Employee continued to work until he went the Dr. on 1-10-13 when he was diagnosed with a fracture in his (R) foot. He continues to work with modified duty.

1/13/12 Coal River Safety reported to me an alleged injury occurring 1/3/12. EE was terminated 1/6/12 when EE was notified he did not report an injury. EE states to C.R.on 1/13/12 that while unloading top sections of the belt structure, he turned and slipped twisting his left knee. He finished his shift and informed his section supervisor of his incident. No reports made till 1/13.
