination route. When they stepped off of the cross-over they slipped & cut head on the hand rail of the cross-over. EE received 3 staples to close the wound but returned to work on the next shift. No work was missed due to this incident.

IE was washing down the floor in the #1 drive house on the surface belts. IE stepped off of the stair step onto the concrete floor & felt a pain in lower back. IE did not miss work until 06/13/2018. Dr. hasn't found anything wrong yet but IE hasn't returned to work due to back hurting.

I.E. was unloading vent tubing from a scoop. I.E. tripped over the track rail & landed on the other rail with right knee. I.E. was wearing knee pads. I.E. was taken to the Dr. & only bruising was found. I.E. was released to return to work but stated that I.E. just felt that I.E. couldn't return yet.

EE was walking out of zero gate when the EE twisted the right ankle. EE did not disclose the incident or have incident report filled out until 1/23/18 with complaints of pain to the ankle.

EE was changing a hose on the #29 shield on the longwall face, when foot slipped off a pontoon which caused the EE to twist right knee.

EE was working off of a ladder on the 18 east section. When dismounting the ladder, slipped and fell backwards, hitting lower back on a bucket of plaster located in the area of the ladder. EE finished shift with soreness/tightness of lower back. EE reported to next scheduled shift but requested to follow up with hospital the following morning.

EE was fire-bossing in 0-gate when EE noticed a burning/throbbing feeling in the right lower leg from the top of foot to the knee. EE finished shift and when EE returned for the next scheduled shift, EE reported that EE needed to go to the doctor, that EE could not perform EE's duties as a fire-boss.

EE was operating a weed-eater in the ditch adjacent to the entrance road to the North Portal. While EE was cutting weeds that had grown up in the Rip-Wrap that had been installed for erosion, EE stumbled on a large rock, lost balance and fell on left knee.

EE was walking up the slope belt to change an amp box, when they slipped and twisted their ankle. They continued up the slope, completing the task and walked back down the slope.

EE was washing the chute and had left arm over the chute when the platform EE was standing on broke and EE's left arm got caught on the top of the chute.

Injured and another employee were carrying a shaft out of the belt line to the track. When the injured went through the curtain, tripped over a rail car stop and fell, landing on a rock, causing a contusion to the lower back.

Time is approximate. Injured was washing on the slope belt when the employee fell down the slope causing bruising to the left side of the body.

Injured was stepping over a relay bar and twisted right knee. Injured went to doctor on 6/29/16.

Injured was cleaning mud off of the belt when the belt tensioner engaged causing the injured to strike against belt structure causing pain in the back and hip.

Injured was walking to power center on a continuous mining section and stepped in a hole spraining the right knee.

Employee was walking along the Main North 1 belt line when the employee stepped in a water hole and twisted right knee.

Injured was pulling on a rock dust hole when boot became stuck in the mud causing injured to twist and injure right knee.

Employee was moving belt and tripped over the feeder cable. He twisted and sprained his left knee.

Employee was walking through deep wet rock dust and got stuck. He twisted his knee getting out.

Employee slipped and fell landing on his wrist.

Employee was loading bad rollers onto the slope belt. Employee slipped and fell while carrying a roller. The roller hit the belt structure and then struck the employee resulting in a contusion to the left knee and shin.

Employee was traveling with an MSHA inspector. Employee was attempting to bend a sander bar when the bar slipped causing the employee to slip and fall. The lower left foot was became caught as the injured fell resulting in a broken leg.

Injured was hanging ventilation tubing in the face of #4 entry on the 15 East section. Injured stepped in a hole and suffered an ankle sprain to the left ankle.

Walking to mandoor, employee tripped and hit his chin on the rail.

Employee was tramming a continuous miner and tripped over the miner cable twisting the left knee.

Stepped of the mine ride and twisted his ankle. Went to the hospital and determined that it was a sprain. Later found out that some ligaments were torn and required surgery.

Injured was fire bossing a beltline when they stepped in a hole and suffered a sprain to the left ankle.

Injured has just thrown a track switch and was crossing over the track when they tripped over the haulage rail and fell, straining the left wrist and left knee.

Employee was walking on uneven ground. He slipped and fell twisting his ankle and fracturing a toe.

Employee got foot stuck in the mud. When he leaned forward to pull it out he twisted his right ankle.

Employee was in squatting position and knee gave out when standing up.

Employee was walking down the beltline and stepped in a hole twisting his knee.

Employee was walking across Sump Walkway and tripped cutting his knee.

Employee slipped in mud and tried to catch himself, straining his back.

Employee stepped off motor on uneven ground and twisted his ankle.

While Fire bossing the slope, employee stepped on a rock and fell twisting her right knee.

Employee was stepping out of a manbus when he lost his footing and fell. He hit his right forearm on a door frame cutting it. This required stitches.

While walking down stairs employee misjudged the distance of the last step and fell. Employee has an Inguinal Hernia. Medical evidence shows this may be a re-occurrence of a non work related previous injury.

While rock dusting employee slipped and fell, striking his right elbow on a pipe. Employee went to the Doctor on June 18,2012. Employee did not lose any time but was put on restricted duty.

Employee stepped down after hanging tubing into a hole and twisted his ankle.

EE was walking out of plant when he tripped and struck door with his left shoulder. EE wanted to wait a few days to see if the pain would go away but it didn't, EE was taking to seek treatment on 3-26-12. EE has been on light duty since 7/11. EE has had an MRI done on his left shoulder and seeing another doctor about his rotator cuff.

Employee was cleaning(shoveling), when he slipped and twisted his right knee.

Employee was shoveling under the belt and the area he had to shovel had an old air hose in the way. When he pulled on it the hose came free quicker then he anticipated and he fell catching himself with his left hand. He landed on his left hand causing fractures. This originally was a report only.

Employee fell while walking the slope causing a torn meniscus.

Employee tripped on rock and fell on right knee.

Employee tripped over Timbers and pump parts

Employee was dismounting haul truck on the off operator side (passenger) when he lost his balance when he missed reaching for the handrail. No three point contact was used. The employee then fell to the ground (Approx. six feet) landing on his left foot causing a broken bone in his foot.

Employee stated they were cutting out a canned crib with an axe. Canned crib came loose and lodged, EE tried pushing it over, when pushed it spun & hit a roof jack. Jack became dislodged & struck EE in collarbone area, leg & inner thigh knocking him against rib injuring lower back.

Employee was pulling dust hose back, He was warned to watch out for the structure laying against the rib. He tripped over structure and fell on his back.
