EE was putting a piece of steel in the steel dumpster and it caught onto EE's glove, causing EE's right ring finger to get smashed between 2 pieces of steel.

EE fell off a trailer while unloading a screw. The accident was noted but no action taken at the time. In December ee went in for a hip replacement surgery that resulted from earlier fall that caused a broken femur.

EMPLOYEE WAS MANUALLY MOVING ROCK THE SIZE OF A BASKETBALL FROM ATOP A CRUSHER WITH GLOVED HANDS. WHILE PICKING UP THE ROCK TO THROW TOWARDS CRUSHER HOPPER, ROCK SHIFTED AND PINCHED LEFT INDEX FINGER BETWEEN ROCK AND HINGE. RESULTS WAS BROKEN LEFT FINGER TIP AND 5 STITCHES ON FINGER.

Worker was repairing a sand screw. Employee was with another worker pulling a steel flight back into place with a come-a-long when the steel that the come-a-long was hooked to broke and flew back cutting the employees lower left jaw area. employee required 8 stitches.

EE injured left shoulder by propping himself up by his elbows on armrests in loader to avoid bumpy conditions.

While opening the tailgate to a F250 pickup truck, the employee felt a "pop" in his right arm - biceps tendon.

EE was switching a flop gate, the wrench he was using slipped off. EE fell and caught himself with his left hand resulting in a injured shoulder.

Employee was standing on top of skidsteer while lifting a counterweight system into place. He slipped and fell, landing on his back. He initially could do all his duties. He had not been working but felt as his back was getting better. He went to his doctor and his doctor took him off work on 8/28/07.

Employee stepped out of manlift and slipped. He grabbed on to the conveyor belt to stop himself from falling. He ruptured the tendon in his right bicep.

While putting on additional wraps to the timber head, a sudden release of tension on the rope occurred. This caused injury to both hands and left arm.

WHILE GRINDING OFF OLD CLIPS, GRINDER SLIPPED & CUT FINGER.
