EE fell off a trailer while unloading a screw. The accident was noted but no action taken at the time. In December ee went in for a hip replacement surgery that resulted from earlier fall that caused a broken femur.

Mostrando los 12Employee parked haul truck, and was dismounting at the end of shift. Employee missed the bottom rung with right foot, and landed on the ground buckling right knee resulting in a strained right calf.
ITO Non-Mine EE hauling finished products offsite. Before loading EE used the restroom at the office. Using 3 points of contact EE's left foot slipped, EE hit shin on the top front support bracket. Caused 2" Cut just below EE's left knee, 1st aid admitted on site. Driver went to Hospital for 3 stitches and returned continued driving.

Employee was standing on top of skidsteer while lifting a counterweight system into place. He slipped and fell, landing on his back. He initially could do all his duties. He had not been working but felt as his back was getting better. He went to his doctor and his doctor took him off work on 8/28/07.
Employee reported this injury on 6/20/07. He stated that he was grinding inside a hopper for the power screen. He was climbing out when he slipped and caught himself with his hand. He heard a loud snap in his right shoulder. He said it didn't hurt at the time but became increasingly painful and he couldn't raise his arm above his shoulder.
EMPLOYEE WAS STEPPING FROM CABLE DRUM ON DRAGLINE TO THE HOOD WHEN HE SLIPPED & HIT HIS LEFT SHIN ON THE METAL HOOD. EE WOULD NOT FILL OUT PAPER WORK OR FILE A CLAIM UNTIL THE BRUISING FROM THE INJURY WOULD NOT GO AWAY. REPORTED TO SUPERVISOR ON 11/16/05.
Employee stepped out of manlift and slipped. He grabbed on to the conveyor belt to stop himself from falling. He ruptured the tendon in his right bicep.
