EE was using a cross-over to go make a seal on the examination route. When they stepped off of the cross-over they slipped & cut head on the hand rail of the cross-over. EE received 3 staples to close the wound but returned to work on the next shift. No work was missed due to this incident.

Mostrando 50 de 111IE was washing down the floor in the #1 drive house on the surface belts. IE stepped off of the stair step onto the concrete floor & felt a pain in lower back. IE did not miss work until 06/13/2018. Dr. hasn't found anything wrong yet but IE hasn't returned to work due to back hurting.
I.E. was unloading vent tubing from a scoop. I.E. tripped over the track rail & landed on the other rail with right knee. I.E. was wearing knee pads. I.E. was taken to the Dr. & only bruising was found. I.E. was released to return to work but stated that I.E. just felt that I.E. couldn't return yet.
EE was walking out of zero gate when the EE twisted the right ankle. EE did not disclose the incident or have incident report filled out until 1/23/18 with complaints of pain to the ankle.
EE was changing a hose on the #29 shield on the longwall face, when foot slipped off a pontoon which caused the EE to twist right knee.

EE was operating a weed-eater in the ditch adjacent to the entrance road to the North Portal. While EE was cutting weeds that had grown up in the Rip-Wrap that had been installed for erosion, EE stumbled on a large rock, lost balance and fell on left knee.
EE was walking up the slope belt to change an amp box, when they slipped and twisted their ankle. They continued up the slope, completing the task and walked back down the slope.

Employee was dismounting haul truck on the off operator side (passenger) when he lost his balance when he missed reaching for the handrail. No three point contact was used. The employee then fell to the ground (Approx. six feet) landing on his left foot causing a broken bone in his foot.
Employee stated they were cutting out a canned crib with an axe. Canned crib came loose and lodged, EE tried pushing it over, when pushed it spun & hit a roof jack. Jack became dislodged & struck EE in collarbone area, leg & inner thigh knocking him against rib injuring lower back.
Employee was pulling dust hose back, He was warned to watch out for the structure laying against the rib. He tripped over structure and fell on his back.
